Registration<br />
<strong>Conference</strong> registration includes admission to all conference sessions, as well as entrance to the Exhibit Hall and Exhibit Hall events.<br />
Hands-On courses require separate registration. Each registrant will also receive a <strong>Conference</strong> Program on-site, an attendee bag<br />
and a <strong>Conference</strong> At A Glance. Your name badge is your passport to all conference activities and must be worn at all times.<br />
Online registration for the conference will close at midnight Friday, February 22nd, <strong>2013</strong> in preparation for opening at the Vancouver<br />
Convention Centre. On-site registration will be available at the times listed below, beginning Tuesday, <strong>March</strong> 5th.<br />
If registering by mail, we will accept the enclosed registration form if received in the <strong>Pacific</strong> <strong>Dental</strong> <strong>Conference</strong> office by 4pm Friday,<br />
February 22nd <strong>2013</strong>. (<strong>Pacific</strong> <strong>Dental</strong> <strong>Conference</strong> 305–1505 West 2nd Avenue, Vancouver, BC V6H 3Y4)<br />
The applicable fees will be charged as of the date the form is received by our office. Please note: All registration confirmations are sent by<br />
email. To receive a confirmation, please include an email address on your form.<br />
Name badges for Canadian and US attendees will be mailed to all those registered by Friday, February 17th, <strong>2013</strong>. For registrations received<br />
after February 17th, <strong>2013</strong>, badges must be picked up at the registration area at the Vancouver Convention Centre, West Building.<br />
For international registrations, badges must be picked up on-site (badges will not be mailed out).<br />
On-site Registration will be available on Level One of the Vancouver Convention Centre, West Building during the following hours:<br />
Tuesday, <strong>March</strong> 5: 2:00pm to 6:00pm<br />
Wednesday, <strong>March</strong> 6: 7:00am to 7:00pm<br />
Thursday, <strong>March</strong> 7: 7:30am to 6:00pm<br />
Friday, <strong>March</strong> 8: 7:30am to 6:00pm<br />
Saturday, <strong>March</strong> 9: 7:30am to 4:00pm<br />

Continuing Education Information<br />
It is the responsibility of each individual to submit their own report to their provincial authority.<br />
Attendees Who Require Record of Attendance<br />
Registrants should consult with their regulatory authorities for CE credit requirements prior to attending the conference. It is the responsibility<br />
of each individual requiring a conference CE report to be scanned in and out of every session to ensure accurate reporting for CE credits.<br />
For those attendees who scan, individual CE attendance reports can be downloaded from the PDC website after the conference.<br />
British Columbia Registrants<br />
For Dentists and Certified <strong>Dental</strong> Assistants (CDAs) must submit their CE credits to the College of <strong>Dental</strong> Surgeons of BC.<br />
CE credits may be accrued as follows:<br />
1. Hour-for-hour credit, to a maximum of 5 hours, for general attendance at the conference/exhibits<br />
and<br />
2. Hour-for-hour credit, based on individual session attendance.<br />
Delegates can submit credits through the CDSBC website www.cdsbc.org, email completed CE form to ce@cdsbc.org, fax the form<br />
to 604-736-3843 or toll-free 1-866-734-9448 or mail the form directly to CDSBC.<br />
Some sessions may not be eligible for CE and will be identified under the session write-up in the final <strong>Conference</strong> Program.<br />
Note: You must attend the conference to be able to receive credits.<br />
BC <strong>Dental</strong> Hygienists receive hour-for-hour credit, to a maximum of 5 hours, for general attendance at the conference/exhibits and<br />
hour-for-hour credit for individually attended courses.<br />
Alberta & Saskatchewan Registrants<br />
Reports of attendance will be forwarded to the Alberta <strong>Dental</strong> Association and College, and the College of <strong>Dental</strong> Surgeons of Saskatchewan<br />
only.<br />
Alberta Registered <strong>Dental</strong> Hygienists must submit their conference attendance information directly to the CRDHA within 120 days.<br />
Continuing Competence Program credits are determined by the CRDHA Competence Committee. Some courses may not be eligible for credit.<br />
All other delegates must submit CE reports to their own licensing boards.<br />
