Other rare entities which might present with peritoneal carcinnomatosis are the peritoneal mesothelioma and sarcomas. Although there are only few protocols comparing CRS ind HIPEC to traditional therapies, surgery in combination with intraperitoneal chemotherapy should be considered a therappeutic option because mortality is high in conventionally treated patients. In GIST there are established medical therapies [11] and CRS in combination with local therapy has therefore to be seen as investigational. All tumors in which CRS and HIPEC might be considered a therapeutic option with good evidence based results are summarized in Table 1. Preoperative Staging and Eligability All patients with known perittoneal carcinomatosis from the above mentioned tumor entities should be considered for CRS + HIPEC. Due to the potential risks of the procedure and the enormous consumpttion of resources, however, this procedure has to be limited to patients in which a benefit might be achieved. When suspecting peritoneal carcinomatosis preoperatively a contrast enhanced computed tomography is useful to predict the probability of an adequate cytoreduction [13]. Especially the tumor volume in small bowel mesentery, proximal jejunum, proximal and distal ileum, the mesentery configurration and the obstruction of bowel segments correlate with the completeness of cytoreduction [14]. MRI and PET seem to be less acurate than the CT [15] but can be selecttively considered in high-risk patients. Laparoscopy is a safe and efficent method for staging. Valle et al did not observe any mortality or trochar port site metastases [16]. Still precauttions have to be taken that in a primary exporation the trochars should be placed in mid-line and the surrounding tissue should be resected at the time of cytoreduction [17]. Although the tumor markers CEA, CA 125 and CA 19-9 were related to the achievemment of optimal CRS and recurrrence-free interval in ovarian cancer [18] and pseudomyxomma peritonei [19] they are not regarded as valid parameters of exclusion or confirmation of a diagnosis due to their low specificity and sensitivity [15]. Gastroscopy and colonoscopy are mandatory in all patients to exclude an involvement of the gastrointestinal tract. After completion of the staging as well as during surgery the size of tumor nodules and the localization of the nodules on the peritoneal surface should be described using the peritonneal cancer index (PCI) [20]. In colorectal cancer a PCI > 20 is associated with a significantly worse outcome [21] and those patients should therefore be excluded from surgery. A worse outcome has also been shown for lymph node positive tumors [22] and in patients with more than 3 liver metastases [23]. عند الشك بوجود اأورام الكارسينوما يف الربتيوان قبل اجلراحة فاإنه يجب اإجراء تصوير طبقي حموري الظليل وذلك لتحديد احتمال اإجراء استئصال جراحي للخاليا )13(. خاصة حجم الورم يف االأ معاء الدقيقة,‏ الصائم القريب,‏ الدقاق البعيد والقريب,‏ ‏شكل املساريق واانسداد اأجزاء من االأ معاء ترتبط جميعها باإمتام اجلراحة املخفضة للخاليا )14(. اإن MRI و PET اأقل دقة من )15(. CT ولكن ميكن اختيارها بشكل انتقائي يف املرضى ذوو اخلطورة العالية.‏ اإن تنظري البطن هو وسيلة هامة واآمنة للتقييم املرحلي.‏ مل يالحظ فاىل وزمالئه وجود اأي انتشارات ورمية اإىل مدخل التنظري اأو حدوث وفيات )16(. ولكن يجب االإ نتباه عند االستكشاف البدئي يف اأن يكون مدخل التنظري على اخلط الناصف ويجب اأن يتم تسليخ االأ نسجة املجاورة عند اإجراءالعمل اجلراحي الإ نقاص اخلاليا )17( بالرغم من اأن العالمات الورمية CEA, CA 125 and CA 19-9 تتعلق بدرجة احلصول على جراحة منقصة للخاليا الورمية مثالية وفرتات بدون حصول نكس يف ‏رسطان املبيض )18( والورم املخاطي يف الربيتوان )19(. لكن هذه العالمات ليست معايري نقي اأو تاأكيد التشخيص نظراً‏ لكون Fig. 4: Intraoperative situs after extensive peritonectomy (Sugarbaker Operation). الشكل 4: احلالة اأثناء اجلراحة بعد استئصال بريتوان مكثف ‏)عملية ‏سوكر بيكر(.‏ نوعيتها وحساسيتها )15(. يجب اإجراء تنظري املعدة وتنظري الكولون لنفي اإصابة اجلهاز الهضمي.‏ بعد اإمتام التقييم املرحلي وخالل اجلراحة يجب وصف العقد الورمية من حيث احلجم والتوضع على ‏سطح الربيتوان باإستعمال مشعر ‏رسطان الربيتوان )20(. PCI يف ‏رسطان الكولون واملستقيم يكون < 20 PCI مرتافقاً‏ مع نتائج اأسواأ )21(. ويجب استثناء هوالء من اإجراء العمل اجلراحي.‏ كذلك ‏شوهدت النتائج السيئة لالأ ورام ايجابية العقد اللمفاوية )22(. وكذلك املرضى الذين لديهم اأكرث من 3 انتقاالت اإىل الكبد )23(. يظهر اجلدول )2( العوامل التي ترتافق مع احتمال اإزالة اخلاليا بشكل كامل,‏ على حسب االتفاق باالإ جماع.‏ ويجب اأن تكون متطلبات اإجبارية ل .HIPEC ,CRS 47 47
