b. NAVMC 10964. The applicant completes Part I of NAVMC 10964, Application<br />

for a Government Vehicle Operator’s Permit, Figure A-1, Appendix A. The applicant’s<br />

commanding officer or designated <strong>Official</strong>/Supervisor signs and dates item 17. If the applicant<br />

is a civilian, item 17 is to be signed and dated by his/her responsible superior.<br />

c. NAVMC 10969. Physical Examination for Motor Vehicle Operators, Figure A-3,<br />

Appendix A, and NAVMC 10970, Medical Examiner’s Certificate for Drivers, Figure A-<strong>15</strong>,<br />

Appendix A has been completed and signed by a medical officer/Independent Duty <strong>Corps</strong>man.<br />

d. Training. The <strong>Base</strong> Installation will provide the applicant with training on<br />

specifics on hazardous material and explosive handling. The applicant must complete the<br />

required operator’s course for the initial training and refresher course every 2 years for<br />

recertification, refer to page 1-2 section 1-5 for references. After completion of training, the<br />

applicant will bring the certificate to the unit licensing office to be maintained in the operator’s<br />

history folder.<br />

e. OF 346. Annotate the Endorsement page of the OF 346 with the new<br />

qualification(s), Figures A-7 and A-8, Appendix A.<br />

3-8 AMMUNITION AND EXPLOSIVE LICENSE<br />

a. OF 346. Applicant submits current OF 346.<br />

b. NAVMC 10964. The applicant completes Part I of NAVMC 10964, Application<br />

for a Government Vehicle Operator’s Permit, Figure A-1, Appendix A. The applicant’s<br />

d. OF 346. Annotate the Endorsement page of the OF 346 with the new<br />

qualification(s), Figures A-8, Appendix A.<br />

NOTE:<br />

Due to the 2-year recertification requirement for hazardous materials and<br />

ammunition/explosive licenses, the endorsement page of the OF 346 must<br />

contain an entry with the type of endorsement with an expiration date 2 years<br />

from the date issue (ddmmmyy).<br />
