unit: Unit, and address of the Licensing Office
Height: Height of operator, in inches, as shown on ID card
Weight: Current weight of operator
Hair color: Operator’s hair color as shown on valid state operators permit (if
applicable)
Eye color: Operator’s eye color as shown on valid state operators
permit (if applicable)
Date issued: Date license issued (ddmmmyy)
Date expires: Date licensing official determines (ddmmmyy)
Signature/Title: Payroll signature and title of Issuing Officer and unit licensing
code
Restrictions: Type in all restrictions to operator. Also, the following
will be placed in this block: “VOID PAGE IF LAMINATED”,
Card #, and “Duplicate” if applicable. Ammo and fuel
endorsements will be placed on a separate endorsement page 2 of
the OF-346.
Type of vehicle: Under TYPES of vehicle and/or Equipment, 1 st line, type
“ENDORSEMENT PAGE ONLY”, 2 ND line type *HAZARDOUS
MATERIAL and Licensing Code from official who upgrades the
license, 3rd line type, MT 12-HR OPERATOR COURSE.
Capacity: For Hazardous material endorsement page, 1 st line, type N/A
adjacent endorsement page only, 2 nd line type “FUEL” and or
“AMMO”.
Qualifying Official: Payroll signature of licensing examiner for AMMO or HAZMAT
entries in the Capacity block.
Other records: 1 st line, type, *EXPLOSIVE DRIVER-MUST HOLD A
CURRENT MEDICAL CERTIFICATE and/or *HAZMAT
DRIVER-MUST HOLD A CURRENT MEDICAL
