Signature of operator: Payroll signature of operator<br />

Date of birth of operator: ddmmmyy<br />

Social Security Number: Last 4 as shown on operator’s ID card<br />

Name/location of issuing<br />

unit: Unit, and address of the <strong>Licensing</strong> Office<br />

Height: Height of operator, in inches, as shown on ID card<br />

Weight: Current weight of operator<br />

Hair color: Operator’s hair color as shown on valid state operator’s<br />

permit (if applicable)<br />

Eye color: Operator’s eye color as shown on valid state operator’s<br />

permit (if applicable)<br />

Date issued: Date license issued (ddmmmyy)<br />

Date expires: Date licensing official determines (ddmmmyy)<br />

Signature/Title: Payroll signature and title of Issuing Officer and unit licensing<br />

code<br />

Restrictions: List any and all restrictions to operator. Also, the following<br />

will be placed in this block: “VOID PAGE IF LAMINATED”,<br />

Card #,. Place “Duplicate”, “Corrective Lens Required”, “Hearing<br />

Aid Required”, “Learners Permit Must Be Accompanied by a<br />

Licensed Driver”, Shop Use Only, Card #, and Page 2 of 3 entries<br />

(if applicable). If the operator is a graduate of the Military<br />

Motorcycle Maintenance Course, his/her license will be restricted<br />

to “**HARD SURFACE ROAD ONLY”. Enter double asterisks<br />

to the left of “Hard Surface Road Only” and Military Motorcycle.<br />

Type of vehicle: Enter TYPES of vehicle qualified to operate and licensing code<br />

from <strong>Official</strong> who originally trained the operator<br />
