1. An amended agreement(s) between the Albany-Schoharie-Schenectady-Saratoga<br />
<strong>BOCES</strong> and the following <strong>BOCES</strong>. The agreement(s) more accurately reflect the<br />
services provided to this <strong>BOCES</strong> during the 2011-12 School Year.<br />

2. An extension to an agreement between the Albany-Schoharie-Schenectady-Saratoga<br />
<strong>BOCES</strong> and Apple, Inc., 1 Infinite Loop, MS90-2CM, Cupertino, California 95014.<br />
<strong>BOCES</strong> will purchase items/applications, related software and networking devices,<br />
excluding iPads and iPhones not available for purchase under NYS Contract PT65428.<br />
The dollar amount is unspecified at this time. The contract period is July 19, 2012<br />
through July 18, 2013. (2012-003e-CA)<br />
3. A first amendment to the lease agreement between the Albany-Schoharie-Schenectady-<br />
Saratoga <strong>BOCES</strong> and Clark Industrial Park, LLC, 4 Walker Way, Albany, New York<br />
12205. <strong>Capital</strong> <strong>Region</strong> <strong>BOCES</strong> will continue to lease warehouse space at 25 Walker Way,<br />
Albany, New York. The annual rent is $44,550. The first amendment to the lease is for<br />
the period July 1, 2012 through June 30, 2014 and is contingent upon approval of the State<br />

4. An agreement between the Albany-Schoharie-Schenectady-Saratoga <strong>BOCES</strong> and<br />
Blue Shield of Northeastern New York, 30 Century Hill Drive,Latham, New York<br />
12110. Blue Shield will provide participating <strong>BOCES</strong> employees with health insurance<br />
for the period July 1, 2012 through June 30, 2013. Rates are as follows (2012-132-CA):<br />

<strong>Capital</strong> District Physicians Health Plan (CDPHP), 500 Patroon Creek Blvd, Albany,<br />
New York 12206. <strong>Capital</strong> District Physicians Health Plan will provide participating<br />
<strong>BOCES</strong> employees with specific health insurance for the period July 1, 2012 to June<br />
30, 2013. Rates are as follows (2012-133-CA):<br />

employees with vision care coverage for the period July 1, 2012 through June 30,<br />
2013. The renewal rate is the same rate used for 2011-12. Rates are as follows:<br />
