6. Insure proper and complete preparation ofall cards, tags, and badges.c. Process applications in accordance with theseregulations for those persons who can report inperson to place of issue.d. Prescribe the procedure for issuance for thosepersons who cannot report in person to place ofissue. "e.' Insure that the reverse side of DD Form 2A(green), and DD Form 489, issued to protectedpersonnel, as defined in paragraph 3, is overstampedby an ordinary rubber stamp or overprintedin the form of a cross made up of a doubleline, each line to be not over one thirty-second ofan inch in width with approximately one thirtysecondof an inch between the lines. The overallheight and width of the cross will be one and onehalfinches. Impressions will be made with red,permanent, waterproof type ink. In the center ofthe stamp the word "CHAPLAIN" or "MEDIC"will be clearly reproduced. Caution will be takennot to obscure the fingerprint portion of the card.DD Form 528 will be overstamped in a like manneron the front./. Uniform withouthead covering will be worn by military personnel.Uniform is not required for retired militarypersonnel.e. Paper. The final prints will be made onsingle weight paper. Any heavier weight paperwill interfere with proper lamination of the completedidentification card./. Processing negatives and prints. When normalprocedures are used in negative development,the prints should be printed slightly darker thannormally required since the lamination processtends to lighten the print. When fast developmentmethods are used, necessary adjustments willbe made to obtain the same results.12. Fingerprinting, a. Fingerprints necessaryfor the preparation of identification cards will bemade in accordance with the instructions containedin TM 11-403. However, only the fingerprints ofthe left and right index fingers are used. Caremust be exercised to insure clear legible impressions.In the event that an individual has eitheror both of those fingers missing, a notation will bemade in the space provided, and prints of the nextfinger will be substituted.fi. Prior to lamination, the fingerprints on thecard will be checked for legibility. If the imprintsappear smudged or spotted because of temporaryscar tissue, it is advisable, if time permits,for the individual to return at a later date (within3 weeks) for fingerprinting. If time does not permit,notation will be made on the card to the effectthat suitable fingerprints could not be made at thattime, and request will be made for reissue of a cardat a later date when the fingers are in better conditionfor fingerprinting.13. Signatures. On those portions of the cardrequiring signatures, the use of a sharp-pointedpen which might tear or injure the fibers of thepaper insert will be avoided. The extreme pressureand temperature to which the card is subjectedmay cause the paper insert to split or explodeat that point during the lamination process.14. Preparation of paper insert for lamination,a. A small amount of rubber cement orother nonstaining adhesive may be used to affix theprint to the paper insert while assembling thestack of cards for lamination. A simple pastemade of flour and water has been found to besatisfactory, provided a minute amount is used.Care will be taken when assembling the paper insertand plastic sheets to leave a trimming marginof at least one-sixteenth inch on each side. Thelaminated card will not be trimmed too closelysince the use of the corner rounder may break theair seal and render the card useless.&. When placed on the paper insert, the photographwill be centered so that the edges are parallelto the edges of the paper insert.c. Lamination will be in accordance with the instructionscontained in TM 11-2369 and TM 11-2317.15. Accountability. Forms listed in paragraphs4 a, 6, c, d, e, g, and h are accountable, andissuing authorities will maintain a record of eachcard issued and of each card destroyed prior toissue.16. Loss. Loss will be reported promptly onDA Form 428 (Application for IdentificationForm), or DD Form 1172 (Application for Uniformed<strong>Services</strong> Identification and PrivilegeCard) as appropriate, which will serve also as anapplication for issuance of a new card, providedthe individual continues to be otherwise eligibletherefor. DA Form 428 or DD Form 1172 willcontain a statement to include the circumstancessurrounding the loss, action taken to effect recovery,and card number if available.17. Surrender and disposition, a. Cards willbe surrendered to appropriate authority upon—(1) Expiration.(2) Change in status affecting eligibilitytherefor.(3) Replacement by another card, except asprovided in paragraphs 290 and B5d.(4) Cancellation by proper authority forcause.(5) Determination by commander concernedthat card is no longer required.b. Appropriate authorities will establish controlprocedures to insure that military personnelwill not be released from active duty, nor willconnections with civilians be officially terminateduntil the individual has surrendered his identificationcard or has submitted a sworn statement satisfactorilyexplaining the circumstances surroundingits loss.c.
