dependents do not reside with thesponsor, both copies, of the applicationform (with dependency of primary dependentsand privilege eligibility verifiedby the personnel officer) will be forwardedto the principal dependent, whomay obtain DD Form 1173 from thenearest uniformed service installation.In cases concerning dependents otherthan the primary dependents cited above,both copies of the application form willbe forwarded through military channelsto the Commanding General, FinanceCenter, U.S. Army, ATTN: Allotmentand Deposits Operations, Indianapolis49, Indiana, for determination in connectionwith medical care benefits.If the service member declines to or cannot submitan application for an eligible dependent, thedependent, a guardian, a designated agent, themember's commanding officer, or any installationcommander may initiate the application. Verificationof dependency of wife and unmarriedlegitimate children, stepchildren, and adoptedchildren (for whom there are properly certifiedcourt adoption papers) under 21 years of age willbe made by the member's personnel officer. Thepersonnel officer will also determine the privilegesto-which each dependent is entitled, the "effectivedate" of eligibility for medical care in civilianmedical facilities for each dependent, .when authorized,and the date on which eligibility forprivileges will expire. The "effective date" forcivilian medical care will be determined in accordancewith paragraph 44. The application, 1 induplicate, will then be forwarded to the principaldependent who may obtain DD Form 1173 fromthe nearest uniformed service installation. Whenother than primary dependents are involved, theapplication, in duplicate, will be forwardedthrough military channels to the CommandingGeneral, Finance Center, U.S. Army, ATTN:Allotment and Deposits Operations, Indianapolis49, Indiana, for determination of dependency.b.
