AR 606-^5S-BCTION XACCIDENT INVESTIGATOR'S IDENTIFICATION CARD. 63. Scope and purpose. This section providesfor issue of DA Form 1095 to military personneland civilian employees of ;the Department of theArmy. DA Form 1095 will be used for the followingpurposes only:a. By safety personnel who have a substantiverequirement for positive identification when investigatingArmy installations and activities.b. To permit safety personnel freedom of actionin investigating conditions incident to accidents inwhich military or civilian personnel are involved,Government property is damaged, or the Departmentof the Army has an interest.64. Responsibilities. Heads of <strong>Headquarters</strong>,Army staff agencies; Commanding Generals,United States Army Air Defense Command andUnited States Continental Army Command; ZIV Army commanders; Commanding General, Mili-\j tary District of <strong>Washington</strong>, U.S. Army; ando> major oversea commands are responsible for'M determination of the need for and the issue of DA\ Form 1095. Requests for this form will be limited\to the quantity actually required to meet immediateindividual needs. Letter request to include individualjustification for each form requested willbe addressed to The Adjutant General, ATTN:AGAO, Department of the Army, <strong>Washington</strong> 25,D.C.(DA Form 1095)65. Application. Application will be made iiiduplicate on DA Form 428 (Application forIdentification Card). The responsible official willforward both copies of the application with theletter request to The Adjutant General, ATTN:AGAO, Department of the Army, <strong>Washington</strong> 25,D.C. The Ad j utant General will indicate the cardserial number in the appropriate block of bothcopies of the application and return one copy withthe DA Form 1095 to the responsible official. Theresponsible official will have his issuing authorityprepare and issue the card in accordance with paragraph6.6. The recipient of the issued card willcomplete the appropriate blocks on the applicationacknowledging receipt and the date of receipt.This copy of the application will be retained bythe issuing official until surrender and dispositionof the card in accordance with paragraph 17.66. Preparation. Each DA Form 1095 issuedwill contain a full face photograph prepared inaccordance with paragraph ll, name, title of personto whom issued, signature of the recipient,command, fingerprints of right and left index fingers,physical description, the signature of the issuingofficer or a subordinate designated by him,and the date of issue, and will be laminated betweentwo sheets of clear plastic.SECTION XISECURITY IDENTIFICATION CARDS AND BADGES67. Scope and Purpose, a. This section prescribesminimum standards and specifications forsecurity identification cards and badges for identificationof appropriate military personnel, civilianemployees, contractor employees, and visitors,as an aid in controlling the security of Army installationsand activities, and critical and sensitiveareas located thereon. Personnel identificationsystems are established ,for security reasons to—(1) Provide for the control of entry of personnelinto installations arid facilities.(2) Provide a practical system of positiveidentification of personnel who are authorizedto have access to specific restrictedareas.(3) Facilitate the control of circulation ofpersonnel into, within, and out of restrictedareas. ,(4) Provide a visible means of easily recognizingany limitations of movements oraccess of personnel within restrictedareas.TAGO 678A 23

Security identification cards and badges areissued in those instances where a system of personnelidentification and control is required whicr. isin addition to that provided by the standard identificationmedia specified elsewhere in these regr,ulations. Such security cards and badges willbe issued to designated personnel before movementinto, within, arid out of specified installations, activities;,or restricted areas can >be affected.68. -Definitions, a. For the purpose ofthese regulations, an identification badge as referredto herein may be- 1 -'(1) A laminated or embossed identificationbadge with attachments which permit itto be fastened to the clothing or suspended:; from the neck. '(2) A metal photographic identification. .badge.. 69. Responsibilities, a. The decision as to theneed for, and use of security identification cardsor badges, their withdrawal, or their, reissue restswith the commanders of major commands and/orheads of Army staff agencies. . ..-. ., - : -.b. Commanders of major commands and headsof Army staff agencies are responsible for theprocurement (except for DDor DA jforms), preparation,issue, and use of completed identificationcards and badges and for enforcing their policyon the use of such cards and badges as a securitymeasure. Such, responsibility may be delegated toinstallation commanders where appropriate.c. Commanders of major commands and headsof Army staff agencies are responsible for necessarybudgeting and funding for security identificationcards (except for DD and DA forms) andbadges. Such responsibility may be delegated toinstallation commanders where appropriate.d. Commanders of major commands and headsof Army staff agencies may design the format ofthe security identification card or badge to be employedat their installation and activities, providedthat such format meets the specificationsas outlined in paragraph 70. : In the case of metalphotographic identification badges, commandersof major commands and heads of Army staff agencieswill determine the size, color,'shape, and designwithin the criteria established in paragraph.70.e. Photographic services required in preparationof security identification cards and badgeswill ,be provided by existing photographic laboratoriesarid agencies. Where laboratory facilities• are! not available, photographic services will beprocured locally.170. Specifications, a. Security identificationcards arid badges will be of a type or design andconstruction which will assure an acceptable degreeof tamper resistance and which will meet orexceed the specifications listed herein.(1) Security identification cards. Laminatedor embossed identification cfirdsare considered to meet these' specifica-'""•' tions when they incorporate the follow-'••ing features:(a) Contain an intricate background design,difficult to reproduce by normalphotocopying.(b)- Contain an ink or dye on some part ofthe card which is noticeably affected by• erasure or the heat required to alter orrelaminate the card or badge.(c) Identify as to installation or activity.(d) Contain a recent clear photograph ofthe bearer, the size to be not less than 1inch wide fraction arid 1% 6 inch inheight. (If military personnel, indicatename and service number at thebottom of the picture. If civilian personnel,indicate name and category, i.e.-employee, contractor, vendor, etc. at the(e)bottom-of the picture.) Where bothcards and badges are employed, thephotograph should be made from thesame negative.
