AR 606-5(f) In all cases where the claim is based onthe mental or physical incapacity of achild over 21 or an unremarried widower,a statement from a duly licensedphysician or medical officer will be submitted,indicating that the child or unremarriedwidower is mentally orphysically incapacitated and as a resultis incapable of self-support, andthe period of such incapacity.(g) In cases involving unmarried children,adopted children, or stepchildren over21 but under 23 years of age who areenrolled in a full-time course of studyat an institution of higher learning, thename and location of the institutionwill be furnished.Where any of the above information has been previouslyfurnished to the -Finance Center, U.S.Army, in connection with a claim for quarters orother allowances',. reference to the date and placeof 'submission may be made in the "Remarks'" sectionof DD Form 1172 in lieu of furnishing additionalstatements.c. Dependents of persons other than militarypersonnel. Determinations of dependency will bemade within the continental United States by installationcommanders and overseas by major overseacommanders or commanders designated bythem. Questionable cases will be referred to theCommanding General, • Finance Center, U.S.Army, ATTN: Allotment and Deposits Operations,Indianapolis 49, Indiana. Cases so referredwill include all pertinent facts and supportingpapers.d.. Other dependents. Forwarding of determinationcases to the Commanding General FinanceCenter, U.S. Army will not be permitted to delayissuance of D"D Forms 1173 to other dependents inthe same family whose eligibility has been determinedor when the card is to be used for purposesother than medical care. A card may be issued authorizingprivileges other than medical care, pendingdetermination of eligibility for medical care.44. Determination of "effective date" formedical care in civilian medical facilities. The"effective date" for commencement of eligibilityfor medical care in civilian medical facilities,when authorised, will be the latest of the following,as appropriate:a. 7 December 1956 (effective date of DependentsMedical Care Act (10 U.S.C. 1071)); orb. Date of marriage to sponsor; orc. Date of sponsor's last entry on active dutyafter a break in service of more than 24 hours; ord. Date of sponsor's initial entry on continuousactive duty (no break in service of more than 24hours); ore. Preparation of DD FormIi73 will include the following:a. General. Cards will have a photograph preparedin accordance with paragraph 11 and laminatedbetween two sheets of plastic. In the caseof an applicant who cannot report in person to aplace of issue, a partially processed card will beforwarded to the applicant by certified mail. Inno case will a blank card be so forwarded. In. suchcases, issuing authorities will complete all entriesexcept photograph, expiration date, and signaturesof applicant and issuing officer before thecard is mailed to recipient. Instructions will beincluded with the card advising the applicant regardingentering of signature in the proper blockand obtaining an appropriate photograph. Theapplicant may secure a photograph, without cost atmost military installations or may-secure a photographat applicant's own expense. Photographssecured at personal expense must meet the requirementsset forth in paragraph 11. Upon return tothe issuing authority, expiration date, photograph,and issuing officer's signature will be added to thecard, and lamination accomplished (par. 47).b. Geographical limitations on validity. Alimiting entry will be included in item 3, after orbelow the expiration date, on cards issued to personswhose privilege eligibility under applicableregulations is limited to one installation, command,or area (Examples: "30 Jun 65—VALID ONLYFT BELVOIR" or "1 Aug 64—VALID ONLYUSAREUR.")c. Expiration date (item 3).
