Service number. Individuals on the EmergencyOfficers Retired List will have the notation"EORL" entered in lieu of service number.d. Medical care. In the block following the"Date of Issue" block, enter "No MEDICARE"for members retired under title 10, U.S.C. 1331with less than 8 years' active service to indicatelack of entitlement to medical care. This blockwill be left blank in all other cases.SECTION VTUNIFORMED SERVICES IDENTIFICATION AND PRIVILEGE CARD (DD Form 1173)37. Scope. This section outlines the responsibilitiesof commanders and prescribes proceduresfor the issue of DD Form 1173.38. Purpose, a. The DD Form 1173 is used toidentify persons eligible for medical care, commissaryprivileges, exchange patronage, and admissionto military theaters. It does not replace DAForm 1602. (Civilian Identification Card) (sec.VII) for civilians ordered overseas or for personswho require military identification while in atravel status, or for local identification not involvingthe granting of privileges. Installationcommanders will assure that members with dependentsare aware of the importance that theirdependents be in possession of DD Forms 1173for privilege identification purposes. Further,that if identification cards which have been issuedto their dependents are due to expire prior to terminationof eligibility for ,such cards, it is theresponsibility of the member to make applicationfor a reissue prior to the expiration date. Failureof sponsors to make prompt application for initial,issue, or reissue upon expiration, of DD Form1173 may result in their dependents being deprivedof privileges to which they are entitled and, inthe case of medical care, severe hardship may resultwhere application is delayed until after thedependent becomes ill.b. These regulations in no way alter requirementsfor the establishment of eligibility for privileges; such eligibility, and the use of available Vfacilities and privileges, are governed by the fol- ^lowing regulations: &R. *•/ 6 —/ ?- *(1) Medical care. Dependents 10 yearsof age and older (including unremarried widows11
and widowers) who require military identificationfor privilege control or other purposes, except asprovided in section VII. DD Form 1173 will beissued to dependents less than 10 years of age whorequire military identification because of residencewith a guardian, divorced wife, or other personnot eligible for medical care under existing regulations,or who, because of other special circumstances,require military identification. Medicalcare and other privileges will normally be furnishedchildren under 10 years based upon certificationof eligibility made by the sponsor or otherfamily member and identification card or otherdocumentary evidence of the sponsor's or familymember's eligibility therefor.Note. Adoption of the child of an active duty, retired,or deceased member by a third party terminates thechild's eligibility for medical care.b. Totally disabled veterans and their dependentsentitled to commissary or exchange patronage.c. Dependents of members of the Army NationalGuard of the United States and Army Reserveordered to perform full time duty training for aperiod in excess of 30 days (par. 8/).d. Foreign military personnel and their dependents.e. Dependents of integrated officers. When thestatus and service number of an officer change byreason of his integration into the Regular Armyunder the provisions of the Regular Army AugmentationProgram, DD Forms 1173 previouslyissued to his dependents and bearing his formerreserve status and service number will be replacedby new DD Forms 1173 reflecting his altered statusand servige ^number.^/-DDTorms 1173 will not be issued to any personrequiring privilege identification for less than30 days.40. Cross-servicing. The uniformed serviceshave agreed to assist each other in the issue of DDForms 1173. Applicants will obtain verificationof the application form from the applicant's orsponsor's parent service before requesting issue ofthe DD Form 1173 from another service.41. Application. Application will be made onDD Form 1172 (Application for Uniformed <strong>Services</strong>Identification and Privilege Card) in duplicate.The application will list all eligible membersof the family who are 10 years of age or older.A separate application, in duplicate, will be submittedin" those cases where the determination ofdependency will be made by the Commanding Gen- 1eral, Finance Center, U.S. Army (par. 43). Applicationswill be submitted as follows:a. Dependents of military personnel on activeduty.(1) Officers and warrant officers. Applicationwill be submitted to the issuing authority.Applicant's signature will sufficeto determine dependency (par. 435 (1)).Applicant will complete item k, SectionII, "MC(C) Effective Date," for eachdependent eligible for medical care incivilian medical facilities, in accordancewith paragraph 44. The issuing authoritywill determine the privileges to whicheach dependent is entitled, the date onwhich eligibility for these privileges willexpire, and will then issue DD Form1173 for each dependent claimed, unlessobvious error exists in the execution ofDD Form 1172, in which case it will becorrected. If the dependents do not residewith the sponsor, the sponsor willcomplete the application as outlinedabove and forward both copies to theprincipal dependent who may obtain DDForms 1173 from the nearest Army installation.(Application must be verifiedby sponsor's personnel officer if dependentsdesire to obtain DD Forms 1173 fromany other uniformed service installationnear them.)(2) Enlisted personnel. Application will besubmitted to the personnel officer, whowill verify the dependency of primarydependents (wife, unmarried legitimatechildren, stepchildren, and adopted children(for whom there are properlycertified court adoption papers) under21 years of age). The personnel officerwill also determine the privileges towhich each dependent is entitled, the "effectivedate" of eligibility for medicalcare in civilian medical facilities for eachdependent, when authorized, and the dateon which eligibility for privileges willexpire.
