<strong>Aged</strong> <strong>Care</strong> & <strong>Rehabilitation</strong> <strong>Clinical</strong> Service <strong>Plan</strong> <strong>2007</strong> - <strong>2012</strong>CHIEF EXECUTIVE’S MESSAGEThe demands on our health care system are increasing. Over the next ten years it is expectedthat the population in Sydney South West will grow to 1.52 million people. At the same time, thenumber of older people will increase, with almost 200,000 people aged over 65 years andapproximately 25,000 people over 85 years by 2016. The number of people with disabilities willalso increase with the ageing of the population.Most people will remain healthy and active throughout their lives. However, some peopleparticularly as they age, will find it increasingly difficult to manage due to chronic or persistenthealth problems, an injury or a disability, or a combination of these. At that stage, they willrequire additional health care, and may require admission to hospital due to acute or chronichealth problems. They will also find it difficult to manage at home, requiring additionalassistance, care and support. Although this support will be provided by community services,relatives, friends or neighbours may also need to take on an additional role of carer. These arethe clientele of SSWAHS <strong>Aged</strong> <strong>Care</strong> and <strong>Rehabilitation</strong> <strong>Services</strong>.It is important that there is a service such as the SSWAHS <strong>Aged</strong> <strong>Care</strong> and <strong>Rehabilitation</strong><strong>Services</strong> which can work with all parts of the health and community care system, and supportand care for people with complex needs at this vulnerable time and into the future. It is alsoessential that there is a service that can reach people where ever they need help - in hospital, athome, or in a residential care setting.The environment in which SSWAHS <strong>Aged</strong> <strong>Care</strong> and <strong>Rehabilitation</strong> <strong>Services</strong> and related healthservices operate is complex. There are a large and growing number of health and communityservices which can help those with greater frailty or disability, funded through differentgovernment departments and programs. The cultural and linguistic diversity of the SydneySouth West community adds another dimension to this complexity. SSWAHS services and staffneed to be flexible in how and where they deliver their services, knowledgeable about theservices available, respectful in how they work with clients and their carers, and adaptable inaddressing and coordinating care, so that clients and their families have their needsappropriately met.The SSWAHS <strong>Aged</strong> <strong>Care</strong> and <strong>Rehabilitation</strong> <strong>Clinical</strong> <strong>Services</strong> <strong>Plan</strong> provides direction for healthservices to maintain the quality of life of older people with complex needs and people requiringrehabilitation, for the period <strong>2007</strong> - <strong>2012</strong> and beyond. The recommendations are based onextensive consultation, analysis of evidence based practice, and interpretation of the availabledata. The <strong>Plan</strong> is consistent with NSW Health’s vision of delivering high quality servicesresponsive to the needs of health consumers and the community.This <strong>Plan</strong> provides an appropriate balance of structure, foresight, vision and compassion, for thechallenges ahead.
