ANNUAL RAP AND CARE PLAN TIMELINES The resident is assessed annually by the interdisciplinary care team using the RAI-MDS 2.0 FullAssessment within 366 days of the previous Full Assessment, along with RAPs and care plans review andupdates. Annual RAI-MDS 2.0 Assessment and RAP Assessment Summaries can be completed earlier than 366days but not later. Where a significant change in the resident’s health status has resulted in the completion of aninterdisciplinary assessment, then the interdisciplinary assessment may replace the quarterly assessmentfor that quarter.
