HISTOPATHOLOGY>In addition to the general requirements for the examination report, does the histopathology report include atleast the following required information:881. EA description of the gross specimen with a list of all sections submitted and a key to their locations, if appropriate?882. EA narrative description of the morphologic findings, if appropriate?883. EA morphologic diagnosis?884. EThe signature of the reporting pathologist?885. EComments and recommendations as appropriate?886. EIf a computer report is generated with an electronic signature, is it authorized by the individual who performedthe examination and made the diagnosis?The laboratory shall have and must follow policies and procedures for retention and organization of records,reports, slides and blocks.887. EDoes the laboratory retain reports and slides for at least 10 years and tissue blocks for 2 years or as requiredby local, regional, or national regulations?This is the minimum requirement under CLIA. Laboratories are encouraged to retain all material for a minimumof 10 years for pathology.888. EAre the slides and reports stored in orderly, cross-indexed files?889. EDoes the laboratory maintain written acknowledgment of all slides which are loaned to proficiency testing programs?890. EDoes the laboratory maintain records of slides loaned or referred for purposes other than proficiency testingprograms?891.
