CRITERIA FOR QUALITY LABORATORY PERFORMANCE>Post-AnalyticReference to procedural requirements in Analytic Section (APM 15, 18, 19)PST 1 RDoes your laboratory have a written procedure to correct laboratory errors when they are detected?The laboratory policy should define who to notify when an error occurs, how to correct the error, the importanceof maintaining the original and corrected report in case medical decisions or procedures were initiatedbased on the erroneous result. It should also require that each of these items be documented for reviewas part of the quality assessment process.It is important that the laboratory track errors and evaluates the circumstances associated with them, accordingto their established Incident Management Program, for consideration of potential harm to patients. SeeCriteria QA 20.When an error occurs in your laboratory: (PST 2-4)PST 2 RDo you have a written procedure to notify the proper individual of the correction?It is important to notify the person who ordered the test that an error has occurred and, if applicable, anyother individual who may be responsible for seeing that appropriate action is taken, in the absence orunavailability of the requester. This should be part of the laboratory’s policy manualPST 3 RDo you provide a corrected report to the proper individual?The corrected report should be labeled as such and provide copies to both the person who ordered the testand if applicable the individual using the test results.PST 4 RDo you maintain the original and corrected reports for at least two years, five years if it is anImmunohematology report, or ten years if it is a Pathology or Pap smear report?This criterion addresses errors in testing performed onsite (including Pathology, if applicable) in addition totesting provided at a reference laboratory.PST 5 RIf your laboratory refers tests: Is there a policy that test reports from your reference laboratory may not bealtered?You may not change any reports sent to you by a reference laboratory. If an error was made and you are notifiedverbally by the reference laboratory, make note of this and make sure you obtain a corrected report fromthe reference laboratory.PST 6 RDoes the laboratory policy prohibit reporting test results when they exceed the reportable range establishedby the laboratory and is corrective action taken and documented in accordance with any deviation in this policy?Patient results should not be reported in numbers when the results exceed the reportable range. The resultsmay be reported as “greater than” the maximum verified reportable range.
