Dept. of Medicine, Semmelweis University, H-1083 Korányi S. u. 2/a, Budapest,HungaryBackground: Autonomic neuropathy may be present in chronic alcoholic andnon-alcoholic liver diseases (1, 2) and is associated with a poor prognosis in thesepatients (3). In our previous study autonomic and sensory nerve dysfunction wasdetected in patients with primary biliary cirrhosis (4). Neuropathy has not beenevaluated up to now in patients with anti-HCV-positive chronic liver disease - it was aimof our present study.Patients and methods: Examined were 12 patients with anti-HCV-positive chronic liverdisease, controls were 14 healthy subjects. The five standard tests of cardiovascularautonomic function were applied. Peripheral sensory function was studied by theNeurometer (Neurotron, Baltimore, MD, USA). Neuroselective current perceptionthresholds (CPT) for 2 kHz, 250 Hz. 5 Hz were assessed evaluating large myelinated,small myelinated and small unmyelinated sensory nerve fiber function, respectively.Median and peroneal nerves (digital branches) were studied.Results: Autonomic neuropathy was observed in 7 patients, heart rate response todeep breathing was abnormal in 3 patients, 30/15 ratio in 1 patients, Valsalva ratio in2 patients while sustained handgrip test in 5 patients. 4 patients had one, 2 patients hadtwo and 1 patient had three abnormal autonomic function test. Sensory neuropathycould be detected in five patients at peroneal nerve testing; two of them had mediannerve damage as well. CPT at 2 kHz was significantly higher in patients compared tocontrols at median nerve (p < 0.01) as well as at peroneal nerve testing (p < 0.001)indicating hypaesthesia. Four patients had both autonomic and sensory nervedysfunction while another 4 patients were free of neuropathy.Conclusion: Early to moderate autonomic abnormalities and impairment of largemyelinated sensory nerve fiber function are the main characteristic features ofneuropathy in patients with anti-HCV-positive chronic liver disease.
