KWHP’s previous Strategic Plan 6 takes a comprehensive review <strong>of</strong> <strong>the</strong> organisation in <strong>the</strong> context <strong>of</strong>local <strong>and</strong> national developments. The strategy has mapped against <strong>the</strong> Choosing Health Agenda,Local Area Agreements <strong>and</strong> comprehensively set out a plan <strong>of</strong> implementation across <strong>the</strong>organisation. It is informed by a set <strong>of</strong> community centred values. The plan itself covers:- operational partnerships with local providers- strategic networking with o<strong>the</strong>r Health Living Initiatives- community, volunteer <strong>and</strong> service user involvement- diversification <strong>of</strong> funding sources- social enterprise income <strong>and</strong>- site management <strong>of</strong> <strong>the</strong> Health ParkProject areaPathways to Health responds to needs identified in local community consultations <strong>and</strong> public healthdata for south Bristol. This highlights <strong>the</strong> high incidence <strong>of</strong> households with a smoker, early deaths,<strong>report</strong>ed mental health problems <strong>and</strong> child <strong>and</strong> adult obesity.Project rationale <strong>and</strong> <strong>the</strong>ory baseKWHP markets its services through a range <strong>of</strong> localoutlets <strong>and</strong> has focused on building relationshipswith primary care pr<strong>of</strong>essionals for direct referralor recommendation. Residents within definedwards/super output areas are eligible for <strong>the</strong>service, although <strong>the</strong>re is scope for individualsoutside <strong>the</strong> area to access services under specialcircumstances.Pathways to Health project <strong>of</strong>fers four mainservices: an integrated package <strong>of</strong> motivationalguidance <strong>and</strong> massage <strong>the</strong>rapies; a weightmanagement course; diet <strong>and</strong> exercise basedactivities for younger people; <strong>and</strong> subsidisedmassage <strong>and</strong> complementary <strong>the</strong>rapies. Theseactivities are ei<strong>the</strong>r delivered on an individualbasis or through small groups. Whilst <strong>the</strong> specificarrangements <strong>and</strong> target groups for each activityvary, <strong>the</strong>y share some common elements.
