programme, pulmonary rehab <strong>and</strong> low-key stimulating activities for people at risk <strong>of</strong> low-levelmental ill health).The team has been able to <strong>of</strong>fer <strong>the</strong>ir expertise to o<strong>the</strong>r third sector organisations, <strong>the</strong>rebystretching <strong>the</strong> reach <strong>of</strong> <strong>the</strong> project. This is highlighted in work with Home Start in Teignbridge withwhom <strong>the</strong> team have jointly delivered healthy eating sessions for families, whilst also equipping <strong>and</strong>training <strong>the</strong>m up to be able to go on delivering to a wider client group.The consortium approach has enabled <strong>the</strong> team to find common ground with o<strong>the</strong>r healthy livingcentres <strong>and</strong> to work in partnership with <strong>the</strong>m on o<strong>the</strong>r pieces <strong>of</strong> work. An example <strong>of</strong> this is <strong>the</strong>irwork with Upstream Healthy Living Centre, with whom <strong>the</strong>y are now delivering <strong>the</strong> Walking forHealth initiative in Mid Devon.Part <strong>of</strong> <strong>the</strong> project has been about researching <strong>and</strong> making links with o<strong>the</strong>r organisations that <strong>the</strong>team can refer service users. The staff team have done this in <strong>the</strong>ir own areas <strong>of</strong> expertise <strong>and</strong> thisinformation has <strong>the</strong>n been shared amongst <strong>the</strong> team <strong>and</strong> wider throughout <strong>the</strong> whole organisation,<strong>the</strong>reby raising awareness more widely.ConclusionsKey lessonsThe team have learned that in order for people to make sustained lifestyle changes <strong>the</strong>reneeds to be an intense level <strong>of</strong> support available over a comparatively long period <strong>of</strong> time<strong>and</strong> that this is <strong>the</strong>refore an element which needs adequate resourcing.The team has discovered that one <strong>of</strong> <strong>the</strong> best ways to engage with GPs is to attend <strong>the</strong>irpractice meetings. Attending meetings <strong>and</strong> outlining services to groups <strong>of</strong> GPs promoted<strong>the</strong> most discussion <strong>and</strong> brought about <strong>the</strong> highest number <strong>of</strong> referrals.The team have found word <strong>of</strong> mouth to be one <strong>of</strong> <strong>the</strong> best forms <strong>of</strong> promotion. Getting outinto communities <strong>and</strong> working with people to create <strong>the</strong> services <strong>the</strong>y need has been <strong>the</strong>key to <strong>the</strong> success <strong>of</strong> <strong>the</strong> project.Opportunities for <strong>the</strong> futureThrough work with <strong>the</strong> SWWB Programme <strong>the</strong> team has already been involved in severalWell UK pieces <strong>of</strong> work <strong>and</strong> are looking to build on this, along with future partnershipworking opportunities with o<strong>the</strong>r healthy living centres.The team will be continuing to work with partners in health <strong>and</strong> social care around <strong>the</strong>commissioning <strong>of</strong> services.With <strong>the</strong> community links <strong>the</strong> team has been building on through <strong>the</strong> New Steps project willbe considering involvement in <strong>the</strong> Big Society Community Organisers project.172 SWWB Final Evaluation Report
