PresentationFive years after publication of the guidel<strong>in</strong>e “<strong>Perioperative</strong> Antibiotic Prophylaxis <strong>in</strong>Adults” by the National Guidel<strong>in</strong>e Programme (PNLG) as it was then called, thisupdated re-edition has been developed. It <strong>in</strong>cludes graded recommendations <strong>in</strong>tendedto provide healthcare practitioners with a set of evidence-based criteria supplementedwith cl<strong>in</strong>ical experience as a guide to when and how it is appropriate to adm<strong>in</strong>ister<strong>antibiotic</strong> <strong>prophylaxis</strong> <strong>in</strong> adult surgery.The guidel<strong>in</strong>e is part of the National System of Guidel<strong>in</strong>es (SNLG) programme, fruitof an agreement between the Department for Health Plann<strong>in</strong>g, the M<strong>in</strong>istry for Healthand the Italian Health Institute.The aim of this agreement is to produce high-quality publications to promote correctpractice throughout the National Health System.The updated guidel<strong>in</strong>e responds to a need to assess recently published materialregard<strong>in</strong>g perioperative <strong>antibiotic</strong> <strong>prophylaxis</strong> <strong>in</strong> order to verify whether therecommendations proposed <strong>in</strong> the first edition should be amended <strong>in</strong> the light of morerecent evidence. In response to criticisms of the earlier guidel<strong>in</strong>e, the work<strong>in</strong>g groupdecided to extend the recommendations to the first 100 surgical Diagnosis-RelatedGroups (DRG), analys<strong>in</strong>g newly available evidence and, when there was no evidencebase, to formulate recommendations based on the consensus op<strong>in</strong>ion of the panel.F<strong>in</strong>ally, issues relat<strong>in</strong>g to the choice, dosage and procedures for the adm<strong>in</strong>istration of<strong>antibiotic</strong>s have been addressed <strong>in</strong> greater detail. To this end the present edition<strong>in</strong>cludes models that operat<strong>in</strong>g units may use as a framework with<strong>in</strong> which to adapttheir own guidel<strong>in</strong>es to local sett<strong>in</strong>gs.The conditions prevail<strong>in</strong>g <strong>in</strong> <strong>in</strong>dividual operat<strong>in</strong>g units are <strong>in</strong> effect a major reason forissu<strong>in</strong>g a new edition of the guidel<strong>in</strong>e. The work<strong>in</strong>g group has aimed to create a toolthat can be used extensively and that will expand knowledge and stimulate debate onan issue – perioperative <strong>in</strong>fections and how best to prevent them – that rema<strong>in</strong>shighly topical.
