Dirty:encountered or there is visible contam<strong>in</strong>ation of the wound, suchas gross spillage from a hollow viscus dur<strong>in</strong>g the operation orcompound/open <strong>in</strong>juries operated with<strong>in</strong> four hoursoperations <strong>in</strong> the presence of pus or where there is a previouslyperforated hollow viscus, or compound/open <strong>in</strong>juries more thanfour hours old.Emergency contam<strong>in</strong>ated or dirty operations call for <strong>antibiotic</strong> therapy rather than<strong>prophylaxis</strong> and are thus beyond the scope of this guidel<strong>in</strong>e.Insertion of prosthetic devicesThe <strong>in</strong>sertion of a prosthetic device <strong>in</strong>creases the risk of wound <strong>in</strong>fection and SSIbecause it reduces the host’s defences 34 . Where a prosthetic device is present even alow bacteria count is sufficient to cause <strong>in</strong>fection. Antibiotic <strong>prophylaxis</strong> is normallyrecommended for surgery <strong>in</strong>volv<strong>in</strong>g the <strong>in</strong>sertion of prosthetic devicesLength of preoperative hospital stayIn the first 48 hours of his or her stay <strong>in</strong> hospital the patient’s sk<strong>in</strong> is graduallycolonised by nosocomial bacterial stra<strong>in</strong>s 25 , particularly if he or she is receiv<strong>in</strong>g<strong>antibiotic</strong>s as therapy. These stra<strong>in</strong>s are often <strong>antibiotic</strong>-resistant and may causecontam<strong>in</strong>ation dur<strong>in</strong>g the operation, encourag<strong>in</strong>g the onset of multi-resistant germassociatedSSI and <strong>in</strong>creas<strong>in</strong>g the length of postoperative hospital stay 35 .For this reason it is important to limit the preoperative stay; where this is not possibleor when subsequent operations are performed dur<strong>in</strong>g the same stay, this must betaken <strong>in</strong>to account when choos<strong>in</strong>g an <strong>antibiotic</strong> for <strong>prophylaxis</strong> 23 .Duration of surgeryThe duration of each operation is directly correlated with the risk of wound <strong>in</strong>fectionand this risk is additional to that of the class of operation 32 .
