PHYSICAL CONTROL IN CAREDuty ManagerThe Duty Manager must ensure that:1. The Use of Force/Incident Report Form is completed in full2. An Exception Report is completed where necessary (STC’sonly)3. Every member of staff who was involved in any use of force hascompleted an Officer’s Statement4. An Injury Report (body map) has been completed on any youngperson involved in the incident5. The Duty Director/Director has been informed6. All Use of Force incidents are recorded within twelve hours ofoccurrence7. The incident must be properly recorded and all paperworkstored appropriately8. All reports are completed individually in a secure area withrestricted access.
