There may be a tendency to underestimate cardiovascular risk inpatients with very high HDL-C levels. We carried out a retrospective study of patients with HDL-C levels > 90thpercentile to assess their cardiovascular status/risk.Objective: To phenotype patients with very high plasma levels of HDL-C.MethodsMethods <strong>and</strong> ulation statistics, the cohort had a lower incidence of smoking <strong>and</strong> physical inactivity, higher moderatealcohol intake, infrequent obesity <strong>and</strong> a healthy diet. There was a surprisingly low incidence of Type 2 DiabetesMellitus (only two of 113 patients). A significant percentage of the patients had either evidence of cardiovasculardisorder or highFraminghamrisk score (FRS). 15 patients had a personal cardiovascular history: 6 had suffered MI, 1had positive stress test <strong>and</strong> 8 had carotid plaques. In addition, 16 had a FRS of greater than 20%.ConclusionVery high plasma levels of HDL-C in our cohort were associated with a healthy lifestyle <strong>and</strong> low incidence of diabetes.However, there was a significant number of individuals with preclinical or clinical vascular disease.Poster <strong>Presentations</strong> * 2 0 1 051
