ABOUT FAMILY OUTREACH MCH Family Outreach supports families during crisis situations, while strengthening the family and supporting each child’s healthy development. MCH offers family preservation services, foster care, education and outreach, and services to support grandparents who are raising their grandchildren. Services are offered across Texas and in New Mexico. FOSTER CARE OUTCOMES (Fiscal Year 2011-2012) 100% EDUCATION The percentage of schoolage children who remain in school or graduate while receiving services. 100% SAFETY The percentage of children who are safe from abuse/ neglect while in care. 98% TRANSFERS The percentage of children who transfer less than two times between MCH foster homes. 90% DISCHARGES The percentage of children who are discharged to less restrictive environments consistent with their placement. 6 MCH Family Outreach Highlights in MCH Family Outreach from 2011-2012 Nurturing Parenting MCH Family Outreach adopted, conducted training and began utilizing the Nurturing Parenting curriculum to support our outreach efforts in each community with parent education. This program enables us to help families increase their skill level and knowledge of the developmental process of their children, improve their interactions with their children, and provide education to parents in vulnerable situations. Family Solutions MCH Family Outreach implemented Family Solutions in six offi ce locations. This will allow us to work more closely with families to fi nd ways for them to meet the basic needs of their children through coordinated family assessment, service planning, resource development and ongoing case management services. The program design will facilitate our continued efforts to work with community partners in each area. Family Solutions will allow us to have a greater impact on the families we serve, and it supports our goal to implement evidence-based programs with outcomes that are based on standardized measures. Continuous Quality Improvement MCH Family Outreach continued to revise and improve the Continuous Quality Improvement (CQI) processes for outreach services. The CQI process helps us monitor our outputs and outcomes and holds us accountable to established goals for outreach services. Staff members are aware of the process and actively participate to help the department comply with standards, contracts, policy and procedure, and expectations of several outside entities. MCH FAMILY OUTREACH PROFILE MCH and UTMB: Sharing a life-changing mission for newborns The MCH outreach offi ce in Houston and the University of Texas Medical Branch (UTMB) in Galveston have partnered for more than 10 years by serving mothers who give birth in prison. Mothers in the Texas Department of Corrections (TDC) unit, Carole Young Medical Facility, often are in need of a loving temporary home for their newborns. More than a decade ago, a TDC chaplain saw a need for these mothers and voiced his concerns to the UTMB staff. The UTMB caseworker contacted our Houston outreach offi ce. The relationship evolved and now MCH is known as a trustworthy and reliable agency for voluntary placements while the mothers complete their prison sentence. “It feels great to be able to help parents who do not have anyone else willing or able to care for their children,” said Veronica Whalon-Peters, director of the Houston offi ce. “MCH has great foster parents who love, care and nurture these little ones while their parents work on improving their situation for themselves and their children.”
MCH Family Outreach Partnerships MCH outreach offi ces partner with agencies, churches and other groups to meet specifi c needs in communities throughout Texas and New Mexico.
