<str<strong>on</strong>g>recommendati<strong>on</strong>s</str<strong>on</strong>g> will be made available via this new<br />
search functi<strong>on</strong>.<br />
The Maternal and Perinatal Health and Preventing<br />
Unsafe Aborti<strong>on</strong> team of the <str<strong>on</strong>g>WHO</str<strong>on</strong>g> Department<br />
of RHR, in collaborati<strong>on</strong> with the Departments of<br />
NHD and MCA and other partners, will support<br />
nati<strong>on</strong>al and subnati<strong>on</strong>al working groups to adapt<br />
and implement the guideline. This process will<br />
include the development or revisi<strong>on</strong> of existing<br />
nati<strong>on</strong>al guidelines or protocols in line with the <str<strong>on</strong>g>WHO</str<strong>on</strong>g><br />
guideline. The GREAT Network (Guideline-driven,<br />
Research priorities, Evidence synthesis, Applicati<strong>on</strong><br />
of evidence, and Transfer of knowledge) will be used<br />
to bring together relevant stakeholders to identify<br />
and assess the priorities, barriers and facilitators to<br />
guideline implementati<strong>on</strong>, and to support the ef<strong>for</strong>ts<br />
of stakeholders to develop adaptati<strong>on</strong>s and guideline<br />
implementati<strong>on</strong> strategies tailored to the local<br />
c<strong>on</strong>text (214). This includes technical support <strong>for</strong><br />
local guideline implementers in the development of<br />
training manuals, flow charts and quality indicators,<br />
as well as participati<strong>on</strong> in stakeholder meetings.<br />
6.2 Applicability issues<br />
Anticipated impact of the guideline <strong>on</strong> the<br />
organizati<strong>on</strong> of ANC<br />
Effective implementati<strong>on</strong> of the <str<strong>on</strong>g>recommendati<strong>on</strong>s</str<strong>on</strong>g><br />
in this guideline will likely require reorganizati<strong>on</strong> of<br />
<strong>care</strong> and redistributi<strong>on</strong> of health-<strong>care</strong> resources,<br />
particularly in low- and middle-income countries<br />
(LMICs). The potential barriers to implementati<strong>on</strong><br />
include the following:<br />
• nlack of human resources with the necessary<br />
expertise and skills to implement, supervise and<br />
support recommended practices, including client<br />
counselling;<br />
• nlack of infrastructure to support interventi<strong>on</strong>s, e.g.<br />
lack of power to support ultrasound equipment;<br />
• nlack of physical space to c<strong>on</strong>duct individual or<br />
group-based counselling;<br />
• nlack of community understanding of the new<br />
model of <strong>care</strong>, particularly around the c<strong>on</strong>tact<br />
schedule and potentially l<strong>on</strong>ger wait times;<br />
• nlack of physical resources, e.g. equipment, test kits,<br />
supplies, medicines and nutriti<strong>on</strong>al supplements;<br />

• nlack of effective referral mechanisms and <strong>care</strong><br />
pathways <strong>for</strong> women identified as needing<br />
additi<strong>on</strong>al <strong>care</strong>;<br />
• nlack of understanding of the value of newly<br />
recommended interventi<strong>on</strong>s am<strong>on</strong>g health-<strong>care</strong><br />
providers and system managers.<br />
• nlack of health in<strong>for</strong>mati<strong>on</strong> management systems<br />
(HMISs) designed to document and m<strong>on</strong>itor<br />
recommended practices (e.g. client cards,<br />
registers, etc.).<br />
Given the potential barriers noted above, a<br />
phased approach to adopti<strong>on</strong>, adaptati<strong>on</strong> and<br />
implementati<strong>on</strong> of the guideline <str<strong>on</strong>g>recommendati<strong>on</strong>s</str<strong>on</strong>g><br />
may be prudent. Various strategies <strong>for</strong> addressing<br />
these barriers and facilitating implementati<strong>on</strong> have<br />
been suggested in the list of c<strong>on</strong>siderati<strong>on</strong>s at the end<br />
of Chapter 4.<br />
M<strong>on</strong>itoring and evaluating the impact of<br />
the guideline<br />
The implementati<strong>on</strong> and impact of these<br />
<str<strong>on</strong>g>recommendati<strong>on</strong>s</str<strong>on</strong>g> will be m<strong>on</strong>itored at the healthservice,<br />
regi<strong>on</strong>al and country levels, based <strong>on</strong> clearly<br />
defined criteria and indicators that are associated<br />
with locally agreed targets. In collaborati<strong>on</strong> with<br />
the m<strong>on</strong>itoring and evaluati<strong>on</strong> teams of the<br />
<str<strong>on</strong>g>WHO</str<strong>on</strong>g> Departments of RHR and MCA, data <strong>on</strong><br />
country- and regi<strong>on</strong>al-level implementati<strong>on</strong> of the<br />
<str<strong>on</strong>g>recommendati<strong>on</strong>s</str<strong>on</strong>g> will be collected and evaluated in<br />
the short to medium term to evaluate their impact <strong>on</strong><br />
nati<strong>on</strong>al policies of individual <str<strong>on</strong>g>WHO</str<strong>on</strong>g> Member States.<br />
Interrupted time series, clinical audits or criteri<strong>on</strong>based<br />
audits could be used to obtain the relevant<br />
data <strong>on</strong> the interventi<strong>on</strong>s c<strong>on</strong>tained in this guideline.<br />
6.3 Updating the guideline<br />
In accordance with the c<strong>on</strong>cept of <str<strong>on</strong>g>WHO</str<strong>on</strong>g>’s GREAT<br />
Network, which employs a systematic and c<strong>on</strong>tinuous<br />
process of identifying and bridging evidence gaps<br />
following guideline implementati<strong>on</strong> (214), the<br />
proposed guideline will be updated five years<br />
after publicati<strong>on</strong> unless significant new evidence<br />
emerges that necessitates earlier revisi<strong>on</strong>. The <str<strong>on</strong>g>WHO</str<strong>on</strong>g><br />
Steering Group will c<strong>on</strong>tinue to follow the research<br />
developments in the area of ANC, particularly <strong>for</strong><br />
those questi<strong>on</strong>s <strong>for</strong> which no evidence was found and<br />
those that are supported by low-quality evidence,<br />
where new <str<strong>on</strong>g>recommendati<strong>on</strong>s</str<strong>on</strong>g> or a change in the<br />
published recommendati<strong>on</strong> may be warranted,<br />
