the completed DOI <strong>for</strong>m. The resp<strong>on</strong>sible technical<br />

officer collated and reviewed signed DOI <strong>for</strong>ms and<br />

curriculum vitae, in c<strong>on</strong>juncti<strong>on</strong> with the director of<br />

the <str<strong>on</strong>g>WHO</str<strong>on</strong>g> Department of RHR and, with input from<br />

the Steering Group, determined whether a c<strong>on</strong>flict of<br />

interest existed. Where any c<strong>on</strong>flict of interest was<br />

declared, the Steering Group determined whether it<br />

was serious enough to affect the individual’s ability<br />

to make objective judgements about the evidence<br />

or <str<strong>on</strong>g>recommendati<strong>on</strong>s</str<strong>on</strong>g>. To ensure c<strong>on</strong>sistency, the<br />

Steering Group applied the criteria <strong>for</strong> assessing the<br />

severity of a c<strong>on</strong>flict of interest in the <str<strong>on</strong>g>WHO</str<strong>on</strong>g> handbook<br />

<strong>for</strong> guideline development (14).<br />

All findings from the received DOI statements<br />

were managed in accordance with the <str<strong>on</strong>g>WHO</str<strong>on</strong>g> DOI<br />

guidelines <strong>on</strong> a case-by-case basis. Where a c<strong>on</strong>flict<br />

of interest was not c<strong>on</strong>sidered significant enough to<br />

pose any risk to the guideline development process<br />

or reduce its credibility, the expert was <strong>on</strong>ly required<br />

to declare such c<strong>on</strong>flict at the GDG meeting and no<br />

further acti<strong>on</strong> was taken. C<strong>on</strong>flicts of interest that<br />

warranted acti<strong>on</strong> by <str<strong>on</strong>g>WHO</str<strong>on</strong>g> staff arose where experts<br />

had per<strong>for</strong>med primary research or a systematic<br />

review related to any guideline <str<strong>on</strong>g>recommendati<strong>on</strong>s</str<strong>on</strong>g>;<br />

in such cases, the experts were restricted from<br />

participating in discussi<strong>on</strong>s and/or <strong>for</strong>mulating any<br />

recommendati<strong>on</strong> related to the area of their c<strong>on</strong>flict<br />

of interest. At the final GDG meeting, members<br />

were required again to state any c<strong>on</strong>flicts of interest<br />

openly to the entire group, and were required to<br />

submit a signed and updated versi<strong>on</strong> of their earlier<br />

DOI statements. A summary of the DOI statements<br />

and in<strong>for</strong>mati<strong>on</strong> <strong>on</strong> how c<strong>on</strong>flicts of interest were<br />

managed are included in Annex 3.<br />

2.14 Document preparati<strong>on</strong> and peer<br />

review<br />

Following these three GDG meetings, members of the<br />

Steering Group prepared a draft of the full guideline<br />

document with revisi<strong>on</strong>s to accurately reflect the<br />

deliberati<strong>on</strong>s and decisi<strong>on</strong>s of the GDG participants.<br />

This draft guideline was then sent electr<strong>on</strong>ically to<br />

the GDG participants <strong>for</strong> further comments be<strong>for</strong>e<br />

it was sent to the ERG. The Steering Group <strong>care</strong>fully<br />

evaluated the input of the peer reviewers <strong>for</strong> inclusi<strong>on</strong><br />

in the guideline document and made revisi<strong>on</strong>s to the<br />

guideline draft as needed. After the GDG meetings<br />

and peer review process, further modificati<strong>on</strong>s to<br />

the guideline by the Steering Group were limited to<br />

correcti<strong>on</strong>s of factual errors and improvements in<br />

language to address any lack of clarity. The revised<br />

final versi<strong>on</strong> was returned electr<strong>on</strong>ically to the GDG<br />

<strong>for</strong> final approval.<br />

2.15 Presentati<strong>on</strong> of guideline c<strong>on</strong>tent<br />

A summary of the <str<strong>on</strong>g>recommendati<strong>on</strong>s</str<strong>on</strong>g> is presented in<br />

Table 1 within the executive summary at the beginning<br />

of this guideline. As evidence was evaluated <strong>for</strong><br />

several outcomes and six domains (effects, values,<br />

resources, equity, acceptability, feasibility) <strong>for</strong><br />

each recommendati<strong>on</strong>, we have not presented the<br />

decisi<strong>on</strong>s <strong>on</strong> quality of evidence in this summary<br />

table. Summary tables of the main c<strong>on</strong>siderati<strong>on</strong>s<br />

(including certainty of the evidence <strong>on</strong> effects) <strong>for</strong><br />

each recommendati<strong>on</strong> are presented in Web annex 3.<br />

The “Evidence and <str<strong>on</strong>g>recommendati<strong>on</strong>s</str<strong>on</strong>g>” secti<strong>on</strong> of<br />

the guideline (Chapter 3) summarizes the evidence<br />

and other c<strong>on</strong>siderati<strong>on</strong>s reviewed by the GDG at<br />

the Technical C<strong>on</strong>sultati<strong>on</strong>s. To improve readability,<br />

the “values” domain has been described (and<br />

highlighted in a box entitled “Women’s values”) at<br />

the beginning of each secti<strong>on</strong> <strong>for</strong> the five types of<br />

interventi<strong>on</strong>s, instead of <strong>for</strong> each recommendati<strong>on</strong>,<br />

to avoid repetiti<strong>on</strong>. The language used to interpret<br />

the Cochrane review evidence <strong>on</strong> effects is c<strong>on</strong>sistent<br />

with the EPOC approach (30). Evidence assessed as<br />

being of very low certainty is not presented in the<br />

text, but can be found in the Web supplement.<br />

The Steering Group c<strong>on</strong>solidated <str<strong>on</strong>g>recommendati<strong>on</strong>s</str<strong>on</strong>g><br />

into this guideline from other recent, GRC-approved<br />

<str<strong>on</strong>g>WHO</str<strong>on</strong>g> guidelines relevant to the provisi<strong>on</strong> of<br />

comprehensive, integrated routine ANC to women<br />

in certain c<strong>on</strong>texts or <strong>for</strong> certain c<strong>on</strong>diti<strong>on</strong>s. In most<br />

instances, these <str<strong>on</strong>g>recommendati<strong>on</strong>s</str<strong>on</strong>g> are identical<br />

to those found in the specific separate guideline.<br />

Where we have integrated <str<strong>on</strong>g>recommendati<strong>on</strong>s</str<strong>on</strong>g>,<br />

the strength of the recommendati<strong>on</strong> and quality<br />

of the evidence as determined by the respective<br />

GDGs <strong>for</strong> those guidelines has been recorded in<br />

the remarks secti<strong>on</strong> of the recommendati<strong>on</strong>. Such<br />

<str<strong>on</strong>g>recommendati<strong>on</strong>s</str<strong>on</strong>g> are indicated by a footnote<br />

in the ANC guideline text specifying that the<br />

recommendati<strong>on</strong> has been “integrated from” the<br />

specific guideline. A few <str<strong>on</strong>g>recommendati<strong>on</strong>s</str<strong>on</strong>g> required<br />

adaptati<strong>on</strong> <strong>for</strong> the purposes of the ANC guideline,<br />

and the Steering Group c<strong>on</strong>sulted the relevant <str<strong>on</strong>g>WHO</str<strong>on</strong>g><br />

departments that produced the specific guidance<br />

to c<strong>on</strong>firm that adaptati<strong>on</strong>s were c<strong>on</strong>sistent with<br />

original <str<strong>on</strong>g>recommendati<strong>on</strong>s</str<strong>on</strong>g>. Such <str<strong>on</strong>g>recommendati<strong>on</strong>s</str<strong>on</strong>g><br />
