MEDICAL
Brooks Rehabilitation has more than 40 years
of experience in providing physical
rehabilitation services. It offers advanced
therapy and medical care, along with
the compassion, motivation and hope
needed to help people reach a healthy
recovery. Brooks offers many services
including: inpatient rehabilitation and
outpatient therapy at Brooks Hospital on
University, home care (AmeriCare Main
office: 844-238-0064), skilled nursing
at the Bartram Crossing facility, and
assisted living/memory care at Bartram
Lakes.
Baker County Medical Services was established
in 1993 to operate Ed Fraser Memorial
Hospital and W. Frank Wells Nursing
Home. Ed Fraser Hospital offers inpatient
and outpatient care, respiration, physical
therapy and emergency services. The
Dopson Family Medical Center was also
created under Baker County Medical
Services to provide primary and specialty
physician services to the residents of
Macclenny and Baker County.
Flagler Hospital is a 335-bed, acute care
facility that has operated as a not-forprofit
health care institution in St.
Augustine since its founding in 1889.
The hospital provides various medical
services, including psychiatric services
in the outpatient behavioral health
clinic. The independent Leapfrog Group
recently gave Flagler an ‘‘A’’ grade for
patient safety, and for nearly a decade,
the hospital has consistently ranked
among the top 5 percent in the nation
for clinical excellence and patient
safety. In addition, Flagler Hospital was
recognized as a Magnet® organization in
2006 and again in 2012 by the American
Nurses Credentialing Center (ANCC).
The Magnet Recognition Program is a
voluntary organizational credentialing
program that recognizes health care
organizations for quality of care,
nursing excellence and innovations in
professional nursing practice.
Hope Haven Children’s Clinic and Family Center,
founded in 1927, focuses on outpatient
care of children with developmental,
psychological, behavioral or academic
achievement problems. This facility
provides services for children, families
and young adults with special needs.
Hope Haven serves more than 5,000
families each year.
Kindred Hospital North Florida, located in
Green Cove Springs, is an 80-bed fully
accredited transitional care hospital
long-term acute care facility. The hospital
offers a range of services for medically
complex patients who require continued
care and extended recovery time.
Mayo Clinic's campus in Jacksonville is the
premier medical destination center in the
Southeast, and was recently named the
No. 1 hospital in Florida in U.S. News &
World Report’s rankings of best hospitals
in America. The campus was established
in 1986, with a focus on treating complex
conditions and unraveling hard-to-solve
medical problems, pooling the knowledge
of physicians and scientists through
state-of-the-art electronics at its three
locations. Like Mayo’s other locations
in Rochester, Minn., and Scottsdale,
Ariz., the Jacksonville site has education
and research arms that complement its
patient-care services. Research is done
in such areas as Parkinson’s disease,
Alzheimer’s disease, epilepsy and more.
Mayo has the technologically advanced
da Vinci robot, which allows surgeons to
perform minimally invasive surgeries.
Mayo Jacksonville dedicated the first
center in the history of Mayo Clinic
devoted exclusively to cancer research
in February 2004; it’s nationally ranked
cancer program has received the National
Cancer Institute’s highest designation as
one of only two comprehensive cancer
centers in Florida. Mayo Clinic Hospital
is the admitting hospital for Mayo Clinic’s
campus in Florida. It has 304 beds and
22 operating rooms, and offers care in
more than 35 adult medical and surgical
specialties. The hospital includes a fullservice
emergency department, open to
everyone.

Memorial Hospital has been serving Northeast
Florida for more than 45 years now. The
hospital is a 418-bed acute care facility
offering a range of services, including 24-
hour emergency care at three different
locations around the Jacksonville area.
The hospital also features an accredited
Chest Pain Center, certified Stroke
Center, Heart Center, which treats more
than 10,000 patients annually, Bone and
Joint Center, and Memorial’s maternity
center — Special Beginnings. The
Emergency Department at Memorial
treats more than 100,000 patients each
year for a variety of minor and major
ailments, using the most advanced
technology available. The 33-bed
emergency room features comfortable
treatment rooms, kid-friendly rooms and
advanced patient monitoring equipment.
