Social Security Cards<br />
As of August 13, 2012, if you live or receive mail in the Bronx,<br />
Brooklyn, Manhattan, or Queens, you must visit a social security<br />
card center (not a central social security office) to apply for a new<br />
or replacement social security card. Call 800.772.1213 (TTY:<br />
800.325.0778) Mon.–Fri. 7–7. Say “replace a social security card”<br />
to get assistance. Say “help” at any time to speak to a customerservice<br />
representative. Spanish spoken.<br />
Visit www.ssa.gov to find a social security office or card center.<br />
Individuals can apply by mail by requesting and filling out a paper<br />
application. Included in the application must be original picture-ID<br />
documents proving one’s identity and US citizenship (or ID showing<br />
valid immigration documentation). Original documents will be<br />
returned.<br />
