Generic Theatre Standard No 24 - Incident Reporting<br />
Standard Statement: All staff members are responsible for the identification and documentation of<br />
accidents and incidents within the department.<br />
All staff will refer to the following trust policies on the reporting and management of incidents:<br />
• Incident Reporting and Management Policy and Procedure<br />
• Reporting of Injuries, Diseases and Dangerous Occurrences (RIDDOR) Policy<br />
• Serious Incident management Policy and Procedure<br />
• A Policy and Procedure for Being Open<br />
Method:<br />
All staff will receive training on the responsibilities of the employer and the employee in relation to<br />
health and safety, and be familiar with the following policies;<br />
• Health and Safety and Environmental Policy (Introduction & Responsibilities for Health<br />
& Safety)<br />
• Policy for Risk & Incident Management<br />
• Serious Adverse Incident Policy & Procedure<br />
All accidents to, or all incidents involving staff/and or equipment/ adverse incidents/breaches of<br />
protocol or any issues pertaining to patient care and the running of the theatre lists must be<br />
reported to the co-ordinator and a report completed via the trust incident reporting system - Datix.<br />
