Generic Theatre Standard No 09 - Care of the Deceased In the Operating<br />

Department<br />

Please read Allow Natural Death (A.N.D.) Policy and Guidance Framework for Adults &<br />

Children (including neonates).<br />

Care of the Deceased Patient in RCHT Operating Theatre<br />

Contacts in the Event of a Patient Death in the Perioperative Environment<br />

• Bereavement Office staff can be contacted on 01872 252713<br />

• The Bereavement Office is operated by 4 members of staff and open Monday to Friday<br />

– 9.00am to 5.00pm<br />

• The mortuary staff have key liaison with the Bereavement Support Service staff,<br />

coordinating on issues such as viewings and specialised services.<br />

• The mortuary staff work 9.00am to 5.00pm and ‘on-call’<br />

• The Department of Histopathology, staff are based at Royal Cornwall Hospital where<br />

mortuary and post mortem facilities are available.<br />

Following the death of the patient in the perioperative environment<br />

• The requirements of different faiths and religions must be taken into consideration.<br />

• Daytime: The Bereavement Office must be telephoned to be informed.<br />

• Night Time: The Bereavement Office will communicate with the mortuary to determine<br />

any new admissions to the mortuary out of hours. A message can also be left on the<br />

answer phone so Bereavement Staff can pick up once they are in.<br />

• A member of the Bereavement Office staff will then:<br />

Go to the ward to collect any property left behind<br />

Contact a doctor to complete the death certificate<br />

• Visiting the deceased is coordinated by the Bereavement Office; they will also<br />

coordinate and have links with all funeral directors.<br />

• The Bereavement Officer will consult the mortuary register and liaise with ward staff<br />

and the chaplain regarding post mortems, funeral requirements and consent, advising<br />

the mortuary technicians of requirements<br />

• The mortuary staff will ensure the Pathology Directorate mortuary card is completed<br />

and placed in the correct file.<br />

Signatures<br />

• All entries into the health record, including amendments, should be clearly dated,<br />

timed, signed and the designation of the person making the entry should be clearly<br />

recorded.<br />
