13. Positioning of the Patient<br />
• A record will be made of the positioning of the patient, including pressure-relieving<br />
devices in the patient’s perioperative care plan. Inadvertent nerve damage and<br />
pressure sores are a common source of litigation (AfPP, 2007).<br />

15. The Procedure List<br />
• The clinician undertaking the procedure must take responsibility for compilation of the<br />
procedure list in a recognised printed format; alternatively by designating a member of<br />
their team (AfPP 2006).<br />
16. The list should include:<br />
• Surname, forename, NHS number, date of birth of the patient<br />
• Ward<br />
• Procedure<br />
• Type of anaesthetic<br />
• Surgeon<br />
• Anaesthetist<br />
• Theatre number<br />
• Start time<br />
• Abbreviations must not be used. To avoid ambiguity about the digit on which the<br />
operation is to be performed, fingers should be described as: thumb, index, middle, ring<br />
and little. Toes should be described as halux (or big), second, third, fourth and fifth (or<br />
little).<br />
