Ordering prescriptions<br />

Once you have flled a prescription through CVS<br />

Caremark, you can order reflls by mail in three ways�<br />

You should order your refll 14 days before your current<br />

prescription runs out� Suggested refll d<strong>at</strong>es will be<br />

included on the prescription label you receive from<br />

CVS Caremark�<br />

Three ways to order prescriptions:<br />

• Online� Go to caremark�com� If you are a frst-time<br />

visitor, you’ll need to register using your CVS<br />

Caremark ID number (shown on your CVS Caremark<br />

ID card)� This is the most convenient way to order<br />

reflls and inquire about the st<strong>at</strong>us of your order any<br />

time of the day or night�<br />

• By phone�<br />

– For existing prescriptions:<br />

Call Customer Care <strong>at</strong> 1-800-772-2301 for fully<br />

autom<strong>at</strong>ed refll service� Have your CVS Caremark<br />

ID number ready�<br />

– For new prescriptions:<br />

Complete a mail service order form and send it to<br />

CVS Caremark along with your prescription�<br />

• By mail� Attach the refll label provided with your last<br />

order to a mail service order form� Enclose payment<br />

with your order�<br />

You can expect your medicine to arrive approxim<strong>at</strong>ely<br />

10 calendar days after CVS Caremark receives your<br />

prescription� If you are currently taking a medic<strong>at</strong>ion,<br />

be sure to have <strong>at</strong> least a 14-day supply on hand when<br />

you order� If you don’t have enough, ask your doctor<br />

to give you a second prescription for a 30-day supply<br />

and fll it <strong>at</strong> a particip<strong>at</strong>ing retail pharmacy while your<br />

mail-order prescription is being processed�<br />

Overnight or second-day delivery may be available in<br />

your area for an additional charge� Your package will<br />

include a new mail service order form and an invoice,<br />

if applicable� You will also receive the same type of<br />

inform<strong>at</strong>ion about your prescribed medicine th<strong>at</strong> you<br />

would receive from a retail pharmacy�<br />

Covered prescriptions<br />

For your prescription to be covered, it must meet<br />

CVS Caremark’s coverage criteria� In addition, all<br />

prescriptions are subject to the limit<strong>at</strong>ions, exclusions,<br />

and procedures described in this SPD� When more than<br />

one defnition or provision applies, the most restrictive<br />

applies and exclusions take precedence over general<br />

beneft descriptions�<br />

The following prescription types are generally covered,<br />

but some may require prior approval, be limited in the<br />

amount you can get <strong>at</strong> any one time, or be limited by<br />

the age of the p<strong>at</strong>ient�<br />

• Drugs th<strong>at</strong> legally require a prescription, including<br />

compounded drugs where <strong>at</strong> least one ingredient<br />

requires a prescription, subject to the exceptions<br />

listed in this chapter<br />

• Diabetic test strips, alcohol swabs, and lancets<br />

• Insulin, insulin pens, insulin preflled syringes,<br />

needles, and syringes for self-administered injections<br />

The list of preferred drugs, covered drugs, noncovered<br />

drugs, and coverage management programs and<br />

processes is subject to change� As new drugs become<br />

available, they will be considered for coverage under<br />

the <strong>Plan</strong> as they are introduced�<br />

Diabetic supplies and medic<strong>at</strong>ions<br />

You can purchase drugs and supplies to control your<br />

diabetes for one copay or coinsurance amount when<br />

you submit prescriptions for the diabetic supplies<br />

<strong>at</strong> the same time as your prescription for insulin<br />

or oral diabetes medic<strong>at</strong>ion, or when you submit<br />

prescriptions for multiple insulins or oral diabetic<br />

medic<strong>at</strong>ions, to CVS Caremark Mail Service Pharmacy�<br />

Common diabetic supplies include lancets, test strips,<br />

alcohol swabs, and syringes or needles� The copay or<br />

coinsurance amount you pay will depend on the type of<br />

diabetes medic<strong>at</strong>ion prescribed�<br />

If you purchase diabetic supplies or diabetic<br />

medic<strong>at</strong>ions <strong>at</strong> a retail pharmacy, separ<strong>at</strong>e copays or<br />

coinsurance amounts will apply to each item�<br />
