As President of ANA-
Michigan, it is important
that I stay informed about
legislative or policy issues that
will or could impact my nursing
colleagues in Michigan and
nurses all over the country.
ANA sends me many emails
each day with information on
nursing practice and policy,
nursing positions on ethics
Carole Stacy,
MSN, MA, RN
and innovation. Advancing the nursing profession
is the heart of ANA and ANA-Michigan. We know
that by speaking through our unified voice of 4
million nurses on policy issues, education and
training; we can improve healthcare for our
patients and enhance our practice environment.
Advocacy is an essential component of what ANA
and ANA-Michigan do. Through advocacy, we can
all make a difference in the future of nursing. As
nurses we advocate for our patients every day, but
there are many opportunities for nurses to have
their voices heard and to advocate at the highest
levels of policymaking.
ANA-Michigan has two committees that work
on advocacy issues, our Legislative committee
and our Nursing and Health Policy committee.
These committees do much of the heavy lifting
on identifying issues, creating position papers,
contacting legislators, and often they are the
first ones to carry our voices forward. We are
fortunate that this year we have had ANA-
Michigan members willing to volunteer to work
on these committees. I can remember years
when we had to do some pleading and begging
for nurses to step up and help do this work. This
position has given me the chance to interact with
nurses from around the state and speak with them
regarding advocacy issues we are working on.
Most of these nurses are usually interested and
for most, it’s the first time they have heard some
of this information. I leave with assurances that
they want to be involved, but that is usually the
last I hear from them. I question why we, a small
organization, 3,000 nurses out of the 190,000
nurses in Michigan, work so hard to advocate for
nurses. Don’t those other 187,000 nurses care
about our profession? This is the question that
causes me to toss and turn some nights.
Then along comes State Senator Maureen
Walsh from the state of Washington who throws
out the casual statement that nurses “probably
play cards for a considerable amount of the day.”
Well I think all 4 million nursing voices have been
heard on this subject! In three days, 675,000
signatures were collected on a petition for
Senator Walsh to shadow a nurse for 12 hours.
Did I mention that her office received 17,000
decks of cards last week? That is a tremendous
response from our nursing colleagues. Comments
are still filling up Facebook pages and Show me
your Stethoscope has more comments and shares
then I can count. Again, what a wonderful coming
together when someone “disses” us.
The voices of 4 million nurses across the
country were heard speaking on the issue of some
individuals not respecting the work we do or the
idea that nurses don’t use stethoscopes etc. Why
don’t we use our voices on the issues that really
impact our practice; the nurse license compact,
practicing to the full extent of our education and
scope, protecting the title of nurse? My question
to my nurse colleagues is, where are our voices on
all of the other nursing advocacy and policy issues?
MESSAGE FROM THE EXECUTIVE DIRECTOR
I’m excited to share with
the nursing community
that the ANA-Michigan
membership has approved
for ANA-Michigan to join
and lend support to the
Michigan Coalition to Prevent
Gun Violence. Membership
supported this effort during
the annual meeting held on
March 1 in Grand Rapids.
Tobi Lyon Moore,
MBA
ANA-Michigan recognized the cause and
the necessity for this coalition. The Michigan
Coalition to Prevent Gun Violence (MCPGV) is
an affiliate of States United to Prevent Gun
Violence, a national non-profit organization
focused on supporting state-based gun violence
prevention programs. MCPGV and its partners
work collaboratively to prevent gun violence
through community education and stronger gun
laws. To learn more about MCPGV visit: https://
michigancoalitiontopreventgunviolence.org/
mcpgv/.
Membership also gave ANA-Michigan approval
to sign our support to federal legislation, the
Energy Innovation and Carbon Dividend Act
(H.R. 763) which will drive down America’s
carbon pollution and bring climate change under
control, while unleashing American technology
innovation and ingenuity. This policy was also
introduced in the Senate in 2018 as S. 3791.
A viable climate change solution needs to
be big and lasting — which means bipartisan
legislation. ANA-Michigan is a proud endorser
and fully supports the Energy Innovation and
Carbon Dividend Act. This policy will drive down
America’s carbon pollution while unleashing
American technology innovation and ingenuity.
We support it because it’s:
• Effective—This policy will reduce America’s
emissions by at least 40% in the first 12 years.
• Good for People—This policy will improve
health and save lives. Additionally, the
carbon dividend puts money directly into
people’s pockets every month to spend as
they see fit, helping low- and middle-income
Americans.
• Good for the Economy—This policy will
create 2.1 million new jobs, thanks to
economic growth in local communities
across America.
• Bipartisan—Republicans and Democrats are
both on board, co-sponsoring this bill. Many
Americans support Congress taking action
on climate change, including more than
half of Republicans. Solving climate change
is too urgent to get caught up in partisan
politics.
• Revenue Neutral—The fees collected on
carbon emissions will be allocated to all
Americans to spend any way they choose.
The government will not keep any of the
fees collected.
act, or are interested in endorsing, visit: https://
“If Not Us, Then Who? If Not Now, Then When?”
– Nurses As Change Agents
At ANA-Michigan’s midwinter
conference in Grand
Rapids this year, the theme
was creativity and innovation
in nursing. Conference
attendees heard presentations
on social issues facing our
communities. Regardless
of one’s politics, registered
nurses are inherently qualified
to be change agents in their
Donna J. Craig,
RN, JD
communities. What may not be so evident is
how do registered nurses go from advocating
for patients to advocating for communities? This
article provides guidance on how nurses can serve
to innovate for social and economic change when
serving as a volunteer to non-profit organizations
and associations.
Professional and Personal Talents and Skills
– Registered nurses, by virtue of training and
education, have many skills that are beneficial to
organizations that promote social change and
community need. Registered nurses are by nature
very organized, as demonstrated by their ability to
prioritize the needs of multiple patients. Kindness
and compassion are also attributes of professional
nurses, as is carrying out their professional duties
in an empathic and non-judgmental manner. These
skills are sought out by organizations at the board,
taskforce and committee levels.
In addition to nursing skills, the essential job
functions of a registered nurse require being good
communicators and listeners, whether these skills
are used to educate patients and families regarding
a medical diagnosis or conveying changes in
patient’s condition to other health care team
members. Nurses must also be agile, focused, and
detailed oriented in order to care for their patients.
These characteristics and nurses’ critical thinking
are essential traits that help registered nurses
judge situations and make appropriate decisions.
While these traits, talents, and skills are
sought by community organizations, finding the
right fit may depend on a nurse’s personal skills
and talents. If you are a number’s person, your
niche may be more attuned to overseeing an
organization’s budget or finances. If you are a
“people person,” would your personality better
serve an organization in promoting or presenting
educational programs? To help in determining
where to use your professional and personal skills
in a nonprofit organization capacity, take inventory
and make an assessment of your professional and
personal skills and talents.
Find the Right Fit – The first step in identifying
a community organization that could use your
skills and talents, while meeting your needs, is to
do your research. Search the internet and talk to
friends, relatives, and acquaintances to see if they
are aware of potential organizations where you can
take an active role in advancing the mission of the
organization. An organization’s mission statement
should describe its values, philosophy, and purpose
for formation, and should guide, inspire and
energize you to get involved.
Once potential organizations have been
identified, assess whether they match up with your
passion and needs. If so, reach out to them. Advise
them of your interests, skills, and talents. Find out
more about the organization and determine their
expectations of you in regard to their needs. If you
were to join their board or serve on a task force or
committee, how long would be your commitment?
After gathering information, determine if the
organization’s needs match up with your passion,
expectations and skill set. If so, before you agree
to submit your name for consideration, ask if you
could attend a meeting to see the board, task
force or committee in action. That may not be
possible if confidential matters would be discussed.
The purpose of attending a meeting is to see the
organization in action.
In addition, you should seek out the
organization’s respective board, task force, and
committee members to speak to. To get a good
perspective of the organization it is always good to
speak with a member who has been affiliated with
the organization for some time and a relatively new
member. These discussions provide insight into
whether the organization’s members articulate
and advance the organization’s mission, how they
see their roles, why they chose to become part of
the organization, and what they hope to achieve.
You will also learn whether the members have an
advisory role or are active in directing the business
of the organization.
You Are on the Board, Now What? – Once
you join an organization’s board, task force
or committee, you will have the obligation to
adhere to the organization’s mission and become
the guardian of the organization’s values. With
these obligations come legal and fiduciary
responsibilities, which starts with becoming
familiar with the organization’s bylaws and
policies. Having a fiduciary obligation means you
must conduct yourself in the best interests of the
organization. Acting in the best interests of the
organization means asking questions and getting
the necessary information before voting. This
also means getting clarification on matters until
satisfied that you have all the information needed
to make an informed decision and cast your vote.
Conflicts of Interest May Arise – When
individuals with varying professional backgrounds,
affiliations and outside activities join an
organization, it is inevitable that conflicts of
interest will arise. A potential conflict of interest
arises when an individual’s interests, affiliations or
professional, personal, or social relationships could
influence decisions involving the organization.
If you are aware of any interests, affiliations
or relationships that could conflict with the
organization’s business, you have a duty to advise
the organization. Your duty is one of transparency
in bringing forth information; it is the other
members of the board, task force or committee to
determine if they feel a conflict of interest exists.
If based on the information you convey, a conflict
of interest is established, you have an obligation
to follow the organization’s policies in addressing
the conflict, which may include your recusal
from discussions and voting on the matter. The
bottom line is that you have a ‘duty of loyalty” to
not put your personal interests, affiliations, and
relationships ahead of the organization’s interests.
At all times, the only hat you should be wearing
when at the table discussing the organization’s
business is the organization’s hat. If other factors,
outside the interests of the organization come into
ANA-MICHIGAN 2019 ANNU
POSTER PRESENTERS
ANA-Michigan’s Conference event took place
Thursday, February 28th and Friday, March 1st at the
GVSU L.V. Eberhard Center in Grand Rapids, Michigan.
Thanks to our attendees and exhibitors from all over,
the conference was a hit!
Creativity and Innovation in Nursing was the
theme for this year’s event. Our landscape around
us is continually changing as well as scope of
practice. Creativity and innovation many times go
hand in hand. As nurses, we are open to change
and want to be in the know of innovative and
progressive practices as it applies to the care of our
patients and colleagues.
On Thursday, attendees were able to meet with
exhibitors and our poster presenters. They attended
a round table discussion on legislation and heard
from our speakers throughout the day. The meeting
focused on embracing the evolving role of the nurse
while harnessing the foundations of nursing practice.
Topics of cannabis, gun violence, artificial intelligence
and climate change were topics we showcased with
presenters who are in the cutting edge of their field.
Later in the night, a reception was held at the
Eberhard Center, and various award recipients were
honored. Attendees were able to network and
socialize with each other, connecting with people in
similar fields.
Friday, the Annual Board Meeting began. New
members were elected to the board, past members
were recognized for their service, and various topics
and legislation was discussed. Attendees were able
to listen to the final speakers of the conference in the
afternoon, Elizabeth Del Buono and Cindy Hayes.
We were so pleased with all the presentations and
are happy to see nurses from all over learning and
growing. And if you were unable to attend, stay tuned
for information about next year’s annual conference!
LEGISLATIVE UPDATE
Regulation of Medication Aides and the
Enhanced Nursing Licensure Compact Get
Hearings in Health Policy Committee
Representative Mary
Whiteford, RN, has reintroduced
the enhanced
nursing licensure compact
(eNLC) legislation. HB 4042
would join Michigan with
other states participating in
the eNLC to allow an RN to
obtain a multi-state license and
Monika Miner
have privileges to practice in
any other eNLC state without
having to obtain licensure in each individual state.
The bill would also authorize a nurse who holds a
multistate license to practice registered nursing or
practical/vocational nursing in Michigan under the
eNLC.
The bill was referred to the House Health
Policy committee and had a hearing only on
April 11th. The National Council of State Boards
of Nursing testified in support of the legislation.
Several organizations support the bill such as
ANA-Michigan, the Michigan Health and Hospital
Association, and AARP.
Representative Ben Fredrick has re-introduced
the medication aide legislation with some changes
from last year’s bill. According to the legislative
analysis, House Bill 4098 would allow for the
training and permitting of medication aides, with
conditions running parallel to the training and
permitting of certified nurse aides (CNAs), which
were adopted in 2017. Medication aide would
mean a nurse aide who holds a registration to
engage in practice as a medication aide.
Practice as a medication aide would be defined
as administering regularly scheduled medications
to residents of a nursing home or skilled nursing
facility while under the supervision of a registered
professional nurse. Practice as a medication
aide would not include administering controlled
substances or medications in injectable forms
or the initial administration of medications or
administration of “as needed” medication. A
medication aide must first be registered and
have practiced as a nurse aide and must take
a competency test upon completion of a state
approved medication aide program. The bill had a
hearing only in House Health Policy committee on
May 2. ANA-MI is neutral on the substitute version
Responding to Changes in Healthcare and
the Nursing Workforce
If you believe the profession of nursing has
undergone a lot of change in the past several years,
data from the 2017 Michigan Nursing Licensure Survey
suggest that there is good reason for that. At this
same time, there’s been a proliferation of technology
in both healthcare academic and practice settings
and a dearth of nursing clinical placement availability
reported by nursing educational programs in all regions
of the state. In recent years, the Michigan Department
of Health and Human Services has collaborated with
MPHI, the Michigan Department of Licensing and
Regulatory Affairs (LARA) and nursing stakeholders
Deborah Bach-
Stante, RN, MPH,
Director
in the collection and analysis of Michigan nursing workforce data. Forty
seven percent of nurses renewing their Michigan nursing license in 2017
completed the Michigan Nursing Licensure Survey, providing a rich supply
of data about demographic and employment characteristics of nurses in
Michigan. Following are some highlights from the survey results identifying
key shifts in characteristics of practicing nurses in Michigan, as well as
areas where advanced planning may be necessary due to anticipated
changes in the available nursing workforce. Survey results were weighted
to represent all licensed nurses in Michigan.
Currently, the number of bachelor prepared nurses exceeds those with
an associate’s degree; however, there are still fewer currently licensed,
practicing nurses who began their career with preparation at the bachelor
level than those who entered the field with less education. More than 85%
of actively employed licensed registered nurses (RNs) and 95% of actively
employed licensed practical nurses (LPNs) were providing direct patient
care, with the majority employed full time, 65.1 % and 61.3%, respectively. Of
the nurses providing direct care, more than 60% RNs and approximately 50%
of LPNs were working less than 40 hours per week, including approximately
35% of RNs and 29% of LPNs working 30 -

NURSES LEAD PROGRESS IN CHOOSING
WISELY ® CAMPAIGN
Call for implementation and evaluation of evidence-based recommendations
Reprinted from American Nurse Today
NURSES are masters at
American Board of Internal
Medicine (ABIM) Foundation
expanded its national
Choosing Wisely campaign
about avoiding unnecessary
tests, treatments, and
procedures for patients.
The American Academy of Nursing launched the
Choosing Wisely effort aimed at nurses in 2014.
In June 2018, the Academy announced five new
recommendations for nursing, bringing the total
to 25. Academy President Karen Cox, PhD, RN,
FAAN, and Mary Fran Tracy, PhD, APRN, CNS,
FAAN, the Academy’s Choosing Wisely chair, talk
about the campaign’s progress and next steps.
How have the Academy and nurse experts
contributed to Choosing Wisely?
ANA-Michigan and ANA are empowering nurses
Support ANA and state Advocacy efforts, which
help protect your job, your safety and your rights
as a nurse. The voice of nursing grows stronger
when ANA-Michigan and ANA together speak
out on today’s crucial issues.
Cox: The Academy
accepted ABIM’s challenge to
be the nursing profession’s
champion of the Choosing
Wisely campaign because
of its good fit with the
Academy’s mission of
advancing health policy,
practice, and science.
Academy fellows represent
an unparalleled brain trust of nursing’s most
accomplished experts. Many of them are
leaders of clinical nursing organizations that
also partnered with the Academy to develop our
Choosing Wisely statements.
What role do nurse leaders need to play in the
campaign?
Cox: Some statements reach across all practice
areas. Others are specialized. Clinical leaders need
executive leadership awareness and support to
New Lower Dues –
Only $15/month or $174/year!
Joint members in ANA-Michigan and the
American Nurses Association (ANA) is now just
implement the statements at the point of care
since the statements directly impact quality and
safety outcomes.
What do the nursing statements focus on?
Tracy: So far we’ve issued 25 statements on
things nurses and patients should question. They
span a wide range of clinical situations, including
fetal heart rate monitoring, ambulation and
restraints for older adults, not waking patients
for routine care, and treatment of delirium.
Hair removal at surgical sites and unwarranted
continuous vital signs monitoring in hospitalized
children and adolescents are also included.
How can nurse leaders foster collaboration on
these issues with other clinicians?
Cox: Leaders can raise awareness in clinical
areas within facilities and across health systems.
They can support collaborative initiatives to
change practice habits based on the Academy’s
Choosing Wisely statements.
What steps are needed for implementation,
research, and dissemination of the nursing
recommendations?
Tracy: The statements are evidence-based. Now
we need evidence to show what works and what
doesn’t in implementing these recommendations.
For example, a joint effort by 16 advanced
practice registered nurse teams in 13 states
implemented high-value care initiatives based on
initial recommendations from ABIM’s Choosing
Wisely campaign. We need similar projects for
the Choosing Wisely statements the Academy has
developed for nursing.
Why SKLD?
More like a hospital.
SKLD is more like a hospital than a typical nursing home.
From the scope and caliber of our staff, to our clinical capabilities and our ability to honor personal preferences, SKLD is a cut above.
We offer services and use technologies rarely found in skilled care facilities. From the moment you arrive, it’s clear we’re different.
At SKLD, our care – and our culture – are empowered by Illuminate HC.
Illuminate HC is leading the way in modernizing skilled care.
Driven to be a catalyst of change in a healthcare setting that has been slow to evolve, Illuminate HC created the industry’s leading
Research and Development Lab. More than a place, it’s a mindset of innovation and a hive for collaboration with people inside and
outside of health care.
The results? New solutions to age-old problems. New staffing models. New technologies. New thinking that is improving care and
delighting patients.
Illumination in Action
Illuminate HC works shoulder-to-shoulder with SKLD to elevate patient and family experiences and to nurture a great environment
for staff. Our patients – and our care teams – benefit from Illuminate HC’s ground-breaking ideas, proprietary technologies, unique
service and training programs, and practical, onsite expertise. Their laser focus on continuous improvement, accelerates our capacity to
provide a level of care, comfort and convenience rarely found in nursing homes.
Here are some of the ways SKLD care differs from most:
• We utilize SNFists. A SNFist is a doctor who manages the care of people while they are in a skilled facility. Our SNFists round on patients, lead
care teams, coordinate and communicate with patients, families, hospitals and personal physicians. They get to know their patients, so even small
changes in their health can be noticed early and treated quickly.
• Our nurses can spend more uninterrupted time with their patients because we use unique staff positions to support them. We don’t staff to meet
ratios, we staff to meet needs. And, one of our most important needs is to maximize the time our nurses can spend with patients and families
• We also have a pharmacist on our care team. Most nursing homes rely upon a monthly visit from a pharmacy consultant. At SKLD, we value
the added layer of medication management and oversight an on-staff pharmacist provides. This is critically important for patients with chronic
conditions, complex medication needs and to assure medication protocols change as medical conditions change.
• We use technologies most facilities simply won’t invest in. Like:
- A best-in-class electronic medical record system that supports patient needs before, during and after SKLD care
- A one-of-a-kind communication and service platform that allows SKLD patients to arrange their day, their way
- A patented, award-winning logistics system that allows us to communicate via wearable devices, so we can respond to patient needs quickly
and consistently with the most appropriate staff person
- An app our employees can use to communicate across teams or across facilities and to provide easy, 24/7 access to information and systems
that help them do their jobs
• We don’t simply want the best care teams, we actively cultivate a culture and compensation strategy to attract and retain the best.
• We LOVE our work and it shows in the attitude of our caregivers and in the atmosphere they create for patients and their families.
