President’s Pen<br />

The <strong>Kentucky</strong> <strong>Nurse</strong>s Association (KNA) continues<br />

to serve as the professional association for <strong>Kentucky</strong>’s<br />

nurses. Activities include those that highlight issues<br />

impacting nurses, our ability to provide safe and quality<br />

care as well as education and training that allow this level<br />

of care in all settings. We also are committed to offering<br />

a full array of scholarship opportunities that provide a<br />

solid and enduring foundation for nursing practice. Your<br />

KNA is busy addressing these areas of focus:<br />

First, the KNA School <strong>Nurse</strong> Chapter worked with<br />

me to compose an Opinion Editorial (Op-Ed) that we<br />

submitted to newspapers across the state. The Courier-<br />

Journal (Louisville) published our letter on August 13. I<br />

hope you were able to read it, but if not, it is included<br />

in this edition of <strong>Kentucky</strong> <strong>Nurse</strong> on page 8, or you may<br />

also visit www.courier-journal.com. In the article, we<br />

emphasize the importance of having a nurse in every<br />

school not only as a safety issue for children with health<br />

concerns, but also as an indisputable link with students<br />

who may need a trusted adult to listen and intervene.<br />

Please take time to read this piece and contact your state<br />

senator or representative and encourage them to ensure<br />

that Medicaid funding is used to support measures that<br />

will allow nurses to practice to the full scope of their<br />

licensure. Our children deserve this care and attention.<br />

Next, KNA members are working hard to develop<br />

and provide education that targets practice issues facing<br />

<strong>Kentucky</strong> nurses. These educational activities bring local<br />

solutions to local problems, an approach that is often<br />

not available through continuing education courses. The<br />

KNA Summit, scheduled for Friday, <strong>October</strong> 25 at the<br />

St. Elizabeth Training and Education Center in Erlanger,<br />

Ky., will focus on workplace safety disrupted by violence<br />

against nurses. The goal of this Summit, “Creating a<br />

Safe and Caring Workplace,” is to take away the power<br />

that is present in the workplace violence problem and<br />

move it to the viable solution. Individuals with real world<br />

experience addressing the spectrum of this issue will<br />

share their experiences and interventions. By coming<br />

together to talk about shared challenges and concerns,<br />

we can build workable and sustainable solutions. Our<br />

patients deserve safe and quality care and the nurses<br />

providing that care should expect the same. The Summit<br />

will also include a poster session the evening before<br />

(Thursday, <strong>October</strong> 24) and a discussion about KNA<br />

growth and bylaws at the<br />

business meeting. Please<br />

attend to influence our<br />

growth and direction. For<br />

information regarding the Ruth Carrico<br />

Education Summit and how<br />

to register, visit the <strong>Kentucky</strong><br />

<strong>Nurse</strong>s Association website at kentucky-nurses.org. The<br />

KNA will provide continuing education credits for both<br />

the Summit and the Poster Session, just in time to meet<br />

the requirements for the new licensing period ending on<br />

Thursday, <strong>October</strong> 31. See more information on page 6.<br />

KNA members most recently worked with our<br />

philanthropic arm, the <strong>Kentucky</strong> <strong>Nurse</strong>s Foundation<br />

(KNF), to host the Second Annual “A Taste of Health”<br />

fundraiser to benefit nursing education, scholarships and<br />

research throughout the Commonwealth. Kentuckians<br />

recognize that we are a “foodie” state, so showcasing<br />

more than 25 different restaurants is a natural! More<br />

than 150 KNA members and friends attended this great<br />

event. I am proud to report that the evening was again a<br />

success; check out highlights on page 13. If you missed<br />

the opportunity to attend, you can still visit our website<br />

to purchase chances on our vacation raffles that include<br />

a Vancouver fishing trip, a wine tasting and travel event<br />

and a golf and spa getaway.<br />

Finally, KNA receives numerous requests to<br />

participate as an active partner with other agencies<br />

and organizations sharing interests in nursing practice,<br />

patient care and health outcomes. At present, we are<br />

working to develop ideas regarding how to best meet<br />

these requests and continue to expand the voice of<br />

<strong>Kentucky</strong> nurses statewide. If you are interested in being<br />

part of this broad representation, please reach out to me<br />

org. If you want to be part of this initiative or want to<br />

KNA Bluegrass<br />

The Bluegrass Chapter recently hosted a CE Program,<br />

“Vaccines and their Role in Preventing Outbreaks.” The<br />

chapter is working on membership recruitment and<br />

retention activities for the Fall.<br />

KNA Green River<br />

Plans for the next NCLEX Challenge are underway.<br />

The Green River Chapter will host this annual event on<br />

Thursday, March 26, 2020 at the Preston Arts Center,<br />

2660 Green St., Henderson, Ky.<br />

The chapter recently awarded pre-RN, BSN and MSN<br />

scholarships to Keely Newman, ADN, Andrea Williams,<br />

BSN and Callie Bell, MSN, at its September meeting.<br />

KNA Heartland Chapter<br />

The Heartland Chapter recently hosted its September<br />

meeting and is gearing up for Fall activities. To learn<br />

KNA Nightingale Chapter<br />

At the Nightingale Chapter meeting in August,<br />

Pat Calico presented, “Nursing Perspectives on Pain<br />

Management” and Catherine Weaver will facilitate<br />

“Nursing and Integrative Interventions: You Want to<br />

Do What?!” at the chapter meeting, 6:30–7:30 p.m. on<br />

Tuesday, <strong>October</strong> 15 at Ephraim McDowell Hospital, 213<br />

South Third St., Danville.<br />

KNA Northern Chapter<br />

At its June meeting, Terry Foster facilitated “<strong>Nurse</strong><br />

Burnout: Creating Healthy Work Environments and<br />

in September Mary Price presented, “Motivational<br />

Interviewing.” On the evening of Wednesday, December<br />

11, Kristy Brandubur will discuss being an End-of-Life<br />

Doula. The Northern Chapter hosts its meetings at<br />

the St. Elizabeth Training and Education Center, 3861<br />

Olympic Blvd., Erlanger; all are welcome to attend.<br />

KNA REACH Chapter<br />

The REACH Chapter meets on the Second Tuesday of<br />

the month at 5 p.m. (CT):<br />

• November 12 at Greenview Regional Hospital;<br />

the Chapter will collect donations for Toys for<br />

Tots;<br />

• February 11 at SKYCTC Bowling Green campus<br />

and will collect donations for a local Food Pantry<br />

and<br />

• April 14 at the WKU Health Science complex in<br />

Bowling Green; the Chapter will collect donations<br />

for the Camp for Courageous Kids.<br />

REACH chapter members recently participated in the<br />

South Central <strong>Kentucky</strong> Walk to End Alzheimer’s and the<br />

Special Olympics state softball tournament. Members<br />

will soon participate in the American Heart Association<br />

Walk.<br />

The Chapter will host an NCLEX review night for<br />

students in <strong>October</strong> – TBA.<br />

KNA River City<br />

The KNA River City Chapter and the KNA Ethics<br />

& Human Rights Committee will partner with Galen<br />

College of Nursing to present an “Ethics Forum<br />

Webinar,” 2–4 p.m., September 27 at Galen, 1031 Zorn<br />

Ave., Louisville. The webinar will also be available online<br />

after September 27. The KNA will offer 2.4 contact<br />

hours via KBN and Galen will offer ANCC credits.<br />

The Chapter is also making plans for another paint<br />

party. Look for more details on the KNA website.<br />

At the Second Annual <strong>Kentucky</strong> <strong>Nurse</strong>s<br />

Foundation (KNF) Taste of Health more than<br />

150 guests enjoyed delicious food, bid on great<br />

auction items, purchased chances to “Split the<br />

Pot” and walked away with other great packages<br />

like “A Cake-a-Month” for a year or U of L and UK<br />

fan baskets. This event, hosted at the University<br />

of Louisville and Kosair Charities Clinical and<br />

Transitional Research Building, netted more than<br />

$8,000 to benefit nursing education, scholarships<br />

Ongoing fundraising initiatives include Amazon<br />

Smile, a program that allows shoppers to sign up<br />

(at no cost) to donate a small percentage of every<br />

purchase to a charity such as KNF. Remember to<br />

Legislative Update<br />

KNA counts legislative “wins” from <strong>2019</strong> session<br />

Looking back at the <strong>2019</strong> Legislative Session reminds us about the importance<br />

of advocating for issues that affect nurses, those in their care and the health of all<br />

Kentuckians. KNA members left no stone unturned during the <strong>2019</strong> session and<br />

counts these “wins” that will benefit the health of all across the Commonwealth:<br />

SUPPORT - NURSING PRACTICE<br />

SB 1 – School safety bill includes nurses as additional mental health workforce in<br />

the schools; adds a school nurse to Center for School Safety board. Result – amended<br />

version passed, signed into law by the Governor.<br />

SUPPORT – TOBACCO CONTROL/SMOKING CESSATION<br />

HB 11 – Prohibits tobacco use in schools and on public school property and<br />

activities (K-12). Result – passed, signed into law by the Governor<br />

SUPPORT – NO TAXES ON NON-PROFITS<br />

HB 354 – Exemption of taxes on sales of admissions for nonprofit organizations.<br />

School safety has garnered much attention in recent<br />

years. The passage of Senate Bill 1, the “School Safety<br />

Bill,” in our state demonstrates the type of positive<br />

improved security and support for children. To address<br />

this issue comprehensively, the conversation must<br />

expand to embrace physical health as well. “School<br />

health and safety” is terminology more inclusive of<br />

appropriately addressing safety at school. According to<br />

the <strong>Kentucky</strong> Department of Education, nearly 651,000<br />

children attended school in 173 public school districts<br />

across <strong>Kentucky</strong> in 2017-18. More than 120,000 of<br />

these children had chronic health conditions such as<br />

asthma, diabetes, seizures or life-threatening allergies.<br />

These numbers were higher than the estimated 13-<br />

18% reported nationally. Despite the significant number<br />

of children with medical concerns, there is no state<br />

requirement for <strong>Kentucky</strong> schools to employ nurses.<br />

The National Association of School <strong>Nurse</strong>s<br />

recommends that, “to optimize student health, safety<br />

and learning, a registered school nurse be present in<br />

every school all day, every day.” The American Academy<br />

of Pediatrics echoes this by saying that nurses are in<br />

a “critical position to address unmet health needs”<br />

and supports placement of a professional nurse in<br />

every school. Sadly, <strong>Kentucky</strong> has no reported data<br />

on the number of nurses in our schools; nor are there<br />

requirements for school districts to employ nurses. As<br />

schools have faced budget shortfalls, nursing positions<br />

have been cut leaving vulnerable children to receive care<br />

from unlicensed staff such as school clerks or secretaries<br />

who have multiple additional responsibilities. Often<br />

teachers spend time caring for sick or injured students or<br />

addressing chronic conditions that take time away from<br />

teaching. Recently, California teachers went on strike,<br />

and among their demands was the inclusion of funding<br />

for a nurse in every school. Educators across the country<br />

are recognizing that the expectations placed on them<br />

are greater than is reasonable or in the best interest of<br />

children.<br />

Care for the mental health of children was recognized<br />

in SB 1, and to help fund the mental health requirements<br />

of the bill, the Cabinet for Health and Family Services<br />

submitted a state plan amendment to the federal<br />

Medicaid program. If approved, it will expand the<br />

scope of reimbursable health services that schools can<br />

provide. While this revenue will help support properly<br />

credentialed mental health providers, it is also an<br />

occasion for schools to increase access to school nursing<br />

services. The <strong>Kentucky</strong> Department of Medicaid can<br />

decide how these funds will be used for nursing, and<br />

they must support measures which will allow nurses to<br />

practice to the full scope of their licensure. States such<br />

as Massachusetts, Louisiana and South Carolina have<br />

developed such models and <strong>Kentucky</strong> should do the<br />

same. Keeping children safe at school is much broader<br />

than just securing the perimeter and increasing training.<br />

Parents send their children to school expecting that<br />

they will be in a safe and caring environment. To provide<br />

such a setting effectively, we need nurses in every<br />

<strong>Kentucky</strong> school – children across the Commonwealth<br />

are depending on it.<br />

• Register today for the KNA Annual Summit,<br />

“Creating a Safe and Caring Workplace” on<br />

Friday, <strong>October</strong> 25 at St. Elizabeth Training<br />

& Education Center (SECTEC) in Erlanger, Ky.<br />

We’ll also host a poster session the evening<br />

before on <strong>October</strong> 24 at SECTEC. Read more<br />

• As we speak, we are preparing to launch<br />

our Second Annual Dean’s Membership<br />

Challenge. The KNA will present a trophy to<br />

the winning school.<br />

retain KNA members.<br />

• Thanks to everyone who made our Second<br />

Annual “A Taste of Health” fundraiser a big<br />

Established in 1906, the <strong>Kentucky</strong> <strong>Nurse</strong>s Association (KNA) is the only full-service<br />

member-driven professional organization for the state’s entire nurse population. In<br />

The <strong>Kentucky</strong> Board of Nursing (KBN) is an agency of the Commonwealth of<br />

<strong>Kentucky</strong>, governed by the <strong>Nurse</strong> Practice Act, established in 1914. The Board is a<br />

separate and distinct entity from any professional association. The Board’s role is to<br />

protect public health and welfare by developing and enforcing state laws governing<br />

the safe practice of nursing. The Board is self-supporting through fees assessed for<br />

professional licensure. The agency receives no general fund tax appropriation.<br />

The KBN:<br />

• Grants licensure to practice nursing to qualified applicants. (LPN, RN and<br />

APRN).<br />

• Credentials Sexual Assault <strong>Nurse</strong> Examiners (SANE) and dialysis technicians<br />

(DTs).<br />

• Interprets laws and regulations related to the legal scope of practice.<br />

• Develops regulations (rules) necessary to implement its statutory responsibilities<br />

and duties.<br />

• Establishes minimum standards of nursing practice.<br />

• Sets minimum standards for nursing education and maintain list of approved<br />

schools.<br />

• Defines and describes nonprofessional conduct.<br />

• Takes disciplinary action on licensees in response to nonprofessional conduct.<br />

• Maintains a list of names, addresses and licensure status of persons they<br />

license.<br />

• Approves applications for prescriptive authority for advanced practice nurses.<br />

• Provides scholarships through collected licensure fees.<br />

• Maintains registry of individuals who are credentialed as state registered<br />

nursing assistants (SRNAs).<br />

Intentional rounding is a systematic method of checking on hospitalized<br />

patients at specific intervals to reduce adverse events, anticipate needs,<br />

provide comfort and decrease patient anxiety. The concept behind intentional<br />

rounding was the provision of a holistic approach that meets both medical<br />

and psychological needs of the patient. But does it actually make a difference?<br />

A recent systematic literature review was conducted to examine studies that<br />

considered the effects of intentional rounding in context of the nurse and<br />

patient experience. A total of 18 mixed-method studies were reviewed. The<br />

studies utilized both quantitative data (numbers) and qualitative (words)<br />

methods.<br />

Intentional rounding was found to have overall positive implications for<br />

patients, staff, and healthcare organizations.<br />

It was shown that patient satisfaction levels increased in most of the studies<br />

because of intentional rounding. There was a statistically significant decrease<br />

in fall rates in six of the seven studies that considered fall rates. Hospital<br />

acquired pressure ulcers were only mentioned in two of the studies, but nurses<br />

reported a decrease. In studies that mentioned call light usage, there was an<br />

initial reduction, but there were mixed results on its significance as the weeks<br />

proceeded.<br />

Multiple studies in the review examined nurses’ perception of intentional<br />

rounding. All in all, intentional rounding was perceived as beneficial to nurses,<br />

nurse managers and other non-licensed healthcare workers. Newer nurses and<br />

those with an associate’s degree had a better outlook on intentional rounding<br />

as compared to nurses who were more experienced and had a bachelor’s<br />

degree. Day-shift nurses felt they had more support in being able to perform<br />

intentional rounds when compared to those who worked night shift.<br />

How intentional rounding was introduced, the degree of staff involvement,<br />

as well as the willingness to accept intentional rounding played important roles<br />

in its success. <strong>Nurse</strong>s believed that intentional rounding introduced the right<br />

way increased staff efficiency, improved time management and decreased<br />

interruptions. If the implementation of intentional rounding was not carried<br />

out in an effective way, it was perceived as frustrating, time-consuming,<br />

intrusive and degrading to nurse’s decision-making skills and independence.<br />

The results of this systematic review suggest that intentional rounding<br />

does, in fact, improve patient satisfaction. It has also been shown to<br />

decrease call bell usage, pressure ulcer incidence and fall rates. However,<br />

intentional rounding should be used as a guide, personalized to the patient<br />

and customized to the specific unit. More discussion on who performs the<br />

rounds, how often the rounds are completed and what methods are used to<br />

carry out the rounds are needed. Future research should be performed on the<br />

exact execution of intentional rounds and studies that show how the evidence<br />

is obtained would be beneficial to the success of intentional rounding on all<br />

units.<br />

Proper education to introduce intentional rounding to the unit should be<br />

done as well as continuing education to ensure that nurses understand how<br />

it should be executed. Holding nurses accountable for performing intentional<br />

rounding is crucial to its long-term effectiveness. The way that the rounding<br />

should be documented is unclear and needs a more specific approach. By<br />

having a unit-specific process for performing intentional rounding, it is more<br />

likely that the actions will be carried out and patient safety and satisfaction will<br />

be increased.<br />
