The Services to Improve Access PCW – Partnership<br />
Community Worker Service is focused on ensuring<br />
eligible Maori, Refugee, Migrant and low income people in<br />
Canterbury, have access to primary health care services.<br />
The PCW role based in Lincoln, servicing Ashburton.<br />
The duties and responsibilities include:<br />
• Assess and identify needs impacting client health and<br />
wellbeing<br />
• Provide information and education about support and<br />
resources available in the community<br />
• Facilitate access to health, social services and<br />
community support services<br />
• Support people to attend appointments if necessary<br />
• Accurate, timely management of client information and<br />
reporting<br />
Skills and experience<br />
• Relevant tertiary qualification (eg Social Work) and/or<br />
relevant experience within the health sector.<br />
• Has strong relationship building and networking skills<br />
• Creative problem solver and very organised<br />
• Culturally competent<br />
• IT competent<br />
