Just a few weeks ago<br />

I was talking to several<br />

freshly graduated nursing<br />

students, who were<br />

excitedly sharing their future<br />

plans beyond the NCLEX<br />

with me. It was interesting to<br />

hear of the paths that they<br />

were laying out; each one Kelly Bouthillet<br />

different, but clearly planned<br />

out with various goals in mind. One thing that struck<br />

me was that none of these graduates included<br />

joining professional nursing organizations or finding<br />

a mentor to help them navigate their profession or<br />

transition from student to novice nurse.<br />

If you are reading this message, you are likely<br />

a member of the SCNA or other membership<br />

association and understand the value of belonging<br />

to a professional nursing organization and having<br />

a support system; especially during a time such as<br />

this. There has been much focus on the long-term<br />

impact of the pandemic on the profession and what<br />

can be done to mitigate some of the insults that are<br />

currently happening. While I do not claim to have all<br />

the answers, I do believe that nursing is a unique<br />

community that relies on each other and has the<br />

power to combat some of the negative effects of<br />

COVID while helping the next generation of nurses<br />

find their way (See The Nursing Tribe, SC <strong>Nurse</strong>s<br />

March/May/April 2019).<br />

Staying grounded and focused this past year has<br />

been a challenge for many of us and continues still<br />

both in our personal and work lives. I appreciate the<br />

nursing village, both near and far to call upon for<br />

support. Here are my top five reasons for belonging<br />

to professional nursing organizations:<br />

1. Networking and camaraderie (support)<br />

2. Advocacy<br />

3. Mentorship<br />

4. Leadership development<br />

5. Promote clinical excellence<br />

There are many more reasons and each of us<br />

has their own personal incentives for being part of<br />

nursing organizations. When we work with others<br />

toward common goals, we achieve much more than<br />

the goal itself. And during these past 18 months, that<br />

has come to be extremely meaningful.<br />

As we approach autumn and fall full swing into<br />

work and school schedules and prepare for our<br />

about the ways you can support the profession and<br />

other nurses. There is much we can do to improve<br />

the lives of our colleagues and assist those aspiring<br />

to join the ranks those that call themselves nurse.<br />

to be with her beloved<br />

husband, Gerald D.<br />

Riddle “Poppy”, of<br />

almost 53 years. We<br />

find strength knowing<br />

she was met by her<br />

husband, her Lord and<br />

Savior, along with her<br />

parents, Claude E.<br />

Knight and Mildred B. Knight, at the heavenly gate.<br />

Mrs. Ellen was an avid Christian and a member of<br />

Beulah United Methodist Church in Gilbert, SC for<br />

30 years. Mrs. Ellen was a beloved daughter, mother,<br />

grandmother, great-grandmother, sister, and nurse<br />

practitioner of 30 years. She worked for Lexington<br />

Medical Center in Labor & Delivery and then went<br />

into private practice with Dr. Phillip Levin at Midland’s<br />

OBGYN. Mrs. Ellen loved every patient and their<br />

babies as her own. She was loved by all who knew<br />

her and will be missed dearly. Nana Ellen is survived<br />

by her beloved children, Angela Patnaude, Andy<br />

Riddle (Jackie), and Tori Riddle; her grandchildren,<br />

Rachel Cohn (Robert), Bruce Patnaude, and Kelly<br />

Cox (Jarret); and her great-grandchildren, whom she<br />

loved more than her own children most days, Josie<br />

Cox, Sadie Cohn, Claudia-Joy Shaw, Stella Cohn,<br />

and Sebastian Shaw. She is also survived by her<br />

nieces and nephews, Kendrea Knight, Kenzie Shaw<br />

(Aaron), Cla Knight, and Allison McMahan. She is<br />

also survived by her brother, Ken Knight (Deanne)<br />

and sister, Carolyn McMahan (Joe). She leaves<br />

behind several brothers-in-law, sisters-in-law, nieces<br />

and nephews on her husband’s side.<br />

A memorial service will be held 11:00 a.m.,<br />

Saturday, <strong>September</strong> 4, <strong>2021</strong> at Beulah United<br />

Methodist Church in Gilbert, SC. The family asks that<br />

any donations in her honor be made to the church.<br />

variant caused a surge in cases. 3 This shortage<br />

may become more acute as the state deals with<br />

the aftermath of Hurricane Ida.<br />

On behalf of the American <strong>Nurse</strong>s Association<br />

• To address workforce challenges, Nebraska is<br />

recruiting unvaccinated nurses. 4<br />

(ANA), I write to urge robust and immediate action to<br />

address the unsustainable nurse staffing shortage<br />

facing our country. <strong>Nurse</strong>s have remained steadfast<br />

on the front lines since the beginning of the pandemic,<br />

COVID-19 pandemic has caused 39% of nurses<br />

ages 20 – 39 to report that their commitment to<br />

nursing had decreased.<br />

health burden of the COVID-19 virus. Now, the Delta<br />

variant is causing cases to soar, overrunning hospital<br />

nurses alone can solve. As such, it is crucial that<br />

the Administration mount a whole of government<br />

approach by convening nurses, hospitals, physicians,<br />

other health care personnel, state and federal<br />

have only exacerbated underlying, chronic nursing government officials, and key stakeholders to<br />

workforce challenges that have persisted for years.<br />

Since the nation began COVID-19 mitigation and<br />

response efforts, much focus has been placed on<br />

nurses facing shortages of equipment to appropriately<br />

care for patients. Now, it is imperative that the<br />

Administration acknowledge and take concrete steps<br />

to address a more dire shortage: a crisis-level human<br />

resource shortage of nurses that puts our ability to<br />

care for patients in jeopardy.<br />

ANA is deeply concerned that this severe shortage<br />

of nurses, especially in areas experiencing high<br />

numbers of COVID-19 cases, will have long-term<br />

repercussions for the profession, the entire health<br />

care delivery system, and ultimately, on the health of<br />

the nation. To address this crisis and to ensure<br />

that we have a strong nursing workforce for the<br />

examine, identify, and then implement real solutions to<br />

nursing shortages, including:<br />

1) Addressing the current fatigue and mental<br />

wellbeing of nurses<br />

2) Developing strategies to retain the current<br />

nursing workforce<br />

3) Working with CMS to adopt new payment<br />

methodologies that recognize the value that<br />

nurses bring to patient care<br />

4) Removing barriers to practice faced by nurses<br />

5) Addressing the persistent barriers that limit the<br />

number of qualified nursing students that can<br />

be educated each year<br />

6) Building and maintaining a resilient workforce<br />

to meet our country’s current and future health<br />

care needs.<br />

national nurse staffing crisis and take immediate<br />

steps to develop and implement both short- and<br />

long-term solutions.<br />

Shortages of nursing staff are being reported<br />

across the country:<br />

• Mississippi has reported that it has seen a<br />

decrease of 2,000 nurses since the beginning<br />

of <strong>2021</strong>.<br />

Enclosed please find an initial outline of ANA’s<br />

proposed policy solutions to address the nurse staffing<br />

crisis that we hope will help form the foundation of<br />

conversations convened by HHS on this topic.<br />

ANA is the premier organization representing<br />

the interests of the nation’s over 4 million registered<br />

nurses (RNs), through its state and constituent<br />

member associations, organizational affiliates, and<br />

• Hospitals in Tennessee are operating with<br />

1,000 fewer staff than at the beginning of the<br />

pandemic, prompting them to call on their<br />

individual members. ANA members also include<br />

the four advanced practice registered nurse roles<br />

(APRNs): nurse practitioners (NPs), clinical nurse<br />

and certified registered nurse anesthetists (CRNAs).<br />

ANA advances the nursing profession by fostering<br />

high standards of nursing practice, promoting a<br />

safe and ethical work environment, bolstering the<br />

health and wellness of nurses, and advocating on<br />

health care issues that affect nurses and the public.<br />

RNs serve in multiple direct care, care coordination,<br />

and administration leadership roles, across the full<br />

spectrum of health care settings. RNs provide and<br />

coordinate patient care, educate patients and the<br />

public about various health conditions including<br />

essential self-care, and provide advice and emotional<br />

support to patients and their family members.<br />

HHS as the nation continues to combat the COVID-19<br />

ANA’s proposed policy solutions to address the<br />

HHS Must Convene Stakeholders to Identify<br />

Short- and Long-Term Solutions to Staffing<br />

Challenges.<br />

ANA urges HHS to convene all stakeholders<br />

for a robust discussion of staffing challenges and<br />

potential solutions. These challenges are not solely<br />

contained within the nursing profession, especially<br />

as the health care delivery system continues to<br />

evolve towards a more integrated system. As such,<br />

it is crucial that the agency convenes, in addition<br />

to nurses, hospitals, physicians, other health care<br />

personnel, state and federal government officials,<br />

and key stakeholders to examine, identify, and then<br />

implement real solutions to the nursing shortage.<br />

The focus of these discussions must be to identify<br />

the current challenges and both short- and longterm<br />

solutions. Short-term solutions will allow us<br />

to adequately face the demand of the COVID-19<br />

pandemic response. Long-term solutions will ensure<br />

the nation’s health care delivery system is best<br />

equipped to provide quality care for patients and<br />

stands ready for the future challenges. ANA implores<br />

HHS to have these critical conversations and stands<br />

ready to work with the agency to facilitate them.<br />

HHS must work with CMS to take steps<br />

to appropriately acknowledge nurses in<br />

reimbursement methodologies, ensuring payment<br />

equity for nursing services provided to patients.<br />

The COVID-19 pandemic response has made<br />

clear that APRNs and RNs are indispensable to<br />

providing the care that patients need now and in the<br />

future. For instance, APRNs are a significant source<br />

of primary care, especially in rural and underserved<br />

areas. Further, RNs are responsible for a wide array<br />

of direct care and care coordination services in<br />

community settings as well as hospitals and longterm<br />

care facilities. These health care services are<br />

key in ensuring access to care, a critical aspect of<br />

addressing health inequity. However, there must be<br />

parity in how these vital services are reimbursed.<br />

Recognition through appropriate payment for<br />

nursing services is critical in ensuring a resilient<br />

nursing workforce ready and able to meet future<br />

needs. It is long overdue for nursing services to<br />

be separated from “room and board,” as currently<br />

considered by the Medicare program. <strong>Nurse</strong>s provide<br />

vital services to patients across the care continuum<br />

and the health care delivery system must recognize<br />

their critical role through appropriate reimbursement.<br />

ANA urges HHS to work with CMS to consider<br />

methodologies and approaches that will ensure<br />

payment equity for nursing services.<br />

In light of the rise of Coronavirus variants<br />

and increased COVID-19 contraction, HHS<br />

must provide additional resources including<br />

recruitment and retention incentives and support<br />

to bolster the nursing workforce to meet current<br />

demands for critical health care services.<br />

Hospitals are quickly reaching capacity limits due<br />

to the surge of COVID-19 cases and the nursing<br />

shortages across the country. It is imperative that HHS<br />

continue its thoughtful pandemic leadership and utilize<br />

all available authorities to address this issue. <strong>Nurse</strong>s<br />

are still in need of resources to combat the pandemic<br />

and ANA continues to call on the Administration to act<br />

in response. Standing on the front lines, our nation’s<br />

nurses are becoming increasingly burned out as the<br />

pandemic continues to weigh heavily on them. We are<br />

seeing large numbers of nurses leaving the profession<br />

as a result. This only results in further strain on the<br />

nursing workforce, which was already in a supply crisis<br />

before the pandemic.<br />

ANA appreciates the Administration’s thoughtful<br />

pandemic response to date through issuance of<br />

waivers and other resources to bolster nurses’<br />

ability to provide vital health care services amid the<br />

challenges faced by the health care system.<br />

ANA urges HHS to remove unnecessary<br />

regulatory barriers to APRN practice<br />

In various ways, certain Medicare payment rules<br />

restrict APRN practice above and beyond their<br />

state scope-of-practice rules. Examples include<br />

unnecessary supervision requirements, as well as<br />

payment restrictions for certain Medicare services<br />

provided by APRNs. Such restrictions limit access to<br />

care and beneficiaries’ choice of qualified provider.<br />

Several of these federal practice restrictions have<br />

been waived during the COVID-19 public health<br />

emergency (PHE). As experiences resulting from<br />

these waivers demonstrate, allowing APRNs to<br />

practice to the full extent of their state license<br />

translates to needed system capacity and expanded<br />

access for patients. We continue to call on CMS to<br />

grant permanent regulatory relief for APRN practice,<br />

so that access is not constricted when the PHE ends.<br />

HHS must continue to educate the nation on<br />

the importance of the COVID-19 vaccine and<br />

provide support and resources for widespread

administration of the vaccine and any<br />

subsequent boosters.<br />

Vaccines are critical to the control and prevention<br />

of infectious disease transmission. <strong>Nurse</strong>s play a<br />

critical role in educating the public and fellow health<br />

care colleagues, as well as in the administration<br />

of COVID-19 vaccines. Currently, the nation faces<br />

significant vaccine hesitancy while cases of the Delta<br />

variant increase rapidly, straining an already strained<br />

nursing workforce. HHS must continue to provide<br />

resources and support efforts to educate the public<br />

on the importance of getting the COVID-19 vaccine.<br />

In addition, it was recently announced that<br />

boosters for the already vaccinated will be available.<br />

HHS must provide the necessary resources to<br />

states and localities for successful distribution<br />

and administration of the booster vaccine doses.<br />

The agency must also educate the public on the<br />

importance of receiving the booster doses, when<br />

appropriate. The vaccine is an important component<br />

of COVID-19 mitigation efforts—especially in an<br />

effort to keep patients out of hospitals facing capacity<br />

challenges. ANA urges HHS to provide support<br />

and resources for continued education and ongoing<br />

distribution and administration of the vaccine across<br />

the country.<br />

HHS must ensure a resilient nursing workforce<br />

that meets current and future staffing demands<br />

and ensures access to care for patients.<br />

Prior to the COVID-19 pandemic, nurses already<br />

experienced tremendous levels of stress in their dayto-day<br />

work. The pandemic has further intensified<br />

the feelings of exhaustion, anxiety and being<br />

overwhelmed especially with respect to patients<br />

that are dying and having to inform and comfort<br />

their surviving family members. It is vital the nation<br />

prioritizes the mental health of nurses and other<br />

health professionals who are caring for our most<br />

vulnerable patients. ANA actively advocates to<br />

reduce stigma around seeking help for mental health<br />

and substance use disorders for health professionals<br />

as well as their patients.<br />

Moreover, nurses also must be treated and<br />

compensated appropriately as they provide care<br />

under extraordinary circumstances, so that the<br />

next generation is encouraged to enter the field<br />

and ensure the nation’s readiness for public health<br />

emergencies. Appropriate compensation ensures<br />

that the health care delivery system retains the<br />

nurses needed to provide care to patients. We<br />

are seeing examples throughout the country of<br />

nurses leaving their communities for the higher<br />

compensation offered by travel nurse agencies. This<br />

only serves to further local staffing strains, often in<br />

the most underserved communities.<br />

However, a resilient workforce is achieved not<br />

only by adequate pay, as the working environment<br />

must also allow nurses to flourish in their profession.<br />

<strong>Nurse</strong>s are professionals providing critical health<br />

care services to patients—they should not have<br />

to fight for allotted breaks and other challenges<br />

created byantiquated views of the profession. All too<br />

often, we hear of staffing plans not being enforced,<br />

resulting in long shifts and strains on nurses<br />

providing care. <strong>Nurse</strong>s know best the provisions that<br />

they and their team need, from patient complexity<br />

to layout of the nursing unit. This is just another<br />

instance where health care delivery and outcomes<br />

would be improved by greater nurse involvement. It<br />

is crucial for nurses to take on leadership roles, in all<br />

settings, to meet the demands of our ever-changing<br />

health care system, including being permitted to<br />

practice to the full extent of their education, training<br />

and licensure.<br />

Lastly, the introduction of electronic health records<br />

(EHR) has proved to be burdensome, detracting from<br />

patient-centered care. Documentation and required<br />

recording of various questions is time consuming,<br />

which leaves less time for nurses to connect with<br />

patients. HHS should reevaluate current and future<br />

requirements and ensure the right balance is struck<br />

between the positive impact of EHR in comprehensive,<br />

coordinated care and provider burden.<br />

To ensure a future workforce that meets all the<br />

needs and demands of patient care, it is crucial<br />

that we not only attract students to the nursing<br />

profession but retain skilled nurses throughout<br />

their careers. Effective workforce planning and<br />

policymaking require better data collection and an<br />

improved information infrastructure. ANA encourages<br />

HHS to work with nurses to identify approaches to<br />

bolstering a resilient nursing workforce ready to meet<br />

the demands of today and tomorrow’s health care<br />

delivery system.<br />

Our annual conference will be held in Hilton Head Island, <strong>South</strong> <strong>Carolina</strong>.<br />

With COVID making this year an extremely difficult year for so many, the<br />

APRN chapter is sponsoring a community service project to help underserved<br />

individuals of Hilton Head Island. We have chosen to partner with the Deep Well<br />

Project. The Deep Well Project is a local organization that provides a variety of<br />

resources including food, children’s programs, financial, furniture and basic living<br />

needs to the local community.<br />

We would like to get as much participation as possible and ask that you share<br />

this information with any members. For those that are attending the conference<br />

in person, we ask that you donate supplies or financial donations. Deep Well<br />

provides a monthly list of most needed items which we will post in the coming<br />

weeks. However, several items are staple requests such as toiletries, socks,<br />

diapers and wipes, baby formula, feminine items-maxi pads and tampons, and<br />

non-perishable food items.<br />

The <strong>South</strong> <strong>Carolina</strong> <strong>Nurse</strong>s Foundation Board<br />

of Trustees have been busy reviewing scholarship<br />

applications and are excited to be able to soon release<br />

awardee information. By the time you are reading this<br />

the winners will have been notified.<br />

The Covid variants, the many unvaccinated and<br />

resurgence of the Covid illnesses have stressed<br />

healthcare facilities. The need for more nurses has<br />

never been more relevant than it is now. The <strong>South</strong><br />

<strong>Carolina</strong> Registered Workforce (SCRW) report from<br />

December 2020 demonstrated a steady, slow growth<br />

of nurses by 3.0% since 2016. The problem is the growth does not meet the<br />

increased demand for more nurses. Considering that one-third of nurses are<br />

over the age of 50 (SCRW, 2018). <strong>Nurse</strong>s account for 30% of total hospital<br />

employment.<br />

The need for additional nursing faculty remains a problem. In the American<br />

Association of Colleges of Nursing (AACN) report from 2019-2020 Enrollment<br />

and Graduation in Baccalaureate and Graduate Programs in Nursing, nursing<br />

schools in the U.S. turned away 80,407 qualified applicants form baccalaureate<br />

and graduate nursing programs due to insufficient faculty. In the AACN October<br />

2019 report, there were a total of 1,637 faculty vacancies with a national faculty<br />

vacancy rate of 7.2%. Vacancies requiring a doctoral degree for faculty were<br />

reported to be 89.7%.<br />

Research has demonstrated that the better a nurse is educated, the better the<br />

outcomes are for patients. The future for nursing and nurses has been clearer<br />

than it is today. Education and educational support for nurses is needed. The<br />

<strong>South</strong> <strong>Carolina</strong> <strong>Nurse</strong>s Foundation mission is to assist in meeting this need.<br />

The Foundation needs the support of everyone. If you have ever had your life<br />

touched by a nurse or know of a nurse struggling to achieve that educational<br />

goal, please consider giving to the Foundation. This can be done in many ways.<br />

The license plate initiative shows the world you are a nurse, or you support<br />

nursing, attendance at Palmetto Gold, buying scrubs from “Just Cause Scrub”<br />

and choosing SCNF as your “Just Cause”, and giving during the Midlands Gives<br />

event supports nursing scholarships. The “Friends of the Foundation” help defray<br />

the cost for supplies for the Foundation.<br />

ANA joined the World Health Organization and<br />

other global colleagues in extending the Year of<br />

the <strong>Nurse</strong> into <strong>2021</strong>, which builds on the increased<br />

visibility of nurses’ contributions during the COVID-19<br />

pandemic. The importance and contributions of<br />

nurses and the profession of nursing to the delivery<br />

of high-quality and compassionate health care has<br />

of equity and inclusion in the profession of<br />

nursing; and<br />

• Approved a proposal for a new Early Career<br />

<strong>Nurse</strong> Leader Award to recognize significant<br />

Annual Meeting Update<br />

The Annual Meeting of SCNA will take place<br />

virtually on October 14, <strong>2021</strong> at 6:00PM. All<br />

members are welcome and encouraged to attend.<br />

There is no cost to attend the Annual Meeting.<br />

Actions of the Annual Meeting will include the<br />

annual reports to the membership of the Board of<br />

Directors of SCNA and the announcement of the<br />

<strong>2021</strong> election results. All SCNA/ANA and SCNA-<br />

are in attendance at the Annual Meeting may vote<br />

during that meeting. There is no proxy voting. The<br />

Meeting. This process includes any Resolutions not<br />

in the Annual Book of Reports. The Annual Book of<br />

Reports will be posted to the SCNA website no later<br />

than October 6, 2020. Copies of The Annual Book of<br />

Report will be provided to attendees in digital format.<br />

In order to vote in the <strong>2021</strong> SCNA Election you<br />

must be a current member as of June 1, <strong>2021</strong> and<br />

meet the membership requirements for licensure<br />

found in the current SCNA and ANA Bylaws. The<br />

June 1st date was established based in the election<br />

beginning <strong>September</strong> 20, <strong>2021</strong>. The election process<br />

of SCNA requires each eligible voter to be a member<br />

in good standing at least two (2) full months from<br />

the 1st of the month prior to the month the election<br />

be published on the SCNA website prior to the first<br />

day of balloting. It is up to the individual members<br />

to validate the information and to contact the SCNA<br />

staff during regular business hours (as stated on the<br />

website) prior to voting if the member believes that<br />

he/she was omitted from the voting list in error so<br />

that a correction can be made. Any SCNA members<br />

in good standing may present evidence that there is<br />

an error in the list of current members prior to the<br />

voting and up to the closing of the election. Failure<br />

to notify SCNA of an error in the voting list prior to<br />

the closing of the vote is considered a waiver of the<br />

member’s right to challenge the voting list or the<br />

You Need More Folic Acid than You Think<br />

New recommendations for women of childbearing age<br />

The journey to becoming parents did not start as<br />

an easy one for Chris and Alicia Jones of Conway,<br />

SC.<br />

“We suffered the heartache of miscarriage and<br />

the unimaginable pain of burying our first born child,”<br />

recalls Alicia.<br />

In 2007, Joseph “Hamilton” Jones was born at just<br />

over one pound and 12 inches. He had spina bifida,<br />

hydrocephalus, and a heart defect. Hamilton passed<br />

away just 2 hours and 40 minutes later.<br />

“My husband and I were left choosing a small<br />

white casket, not much larger than the size of a<br />

shoebox for him to be buried in,” said Alicia. “It tore<br />

our hearts out leaving the hospital without him.”<br />

Spina bifida, along with anencephaly and<br />

encephalocele, are classified as neural tube defects<br />

(NTDs), anomalies caused by the failure of the<br />

neural tube to close very early in development. In<br />

the late 1980s, it was discovered that folic acid, when<br />

taken as a supplement by women prior to and early<br />

in pregnancy, could reduce the risk for these life<br />

altering, and sometimes life threatening birth defects.<br />

Since that time, the SC Birth Defects Prevention<br />

Program at the Greenwood Genetic Center has<br />

made it their mission to educate all women of<br />

childbearing age on the importance of taking folic<br />

acid.<br />

“Over the past three decades we have had<br />

much success in reducing the incidence of NTDs<br />

in <strong>South</strong> <strong>Carolina</strong>, a state that once had the highest<br />

incidence of these birth defects in the US.” said Jane<br />

Dean, RN, Statewide Coordinator of the Program.<br />

“These educational efforts, along with a surveillance<br />

program, and folic acid fortification in cereal grain<br />

flours have led to a 60% reduction in NTDs in SC.”<br />

That translates to 70 healthy babies born each<br />

year without an NTD.<br />

Through educational campaigns including<br />

billboards, brochures, social media, and attendance<br />

at health and bridal fairs, the program encourages<br />

all women of childbearing age to take 0.4 mg of<br />

folic acid each day. Women who have had a prior<br />

pregnancy affected with an NTD, like Alicia, are<br />

instructed to take even more folic acid, 4 mg/day, to<br />

prevent a recurrence.<br />

“While we have been pleased with the declining<br />

rate of NTDs in SC, we still continue to see<br />

approximately 25 of these potentially preventable<br />

birth defects in our state each year,” said Roger<br />

Stevenson, MD, who oversees the prevention<br />

program. “Failing to consider additional preventive<br />

measures is simply not acceptable.”<br />

Dean and Stevenson’s efforts to monitor NTDs<br />

in SC, which were recently published in the Journal<br />

of Pediatrics (226:186, 2020), led to the conclusion<br />

that for women in a high-risk state like SC, folic acid<br />

fortification in flour and grains does not provide<br />

sufficient protection against many NTDs. Dean and<br />

Stevenson suggest that the rate of NTDs might be<br />

further reduced by increasing the folic acid dosage to<br />

4mg per day, not only for those with a prior affected<br />

pregnancy, but for all women of childbearing age.<br />

“We shouldn’t wait until a family has suffered a<br />

tragic loss, or a child is born with lifelong disabilities<br />

if we can prevent the first one by suggesting a higher<br />

dose of folic acid,” said Dean.<br />

Higher doses of folic acid, a water-soluble B<br />

vitamin, have been shown to be generally safe.<br />

Potential side effects of higher doses such as<br />

diarrhea, skin rashes, and sleep disturbances<br />

have been reported in certain populations, along<br />

with the potential to mask vitamin B-12 deficiency<br />

and lowering the effectiveness of anticonvulsant<br />

medications. (Eur J Clin Nutr 71:159,2017) However,<br />

such risks are uncommon, and others have<br />

suggested that these risks are overstated and there<br />

should be no upper intake level for folic acid. (Public<br />

Health Rec 39:2,2018).<br />

Thus, the SC Birth Defects Prevention<br />

Program proposes that the standard preventive<br />

dose of folic acid for all women of childbearing<br />

age be increased to 4 mg/day during the three<br />

months prior to conception and the first month<br />

after conception.<br />

Currently, women on a typical diet receive<br />

approximately 100-150 micrograms/day of folic acid<br />

through fortification of cereal grain flours. Therefore,<br />

the recommendation to use 4 mg/day of folic acid<br />

remains the same whether or not the woman uses<br />

folic acid fortified food products.<br />

The additional supplementation should be<br />

prescribed as folic acid alone, rather than by<br />

increasing the number of multivitamins, as the<br />

dosage of other vitamins may become excessive.<br />

Women who experience any potential side effects<br />

of high doses of folic acid should discontinue the 4<br />

mg/day dose and return to a multivitamin with 0.4 mg<br />

or a prenatal vitamin with 0.8 mg.<br />

“Following the loss of Hamilton, I enrolled in<br />

GGC’s NTD research program and began taking a<br />

folic acid supplement, increasing my dosage to 4 mg/<br />

day when we began planning to expand our family,”<br />

said Alicia. “We are now the proud parents of three<br />

healthy children, Brynleigh, Preston, and Ellieana.<br />

We are thankful for the research of the Greenwood<br />

Genetic Center and the efforts spent towards<br />

supporting the families affected by genetic defects<br />

and preventing them from occurring in the future.”<br />

Optimizing Tax Strategy & Charitable Giving with a<br />

Donor Advised Fund<br />

CONSIDERING YOUR OPTIONS<br />

When developing a charitable giving strategy,<br />

there are a number of approaches and solutions to<br />

consider, depending on your unique circumstances.<br />

It’s important to understand each of your options in<br />

order to determine the strategy that aligns with your<br />

best interest. Let’s take a look at one option that may<br />

be worth considering—a Donor Advised Fund (DAF).<br />

One of the advantages of employing the use of<br />

a DAF in your charitable giving strategy is that, in<br />

certain scenarios, it can be utilized to separate your<br />

tax strategy from your charitable giving strategy.<br />

Because you always receive the tax benefits of<br />

contributing to a DAF in the year that the gift is made,<br />

this makes a DAF a viable recipient for directing<br />

charitable donations as part of your annual tax<br />

strategy.<br />

At the same time, the distributions from your DAF<br />

to other charitable organizations can be made over<br />

time, in some cases years after the initial gift is made<br />

to the DAF. It is this space between the original<br />

gift and the final distribution that allows for the<br />

separation of tax and charitable planning. Let’s take<br />

a look at three different scenarios to see how this<br />

might work in the real world.<br />

SCENARIO 1:<br />

Mary needs to make a charitable tax<br />

deduction this year, but hasn’t decided on a<br />

designated charity.<br />

In this scenario, our client Mary, is reviewing her<br />

current tax situation with her accountant who has<br />

determined that she should make a $25,000 gift<br />

to charity in order to offset some of her income for<br />

the current year. While Mary would like to make<br />

this charitable donation, she doesn’t currently have<br />

a relationship with any charities and, as a result, is<br />

uncertain where to donate the gift. By donating to<br />

a DAF before the end of the year, Mary receives a<br />

current year tax deduction, then can take time to<br />

research charities of interest to her while her original<br />

gift continues to grow. Over time, she can advise on<br />

how to distribute the funds within her DAF as she<br />

determines charities that are of interest to her.<br />

SCENARIO 2:<br />

Mary needs a large charitable deduction this<br />

year, but doesn’t feel comfortable donating a<br />

large dollar amount to a single charity all at<br />

once.<br />

Mary typically gives to a few small, local charities<br />

and has concerns about their ability to handle a<br />

large donation as well as the perception should<br />

she suddenly write them a large check. In this<br />

scenario, let’s assume that Mary received a windfall<br />

income this year and, as a result, her accountant<br />

recommended a sizeable charitable donation of<br />

$250,000 be made in order to offset some of that<br />

income. Again, Mary can donate to her DAF this<br />

year. Then over a series of years, she can advise<br />

multiple smaller gifts be made to the charities of<br />

her choice. Her privacy is maintained regarding the<br />

windfall event, and the charities can continue to<br />

operate normally without any potential disruption.<br />

SCENARIO 3:<br />

Mary would like to accumulate her annual<br />

charitable gifts over a period of multiple years in<br />

order to make a single, larger gift.<br />

DAFs can also be used to accumulate many<br />

years of smaller charitable contributions in order to<br />

make a single, large donation. In this scenario, Mary<br />

would like to make a sizeable donation to her alma<br />

mater, perhaps with the intent of receiving larger<br />

recognition than she otherwise would receive for a<br />

smaller, annual gift. In this instance, Mary can make<br />

her regular annual charitable contributions to her<br />

DAF and let them grow within the account; then after<br />

several years, when the balance is large enough,<br />

Mary can advise that it be directed toward her alma<br />

mater. With this larger, single gift, Mary can have<br />

more impact than she otherwise might have with a<br />

series of smaller, annual contributions.<br />

A great benefit of incorporating a DAF into<br />

your annual tax and charitable planning is that it<br />

effectively allows you to separate the tax decisions<br />

that need to be made this year from the process<br />

of identifying and directing gifts toward charitable<br />

beneficiaries. In doing so, you can focus on<br />

optimizing your tax strategy in the current year, then<br />

over time, optimize your charitable giving strategy<br />

in a way that makes the most sense for you and the<br />

final recipients.<br />

NURSE LICENSURE COMPACT(NLC)<br />

New Jersey is allowing nurses who hold active,<br />

unencumbered, multi-state licenses issued by <strong>Nurse</strong><br />

Licensure Compact member states to practice in New<br />

Jersey under their multi-state licenses. New Jersey<br />

plans to implement the NLC within their state later<br />

this year (34th state), but an exact date has not been<br />

determined. Guam passed the <strong>Nurse</strong> Licensure Compact<br />

legislation in February becoming the first U.S. Territory<br />

to do so and plans to implement in 2022 (date TBD). In<br />

addition, Vermont passed legislation in <strong>2021</strong> with plans<br />

to implement early in 2022 (date TBD). Most recently,<br />

Pennsylvania and Ohio passed NLC legislation with dates<br />

for implementation to be set for 2022 – 2023, bringing<br />

the total number of states in the NLC to 37 (plus one US<br />

Territory).<br />

PRACTICE PEARLS<br />

• APRNs should submit proof of updated national<br />

certifications to NURSEBOARD@llr.sc.gov. A copy<br />

of current national certification is required to be on<br />

file in order to keep an APRN license active.<br />

• APRNS are required to notify the BON of<br />

any practice changes (new employment,<br />

unemployment, retired, etc.) within 15 business<br />

days.<br />

• APRNs who lose Rx authority eligibility must<br />

request reinstatement when applicable (check box<br />

on change of practice form).<br />

• Rx authority may be reinstated, within 6 months of<br />

the removal date, if all statutory requirements are<br />

met. After 6 months, a new Rx authority application<br />

is required.<br />

• Licensees may view Licensee LookUp for<br />

licensure changes (expiration dates, Rx authority,<br />

collaborating physicians, etc.).<br />

HOW CAN I CHECK A LICENSE?<br />

To check a nursing license, you may utilize one or all of<br />

the following options:<br />

(A) <strong>South</strong> <strong>Carolina</strong> Licensee LookUp – Go to LICENSEE<br />

LOOKUP on the Board of Nursing’s webpage (www.llr.<br />

sc.gov/nurse). It is recommended that only the first and<br />

last name of the nurse be entered. This site will provide<br />

the nurse’s name, city/state, license number/type,<br />

date issued/expiration, license status, and whether the<br />

license is multi-state or single-state.<br />

(B) Nursys Quick Confirm – Go to www.nursys.com and<br />

click on Quick Confirm, following the instructions as<br />

outlined. This site will provide the nurse’s name, state<br />

of licensure, license type/number, license status,<br />

expiration date and discipline status if noted. At this<br />

time, the only states NOT fully participating in Quick<br />

Confirm are California, Michigan, and Pennsylvania.<br />

Go to www.nursys.com as additional states/territories<br />

may be added.<br />

(C) Other states – Most states have licensee lookup/<br />

licensure verification on their websites. Links to other<br />

boards of nursing can be found at www.ncsbn.org.<br />

(D) You can check for discipline against a <strong>South</strong> <strong>Carolina</strong><br />

nursing license on the Board’s website www.llr.sc.gov/<br />

nurse under BOARD ORDERS.<br />

HAVE YOU MOVED?<br />

Section 40-33-38(C) of the <strong>South</strong> <strong>Carolina</strong> Code of<br />

Laws (<strong>Nurse</strong> Practice Act) requires that all licensees<br />

notify the Board in writing within 15 days of any address<br />

change. In order to not miss important time-sensitive<br />

information, such as an audit notice, legislative updates<br />

or other licensure information, be sure to notify the Board<br />

immediately whenever you change addresses. You may<br />

change your address on the Board of Nursing’s website<br />

(www.llr.sc.gov/nurse). Choose “Change of Address via<br />

Online Portal” under QUICK LINKS.<br />

NOTE: Changing your address with the <strong>South</strong> <strong>Carolina</strong><br />

<strong>Nurse</strong>s Association (SCNA) does not change your address<br />

on your licensing records with the <strong>South</strong> <strong>Carolina</strong> Board<br />

of Nursing.<br />

HOW TO CHANGE YOUR PRIMARY STATE OF<br />

RESIDENCE<br />

In accordance with §40-33-1350 of the S.C. Code<br />

of Laws, Compact rules and regulations will require<br />

each nurse to declare in writing his/her primary state of<br />

residence upon initial application and renewal of the<br />

nursing license.<br />

If at the time of licensure, the primary state of<br />

residence was listed as a non-compact state (example:<br />

WA, OR, NV, CA, OH, NY, etc.) and/or the prospective<br />

licensee has not obtained a SC driver’s license, an<br />

active single-state license in <strong>South</strong> <strong>Carolina</strong> was issued.<br />

If the primary state of residence later changes to <strong>South</strong><br />

<strong>Carolina</strong>, a multi-state license may be obtained by<br />

submitting a Declaration of Primary State of Residence<br />

(PSOR) form available on the Board of Nursing’s website<br />

(www.llr.sc.gov/nurse) under LICENSURE, “Applications<br />

and Forms, under Miscellaneous Forms”.<br />

The completed PSOR form should be sent to<br />

<strong>Nurse</strong>Board@llr.sc.gov with one of the following items as<br />

evidence of <strong>South</strong> <strong>Carolina</strong> residency: 1) <strong>South</strong> <strong>Carolina</strong><br />

driver's license with a home address; 2) <strong>South</strong> <strong>Carolina</strong><br />

Voter Registration card displaying a home address; 3)<br />

Federal income tax return declaring the primary state of<br />

residence as <strong>South</strong> <strong>Carolina</strong>; 4) Military Form # 2058 -<br />

state of legal residence certificate. The Compact primary<br />

residence rule does not apply to military nurses or nurses<br />

in the federal government, unless they are working<br />

outside of their military or government position.<br />

It is important to ensure that you receive information<br />

sent out via e-mail and for renewal of licensure. To change<br />

your email address, please email <strong>Nurse</strong>Board@llr.sc.gov<br />

with the full name on your license, your license number,<br />

your current email and your new email address. You may<br />

also change your email address on the Board of Nursing’s<br />

website (www.llr.sc.gov/nurse). Choose “Change of<br />

CHANGING YOUR NAME ON YOUR NURSING<br />

LICENSE<br />

1. If you have had a legal name change, submit<br />

your written request, along with a copy of the<br />

legal document(s) (example: copy of marriage<br />

certificate, divorce decree, court order, etc.) and<br />

photo ID to LLR-Board of Nursing, PO Box 12367,<br />

Columbia, SC 29211 or you can send the request<br />

and documents by email to <strong>Nurse</strong>Board@llr.sc.gov.<br />

Please indicate in your request whether you will use<br />

your middle name or maiden name for your middle<br />

initial or if you wish to hyphenate your name. Your<br />

request will be processed in the order received<br />

and updated on the website within 24 hours of the<br />

change.<br />

2. You may verify that your name change request<br />

has been processed on LICENSEE LOOKUP on<br />

the Board’s website (www.llr.sc.gov/nurse). Refer<br />

to Section 40-33-36(B) of the <strong>Nurse</strong> Practice Act<br />

regarding statutory requirements for your name on<br />

SCAM ALERT<br />

Scammers are targeting nurses, including here in<br />

<strong>South</strong> <strong>Carolina</strong>, by sending them official looking letters<br />

from Nursing Regulatory Bodies (NRBs) or other State/<br />

Federal agencies. The scammers know the business<br />

of the agencies and NRBs and are connecting with<br />

the nurses using messaging such as “Your license is<br />

suspended or will be revoked unless you connect back<br />

with us and transfer funds to fulfill the financial obligation<br />

related to this inquiry…”<br />

1. If you receive a phone call, do not give out your<br />

personal information during the call. Hang up, and do<br />

not call the number back unless you have determined,<br />

through research, that it is legitimate.<br />

2. Do not reply to a text message asking for you to<br />

call the number provided in the text to discuss your<br />

imminent “suspension or revocation of your license.”<br />

3. Do not reply to an email asking for personal<br />

information.<br />

4. Never call a number provided to you in a suspicious<br />

letter or email.<br />

5. Use your own verified numbers for contacting the<br />

Board, Agency or NRB.<br />

Be vigilant! If you receive a suspected scam call, email,<br />

letter, etc., contact the Board or Agency directly.<br />

BOARD VACANCIES<br />

There currently are three vacancies on the <strong>South</strong><br />

<strong>Carolina</strong> Board of Nursing. Members serve terms of four<br />

years and until their successors are appointed. Board<br />

members are appointed by the Governor with the advice<br />

and consent of the Senate.<br />

Pursuant to Section 40-33-10(A) of the <strong>Nurse</strong> Practice<br />

Act, when appointing members to the Board of Nursing,<br />

the Governor will give consideration to include a diverse<br />

representation of principal areas of nursing not limited<br />

to hospital, acute care, advanced practice, community<br />

health and nursing education. Registered nurse and<br />

licensed practical nurse members must be licensed<br />

in <strong>South</strong> <strong>Carolina</strong>, must be employed in nursing, must<br />

have at least three years of practice in their respective<br />

professions immediately preceding their appointment and<br />

must reside in the district they represent. If you are not<br />

sure of your congressional district, you may check your<br />

district at scvotes.org.<br />

There is one vacancy on the Board of Nursing for a<br />

Registered <strong>Nurse</strong> representative from Congressional<br />

District 6, one vacancy for a Licensed Practical <strong>Nurse</strong><br />

from the state at large, and one vacancy for a public<br />

member. No Board member may serve as an officer of a<br />

professional organization.<br />
