Community Health Centers Inc. is a one-stop shop for medical and dental<br />
care for families. With 16 centers throughout Central Florida, CHC provides<br />
quality and compassionate health care services for children and adults. Its<br />
centers offer pediatrics, family medicine, obstetrics/gynecology, optometry,<br />
internal medicine, dental services for children and adults, behavioral health, a<br />
pharmacy and an on-site lab. Services vary by location, so visit www.chcfl.org<br />

directions. CHC offers expanded hours Monday and Tuesday evenings and<br />
even offers Saturday hours at its Apopka and Winter Garden centers. CHC<br />
accepts Medicaid, Medicare and most private insurance plans. A sliding discount<br />
program, based on family size and income, is available for those with<br />
or without insurance. Community Health Centers is proud to be a Federally<br />
Qualified Health Center organization that began its operations in 1972 in<br />
Apopka. CHC has achieved accreditation as a medical and dental home by<br />
the prestigious Accreditation Association for Ambulatory Health Care.<br />

Rizwan Khan, D.O., FACOI, is a board-certified internist, nephrologist and<br />
University of Central Florida alumnus. He also attended Lake Erie College of<br />
Osteopathic Medicine, where he earned a doctorate in osteopathic medicine.<br />
He currently holds medical licenses in the states of Florida and Oklahoma.<br />
Dr. Khan is a member of many organizations, including the American Society<br />
of Nephrology, American College of Physicians, American College of<br />
Osteopathic Internists, Florida Osteopathic Medical Association, American<br />
Osteopathic Association and Renal Physicians Association. Throughout his<br />
career, he has been published in multiple peer-reviewed publications.<br />

Adnan Ahmed, M.D., FACP, is a former assistant professor of medicine at<br />
the University of Mississippi Medical Center. He is a specialist in clinical hypertension<br />
as designated by the American Society of Hypertension. Dr. Ahmed<br />
graduated from the University of Karachi and Dow Medical College, earning<br />
