Completion instructions for the report<br />

Report on disturbances in banking operations<br />

General instructions<br />

This report is a detailed narrative report on :<br />

• all disturbances resulting in any delay of at least two hours in executing payment<br />

instructions of clients (e.g. bank cards system breakdown, delays in or failure of<br />

transmissions to ICS);<br />

• all disturbances resulting in any hang-up/delay of at least half an hour in processing<br />

payment instructions sent to VIBER, or already settled in VIBER as compared to<br />

normal business operations;<br />

• the use of the facility defined as “Extraordinary Sending Cycle” in the standard<br />

service agreement of ICS;<br />

• the background for any extension of VIBER business hours, as approved by <strong>MNB</strong><br />

(e.g. internal processes, systems affected, the reason for request), and<br />

• on the breakdown of VIBER monitor service of more than half an hour, if the data<br />

supplier uses the VIBER-monitor service of <strong>MNB</strong>,<br />

In the case of a request for the extension of business hours approved by the <strong>MNB</strong>, the<br />

reporting obligation applies to all applicants. When a credit institution operating a<br />

clearing and settlement system requests, on behalf of a member of its own system, that<br />

the <strong>MNB</strong> extends the VIBER business hours, the credit institution operating the clearing<br />

and settlement system shall be responsible for submitting the report.<br />

If the credit institution operating a clearing and settlement system is also obliged to<br />

submit data supplies W08 and W11 with respect to the disturbance specified above, it<br />

will suffice to submit report W08.<br />

The following events are not to be reported:<br />

• any delay attributed to a VIBER crisis situation resulting from a problem at <strong>MNB</strong>,<br />

• any loss of VIBER-monitor service resulting from a problem at the <strong>MNB</strong>,<br />

• any delay attributed to an ICS crisis situation resulting from a problem at GIRO Zrt.<br />

or the <strong>MNB</strong>,<br />

• pre-arranged suspension of business about which clients are notified in advance.<br />

The following information shall be contained in the report:<br />

• name and address of the data supplier, name and phone number of the person<br />

completing the data reports, name of her organisational unit<br />

• services, banking processes and systems affected by the disturbance,<br />

• type, aggregate volume and value of affected transactions,<br />

• starting date and time of the disturbance (hour:minute),<br />

• information on how the disturbance was discovered, the starting date and time of<br />

the disturbance (hour:minute),<br />

• date and time of the elimination of the disturbance (hour:minute), provide details if<br />

the elimination required several stages and the process of the elimination was<br />

gradual<br />

• duration of the hang-up/delay (hour:minute),<br />

• reason for the disturbance. if not known, action taken to investigate the cause of the<br />

problem,<br />

• impact on availability for customers,<br />

• number of complaints received already from customers,<br />

• any other consequences,<br />
