PITIRIASIS LIQUENOIDE
Aguda y crónica
PLEVA y PLC
Características fundamentales
PLEVA y PLC son dos extremos de la misma
enfermedad
Brotes recurrentes de pápulas eritematosas a
purpúricas evanescentes
PLEVA:vesículas, pústulas y costras

PLC: descamativas
Cuadros intermedios o mixtos
AP: dermatitis superficial con queratinocitos
necróticos
Infiltrado LT generalmente monoclonales
Historia
Mucha y Habermann describieron
PLEVA
Juliusberg describió la PLC
En 1902 en el tratado de Brocq
fueron incluídas en un grupo de
dermatosis crónicas idiopáticas
“parapsoriasis”
Mayor frecuencia en niños y sexo
masculino
Patogenia: VIH, QT, ACO
PLEVA: CD8+
PLC: CD4+
Presencia de un clon dominante de
LT
Relación con la papulosis
linfomatoide, LCCT, enfermedad de
Hodking, otros..

ENFERMEDAD LINFOPROLIFERATIVA
PLEVA
Costras,úlceras, vesículas o
pústulas y cicatrización varioliforme.
Lesiones asintomáticas, limitado a
piel
Fiebre, adenomegalias, artritis,
bacteriemia, esclerodermia.
Cursa en semanas
ed papules, some with central crusts,
healing with hyperpigmentation
Comments:An 18 month old boy developed
an asymptomatic widespread red papular
eruption 6 months earlier. Lesions recurred
episodically but never completely cleared. He
was initially diagnosed with varicella and then
pityriasis rosea.
widespread asymptomatic red papules which
evolved into crusted necrotic papules and
healed with post-inflammatory
hypopigmentation. Initially he was diagnosed
with pityriasis rosea, but when the lesions
continured to recur for over a year a biopsy
was obtained which confirmed the suspicion of
PLEVA. He continues to be well without signs
of systemic disease
Body Site:hand / wrist Age:3 years Comments:A healthy
toddler developed a disseminated asymptomatic red papular
eruption with central scale, necrosis and crusting in some
lesions healing with hyperpigmentation. The palms and soles
were involved. Examination revealed no adenopathy or
organomegaly and a skin biopsy from a red papule showed a
benign superficial dermal lymphocytic infiltrate with
dyskeratosis, spongiosis and some hemorrhage.
PLC
Pápulas indolentes descamativas,
rosada a marrones
Cura en semanas o meses
Máculas hipopigmentadas residuales
Distribución difusa:1 año
Distribución central: 2 años
Distribución periférica:3 años
