SERVICE DES MALADIES INFECTIEUSES DEPARTEMENT DE MEDECINE INTERNE SYSTEME NERVEUX Méningite bactérienne 65 ans, immunosuppression, ou +OH Méningite nosocomiale Post-neurochirurgie / shunt Abcès cérébral Durée (j) *iv/po/total SWITCH IV/(im) ORAL Remarques particulières 14/0/14 ceftriaxone 2 g/12h Avant ou avec la dose initiale d’antibiotique administrer en perfusion lente*. Dexaméthasone 0.15 mg/kg/6h pour 4 j à commencer avant ou avec la première dose d’antibiotique 14-21/0/14-21 ceftriaxone 2 g/12h et amoxicilline 2 g/4h 14-21/0/14-21 céfépime 2 g/8h ou céftazidime 2 g/8h et vancomycine 1 g/12h >21/0/>21 ceftriaxone 2 g/12h et métronidazole 500 mg/6h ou méropénem 2 g/8h En cas de listeriose confirmée (L. monocytogenes), remplacer céftriaxone par gentamicine dose de charge 2mg/kg, suivie de 1.5 mg/kg/8h Enlever le shunt et adapter selon microbiologie Traitement intraventriculaire avec vancomycine et/ou gentamicine via drain intra-ventriculaire : à discuter avec <strong>Maladies</strong> Infectieuses Si possible ponction de l’abcès, Hémocultures Durée de traitement : discuter avec <strong>Maladies</strong> Infectieuses * En cas d'urgence, antibiotiques + hémocultures, puis ponction lombaire 21
