Nel presente lavoro viene descritto il caso di una paziente anziana giunta allanostra osservazione, nella quale era presente l’associazione tra dermatomiosite e liposarcoma retro-peritoneale; lasingolarità del quadro clinico, non segnalato in precedenza, è stato arricchito da una breve, ma esaustiva, reviewdella letteratura.La sintomatologia di presentazione della dermatomiosite, quando vi è associata una neoplasia, può talora esseresubdola e trarre in inganno anche clinici molto esperti. Il sospetto diagnostico di tale patologia o comunque di unaconnettivite secondaria ad una forma neoplastica, va sempre preso in considerazione, quando di fronte al soggettoin età avanzata, vi è una presentazione improvvisa senza sintomi prodromici e la presenza di un’elevata velocitàdi eritrosedimentazione.Parole chiave: sindromi paraneoplastiche, dermatomiositi, liposarcoma, autoimmunità, tipi di cancro.Summary: The paraneoplastic syndromes are a real problem for the clinical diagnosis in everyday practice of hospital wards.This paper describes the case of an elderly patient came to our observation, in which there was an association between derma -tomyositis and retroperitoneal liposarcoma, and the singularity of the clinical picture, not reported previously, has been enri -ched by a brief but exhaustive review of the literature.The presenting symptoms of dermatomyositis when there is an associated neoplasia, can sometimes be deceptive and mislea -ding also very experienced clinicians. Colpisce prevalentemente il sesso femminile(2:1), in tutte le età, ma predilige l’infanziae l’età media. Ha un’incidenza compresa tra 2 e 10nuovi casi per milione di abitanti per anno con unap revalenza di 4 su 100.000 abitanti (1).L’ipotesi patogenetica di maggior credito, allaluce delle attuali conoscenze scientifiche, prevedeuna predisposizione genetica (HLA B7, DRw6 negliafro-americani e B8, DR3, DRw52 nella razzacaucasica) sulla quale un fattore scatenante puòinnescare una reazione immunitaria con successivodanno muscolare (2). Virus quali echovirus,coxachie, influenza, epatite B, Herpes, rosolia,Epstein Barr, HIV, vaccini e la penicillamina sonostati annoverati in vari studi scientifici come possibilifattori esterni possibili “trigger” agenti su lasuddetta predisposizione genetica (3,4). Al contrarioquando tale patologia colpisce soggetti con etàsuperiore a 60 anni si è notata un’associazione convari tipi di neoplasie in circa il 15-25% dei casi (5)con predilezione per il sesso maschile.Non sembra esistere una stretta correlazionetemporale tra le due manifestazioni: la neoplasiapuò precedere, concorrere o seguire l’episodiodella miosite come fenomeno paraneoplastico; siconsidera come limite per la comparsa dei sintomiclinici del tumore, i due o tre anni che seguono oprecedono la diagnosi di miosite.Il quadro clinico con il quale si presenta la DMè caratterizzato da astenia con esordio spesso subdolocoinvolgente la muscolatura pro s s i m a l edegli arti e del cingolo scapolo-omerale progressivamenteubiquitaria sino ad interessare anche imuscoli del collo, della fonazione, masticatori edella deglutizione. Per meglio indagare sul sintomoastenia è utile chiedere al paziente se negli
