Evaluation of the effect of parenteral esomeprozole on analytical phase ınteraction and
diagnostic test results

AIM: Although monoclonal antibodies used in immunoassay analyzes are specifically produced for
analytes, there is interaction potential between xenobiotics and reagent antibodies. Clinical
reflection of this interaction in the analytical phase is the reason of false high or false low test
results. In immunoassay method, affected detector antibodies in the reagent solution may cause
false test results that are not noticed through clinicians in health services. The aim of this study is
to investigate the interaction of parenteral esomeprozole.
METHOD: The study was performed using the chemiluminescence immmunassay method
(Siemnes, Centaur XP, Germany) with hormone control material (BioRad, lot: 40333). 1800 µL of
the control solution was taken to the godet and 200 µL of esomeprozole was added in intravenous
form and vortexed and incubated for 20 minutes at room temperature. TSH, FT4, FT3, Mass
CKMB, Troponin-I, CA15-3, CA19-9 CA125, CEA, PSA, Ferritin, B12, PTH, FSH, LH, Progesterone,
Estradiol samples were studied from the control sample. The study was rerun by adding 200 µL
distilled water. Percentage deviations from target value were calculated with bias.
RESULTS: Bias values due to esomeprozole were TSH 9.52%,FT4 46.03%,FT3 -12.01%,CK-MB
mass 2.05%,Troponin-I 0.74%,CA15-3 -1.28%,CA 19-9 0.13%,CA125% -6.42%,CEA 1.99%,PSA
-5.59%,Ferritin -3.77%,B12 -12.07%, PTH 1.53% FSH was affected by -0.68%, LH -
7.08%,Progesterone 3.75%,Estradiol-11.62%.
CONCLUSION: One of the disadvantages of the immunochemical methods is the interaction with
xenobiotics and encountering erroneous results.Esomeprozole caused significant interference in
TSH, FT4, FT3, B12 and estradiol tests. Esomeprozole interference should be evaluated especially
in cases where vitamin B12 deficiency due to antral gastritis is considered.
