leniu płuc oraz przeciwko wątrobowemu GVHD. Pacjent
spełniał kryteria diagnostyczne idiopatycznego
zapalenia płuc: objawy zapalenia płuc, zmiany radiologiczne
w płucach, nieobecność czynnika etiologicznego
w dolnych drogach oddechowych [20]. W leczeniu
początkowo zastosowano kortykosteroidy, jednak ze
względu na brak efektu włączono eksperymentalną
terapię antycytokinową z użyciem TNFR2 (etanercept),
co wynikało z przesłanek o potencjalnej roli
TNFα w rozwoju idiopatycznego zapalenia płuc [2].
Dodatkowym czynnikiem przemawiającym za zastosowaniem
TNFR2 były objawy wątrobowej postaci
GVHD. Łącznie podano 8 dawek etanerceptu, uzyskując
znaczną poprawę obrazu płuc oraz całkowite ustąpienie
zmian w wątrobie. Częściowo, również stosowana
hemodializa mogła wpłynąć na polepszenie się
stanu płuc [29]. Przedstawiony przypadek pokazuje, że
etanercept miał korzystne działanie w leczeniu idiopatycznego
zapalenia płuc u pacjenta po przeszczepieniu
allogenicznych komórek hematopoetycznych.
