A. Miliauskaite et al.: Handinstrumente versus Vector
öffentlichten Daten nach nicht-chirurgischer Parodontaltherapie
mit Hand-, Schall- und Ultraschallinstrumenten [12,
In diesen
Studien wird eine Reduktion der mittleren Sondierungstiefen
zwischen 1,2 mm und 1,8 mm, in initial 4 bis 6,5 mm
tiefen Taschen, gezeigt.
In tiefen Taschen (ab 7 mm) konnte eine mittlere Reduktion
der Sondierungstiefe von 2,3 mm erreicht werden [9].
An einwurzeligen Zähnen führte die nicht chirurgische
Parodontaltherapie mit dem Vector-System zu ähnlichen klinischen
Ergebnissen wie die konventionelle Therapie mit
Handinstrumenten [4, 6, 11].
Weitere kontrollierte klinische
Studien, insbesondere Langzeitstudien, sind notwendig
und bleiben abzuwarten.
5 Schlussfolgerungen
Aus der vorliegenden Studie können folgende Schlussfolgerungen
gezogen werden:
a) die nicht-chirurgische Parodontaltherapie sowohl mit dem
Vector-System als auch mit konventionellen Handinstrumenten
(Gracey-Küretten) führt an mehrwurzeligen Zähnen
zu signifikanten Reduktionen der mittleren Sondierungstiefen
und zu signifikantem Gewinn an mittlerem
klinischen Attachment.
depth and the improvement of the clinical attachment level
mainly depend on the initial pocket depths. There, the mean
probing depth of pockets with an initial depth of 4 to 6.5 mm
was reduced between 1.2 and 1.8 mm.
In deeper pockets (7 mm and more) the mean probing
depth was reduced by 2.3 mm [9].
Nonsurgical treatment with the Vector system in singlerooted
teeth showed similar clinical results as the conventional
therapy with hand instruments [4, 6, 11].
Results of in-vitro studies also indicate that the Vector
system provides a more substance-saving treatment than
hand instruments or conventional ultrasonic devices [5, 2, 8,
3].
Furthermore, patients treated with the Vector system stated
this method significantly less uncomfortable than treatment
with conventional methods [1]. Other authors reported
that local anesthesia was necessary in only a few cases when
nonsurgical periodontal therapy was performed with the Vector
system [7].
However, at the time being no long-term results are available
after nonsurgical periodontal therapy with the Vector
system. Thus, it is, therefore, impossible to definitely assess
the effectiveness of the Vector system, especially if used in
multirooted teeth. Further controlled clinical studies, in particular
regarding long-term results, are necessary.
5 Conclusion
This study allows to draw the following conclusions:
Nonsurgical periodontal therapy with the Vector system
as well as with conventional hand instruments (Gracey curettes)
leads in multirooted teeth to a significant reduction of
the mean probing depth and to a significant improvement of
the mean clinical attachment level.
Between the two methods there were no statistically significant
differences of any of the parameters evaluated.
