INDIAN REGISTRY ADMINISTRATOR:<br />

Starting this new fiscal year, I (Debra A. Smith) will be <strong>ch</strong>arging $10 for adults and $5 for<br />

<strong>ch</strong>ildren/youth, to do status cards. These are the following items I need when I do status<br />

cards, birth events or band transfers.<br />

WHAT IS REQUIRED TO OBTAIN A CERTIFICATE OF INDIAN STATUS<br />

(STATUS CARD)?<br />

One MUST be a registered <strong>In</strong>dian under the <strong>In</strong>dian Act before a status card can be issued.<br />

WHAT DO I NEED TO BRING TO APPLY FOR A STATUS CARD?<br />

For the issuance of a status card you must present:<br />

A CURRENT I.D. – SIZED PHOTO (1” x 1 ½”)<br />

2 VALID PIECES OF THE FOLLOWING IDENTIFICATION:<br />

1. Driver’s License<br />

2. Birth Certificate<br />

3. Employee Identification Card<br />

4. Passport<br />

5. Provincial Identification Card<br />

6. Student Identification Card<br />

7. Baptismal Certificate<br />

8. Provincial Health Card<br />

9. Certificate of Marriage<br />

10. Certificate of Divorce<br />

11. Firearms License<br />

12. Guarantor’s Declaration<br />

ALL CERTIFICATES OF INDIAN STATUS CAN TAKE UP TO (1) WEEK TO PROCESS<br />

ABOSLUTELY NO PHOTOCOPIED<br />

IDENTIFICATION AND/OR<br />

SIN CARDS WILL BE ACCEPTED<br />

For Birth Events: I need the original birth certificate that has both parents’ names on it<br />

and both parents are to sign the parental consent form.<br />

For Band Transfers: I need a letter stating you would like to transfer from one band to<br />

another (listing both bands), along with your full name, band number you are with at the<br />

present time and your address, with copy of your identification.<br />
