Epiphora Overflow of tears due to obstruction of lacrymal duct. Epiphora is more of a<br />

cl<strong>in</strong>ical sign than an absolute diagnosis. This condition constitutes <strong>in</strong>sufficient<br />

dra<strong>in</strong>age of the tear film from the eyes.<br />

Be aware of the dist<strong>in</strong>ction between chronic and acute epiphora. Chronic<br />

epiphora results from long-stand<strong>in</strong>g or unremitt<strong>in</strong>g disorders, and presents a<br />

greater cl<strong>in</strong>ical challenge than acute epiphora. The acute variety most often<br />

results from irritative ocular conditions such as corneal foreign bodies or allergic<br />

conjunctivitis, and usually resolves with treatment of the associated disorder.<br />

Patients with chronic epiphora report excessive lacrimation, <strong>in</strong> some cases to<br />

the po<strong>in</strong>t of tears actually stream<strong>in</strong>g over the lid marg<strong>in</strong>s and down their face.<br />

The symptoms may be exacerbated by environmental factors such as excessive<br />

cold, w<strong>in</strong>d, pollen or other airborne particulate matter, sleep deprivation,<br />

nearpo<strong>in</strong>t stra<strong>in</strong>, or emotional stress. Regard<strong>in</strong>g the latter, some patients may<br />

report that they "cry very easily," or that they are constantly wip<strong>in</strong>g their eyes.<br />

Often, the patient compla<strong>in</strong>s of <strong>in</strong>termittently reduced acuity, ow<strong>in</strong>g to<br />

excessive tears. Irritation to the lids, and <strong>in</strong> particular the <strong>in</strong>ner canthus, is<br />

common because of the constant wett<strong>in</strong>g of that area as well as the cont<strong>in</strong>uous<br />

mechanical abrasion of tissues. Punctate epithelial keratopathy is another<br />

prevalent f<strong>in</strong>d<strong>in</strong>g <strong>in</strong> patients with epiphora. Lid-globe appositional abnormalities,<br />

punctal stenosis and lacrimal sac disorders may also be noted.<br />

Epithelioma Any tumour arised from epithelium.<br />

Epulis A k<strong>in</strong>d of fungoid growth on the gums.this orig<strong>in</strong>ates either <strong>in</strong> gum,periosteum<br />

or on the maxillary bone.when it beg<strong>in</strong>s <strong>in</strong> the gum or periosteum,there is a<br />

reddish or bluish red,hard and pa<strong>in</strong>less swell<strong>in</strong>g of a roundish or oval<br />

shape,grow<strong>in</strong>g out of alveolar process or between the teeth.it is somewhat<br />

movable if it rests upon a broad pedicle.<strong>in</strong> its further progress it assumes the<br />

form of an irregular,fleshy lump,which grow<strong>in</strong>g larger,overlaps one or more<br />

teeth;the adjo<strong>in</strong><strong>in</strong>g teeth are pressed out of their position,and grow crocked or<br />

fall out.the swell<strong>in</strong>g soon bursts through the mucus membrane and presents a<br />

tuberous greyish-red mass,which may be divided <strong>in</strong>to lobes by deep crevices.it<br />

bleeds easily and by slough<strong>in</strong>g,piece of it fall off,form<strong>in</strong>g cavities,which secretes<br />

a most offensive ichorous discharge.when t orig<strong>in</strong>ates from maxillary bone,there<br />

is a swell<strong>in</strong>g of the bone,teeth fall out and morbid growth burst out.<br />

Erethism An abnormal <strong>in</strong>crease <strong>in</strong> nervous irritability or state of excitement or<br />

irritation.an unusualy quick responcse to stimuli.<br />

Erotomania Abnormal, exaggerated sexual behaviour or reaction; preoccupation with<br />

sexuality.<br />

Eryhtema Erythema is characterised by a diff<strong>used</strong> redness of the sk<strong>in</strong>,which,under the<br />

pressure of the f<strong>in</strong>ger,disappears,and leaves not a white,but a yellowish<br />

spot,which at once grows red aga<strong>in</strong>.it gradually disappears and is followed by<br />

desquamation;it is always attended with more or less burn<strong>in</strong>g pa<strong>in</strong>.it is ca<strong>used</strong><br />

by exposure to heat,the rays of sun,and by different irritat<strong>in</strong>g substances.when<br />

it occures <strong>in</strong> children between the folds of the sk<strong>in</strong> around the neck,beh<strong>in</strong>d the<br />

ears,between the thighs etc or <strong>in</strong> fat women under the dependent breasts,and<br />

becomes raw it is called <strong>in</strong>tertrigo.it can also be seen <strong>in</strong> between buttocks.<br />

Erysipelas Rose;st.anthony’s fire,erysipelous faciei,ignis sacer;a contagious disease of the<br />

sk<strong>in</strong> and subcutaneous tissues due to <strong>in</strong>fection with streptococcus<br />

pyogenes,with redness and swell<strong>in</strong>g of affected area,constitutional<br />

symptoms,and sometimes vesicular and bullous lesions.<br />
