Signs and tests<br />
Diagnosis of tetanus is based on the history and physical f<strong>in</strong>d<strong>in</strong>gs that are<br />
characteristic of the disease. Diagnostic studies generally are of little value, as<br />
cultures of the wound site are negative for C. Tetani two-thirds of the time.<br />
When the culture is positive, it confirms the diagnosis of tetanus<br />
Tests that may be performed <strong>in</strong>clude the follow<strong>in</strong>g:<br />
Culture of the wound site (may be negative even if tetanus is present)<br />
Tetanus antibody test<br />
Other tests may be <strong>used</strong> to rule out men<strong>in</strong>gitis, rabies, strychn<strong>in</strong>e poison<strong>in</strong>g, or<br />
other diseases with similar symptoms.<br />
Treatment<br />
Control and reverse the tetany with antitox<strong>in</strong> (tetanus immune<br />
globul<strong>in</strong>).<br />
Give penicill<strong>in</strong> to kill C. Tetani (other antibiotics such as cl<strong>in</strong>damyc<strong>in</strong>,<br />
erythromyc<strong>in</strong>, or metronidazole can be <strong>used</strong> <strong>in</strong> patients who are allergic<br />
to penicill<strong>in</strong>).<br />
Remove and destroy the source of the tox<strong>in</strong> through surgical<br />
exploration and clean<strong>in</strong>g of the wound (debridement).<br />
Treat symptoms. Muscle spasms can be treated with muscle relaxants<br />
such as diazepam.<br />
Bedrest with a nonstimulat<strong>in</strong>g environment (dim light, reduced noise,<br />
and stable temperature) may be recommended.<br />
Sedation may be necessary to keep the affected person calm.<br />
Respiratory support with oxygen, endotracheal tube, and mechanical ventilation<br />
may be necessary.<br />
Expectations (prognosis)<br />
The death rate is high <strong>in</strong> children and <strong>in</strong> the elderly. Wounds on the head or face<br />
seem to be more dangerous than those on the body. If the person survives the<br />
acute illness, recovery is generally complete. Uncorrected episodes of hypoxia<br />
(lack of oxygen) ca<strong>used</strong> by muscle spasms <strong>in</strong> the throat may lead to irreversible<br />
bra<strong>in</strong> damage.<br />
