TICS OF HIV-POSITIVE WOMEN IN THE SELECTED HIGH-PREVALENCE AREAS OF<br />

THE RUSSIAN FEDERATION<br />

2.1. ST PETERSBURG<br />

2.1.1. HIV EPIDEMIC<br />

HIV cases have been registered in St Petersburg since 1987. It is am<strong>on</strong>g <strong>the</strong> regi<strong>on</strong>s with <strong>the</strong> highest<br />

prevalence in Russia. The total number <strong>of</strong> <strong>of</strong>ficially registered cases was 32,045 at <strong>the</strong> end <strong>of</strong> 2005.<br />

The cumulative number <strong>of</strong> HIV-positive people (prevalence 667.5 per 100,000) in St Petersburg is<br />

<strong>the</strong> highest in <strong>the</strong> RF North-West Regi<strong>on</strong> and exceeds <strong>the</strong> nati<strong>on</strong>al rate by 2.9 times.<br />

Several stages can be identified in <strong>the</strong> HIV epidemic in St Petersburg:<br />

1. 1987–1998 – slow <strong>spread</strong> <strong>of</strong> HIV, mainly sexual transmissi<strong>on</strong><br />

2. 1999–2001 – epidemic am<strong>on</strong>g IDUs, maximum incidence rate in 2001 (205.6 per 100,000)<br />

3. 2002–2006 – epidemic still in progress am<strong>on</strong>g IDUs with increasing sexual (heterosexual) transmissi<strong>on</strong><br />

According to WHO criteria, <strong>the</strong> epidemic in St Petersburg can be regarded as c<strong>on</strong>centrated. Sentinel<br />

surveillance data suggest HIV prevalence am<strong>on</strong>g IDUs is at least 30%, with <strong>the</strong> size <strong>of</strong> <strong>the</strong> IDU populati<strong>on</strong><br />

estimated at 100,000. Our sentinel surveys have shown that in 2003 HIV prevalence am<strong>on</strong>g<br />

SWs was 48.1%. The 2005 surveys show a high prevalence <strong>of</strong> drug use am<strong>on</strong>g homeless/aband<strong>on</strong>ed<br />

street-children, with HIV prevalence at 30.7%.<br />

In 2005, 4,045 new HIV cases were registered in St Petersburg, exceeding <strong>the</strong> 2004 figure by 356<br />

cases. The proporti<strong>on</strong> <strong>of</strong> parenteral transmissi<strong>on</strong> in 2005 rose to 67% (64.4% in 2004, 54.7% in 2003,<br />

71.9% in 2002, and 97.3% in 2001). The proporti<strong>on</strong> <strong>of</strong> heterosexual transmissi<strong>on</strong>, <strong>on</strong> <strong>the</strong> c<strong>on</strong>trary,<br />

fell to 7.1% in 2005 (15.6% in 2004). In 24.3% <strong>of</strong> cases in 2005 transmissi<strong>on</strong> routes could not be<br />

identified.<br />

Analysis <strong>of</strong> HIV cases by means <strong>of</strong> transmissi<strong>on</strong> showed that 76.4% cases am<strong>on</strong>g men and 47.3%; <strong>of</strong><br />

cases am<strong>on</strong>g <strong>women</strong> were c<strong>on</strong>tracted through injecting drugs. Heterosexual transmissi<strong>on</strong> accounted<br />

for 14.4% <strong>of</strong> cases am<strong>on</strong>g <strong>women</strong> but <strong>on</strong>ly 3.6% am<strong>on</strong>g men. The male-to-female ratio am<strong>on</strong>g<br />

people living with HIV was 2.1:1.<br />

The means <strong>of</strong> infecti<strong>on</strong> could not be identified with 18.6% <strong>of</strong> men and 36.6% <strong>of</strong> <strong>women</strong>, as <strong>the</strong>se<br />

patients had not been registered at <strong>the</strong> AIDS Centre.<br />

Age-distributi<strong>on</strong> <strong>of</strong> <strong>the</strong> HIV-positive populati<strong>on</strong>: 2.2% aged 15 or younger, 8.1% 16–19, 29.7%<br />

20–24, 32.1% 25–29, 14.5% 30–34, 6.2% 35–39, 3.4% 40–44, 1.9% 45–49, 1.8% over 50.<br />

A total <strong>of</strong> 1,990 children were born to HIV-positive mo<strong>the</strong>rs in St Petersburg between 1987 and 2005<br />

(908 c<strong>on</strong>firmed to be HIV-negative, 45 died, 940 were followed up). Am<strong>on</strong>g HIV-positive <strong>women</strong>,<br />

<strong>the</strong>re were 276 pregnancies and 415 deliveries registered in 2005 (Code 109). It is also noteworthy<br />
