RISK FACTORS IMPACTING ON THE SPREAD OF HIV AMONG PREGNANT WOMEN IN THE RUSSIAN FEDERATION<br />

About 64.5% <strong>of</strong> <strong>women</strong> took ART to prevent MTCT during pregnancy, 66.1% during delivery,<br />

about as many after delivery. Only 12.1% did not receive ART to prevent MTCT.<br />

1.5% <strong>of</strong> <strong>the</strong> <strong>women</strong> became <strong>pregnant</strong> after <strong>the</strong> diagnosis <strong>of</strong> HIV. For most <strong>women</strong>, pregnancy was<br />

planned and ended in delivery. A large proporti<strong>on</strong> <strong>of</strong> <strong>women</strong> after HIV diagnosis received ART at<br />

various stages <strong>of</strong> pregnancy, during and after delivery.<br />

More <strong>women</strong> in Irkutsk than in <strong>the</strong> general sample had received ART both before and after HIV<br />

diagnosis to prevent mo<strong>the</strong>r-to-child transmissi<strong>on</strong> 10 .<br />

2.5. TVER<br />

2.5.1. HIV EPIDEMIC<br />

Isolated HIV cases in <strong>the</strong> regi<strong>on</strong> have been registered since 1987. The epidemic am<strong>on</strong>g IDUs in Tver<br />

started in 1997, <strong>the</strong> year <strong>of</strong> maximum incidence (103.7 per 100,000), which dropped to 39.6 by 2005.<br />

A total <strong>of</strong> 4,561 cases were registered in <strong>the</strong> regi<strong>on</strong> at 1 January 2006, with 2,346 <strong>of</strong> <strong>the</strong>m in Tver.<br />

Regi<strong>on</strong>al prevalence totals 315.9 per 100,000 and exceeds <strong>the</strong> nati<strong>on</strong>al average by 1.4 times.<br />

Prevalence in Tver City is 576.0 per 100,000.<br />

As in <strong>the</strong> o<strong>the</strong>r Russian regi<strong>on</strong>s, it is <strong>the</strong> 21–30 age group which is most involved in <strong>the</strong> epidemic.<br />

The largest female group was aged 21–24 accounting for 30.9% <strong>of</strong> <strong>the</strong> sample. Am<strong>on</strong>g men, 46.1%<br />

bel<strong>on</strong>ged to <strong>the</strong> 25–30 age group.<br />

The proporti<strong>on</strong> <strong>of</strong> parenteral transmissi<strong>on</strong> in <strong>the</strong> regi<strong>on</strong> was 44.8% in 2005, compared to 72.5% in<br />

2001. Sexual transmissi<strong>on</strong> by 2005 had increased, reaching 50.4%, compared to 26.5% in 2001.<br />

The male-to-female ratio am<strong>on</strong>g all HIV cases at 1 January 2006 was 1:1.<br />

The proporti<strong>on</strong> <strong>of</strong> new cases am<strong>on</strong>g <strong>pregnant</strong> <strong>women</strong> (Code 109) in <strong>the</strong> regi<strong>on</strong> was 0.2% at 1 January<br />

2006.<br />

In 2005, approximately equal proporti<strong>on</strong>s <strong>of</strong> <strong>women</strong> ended pregnancy with childbirth as with aborti<strong>on</strong>;<br />

For <strong>the</strong> first time, a group emerged <strong>of</strong> HIV-positive <strong>pregnant</strong> <strong>women</strong> deciding to c<strong>on</strong>tinue <strong>the</strong>ir<br />

pregnancy and receive antenatal care at medical facilities.<br />

There were 479 children born to HIV-positive mo<strong>the</strong>rs in <strong>the</strong> regi<strong>on</strong> at 1 January 2006, with 239 <strong>of</strong><br />

<strong>the</strong>m in <strong>the</strong> city <strong>of</strong> Tver.<br />

In 2002, sentinel sero-epidemiological and behavioural surveys were c<strong>on</strong>ducted am<strong>on</strong>g IDUs in<br />

Tver. The prevalence rate am<strong>on</strong>g “street-based” IDUs was 55.4%.<br />
