RISK FACTORS IMPACTING ON THE SPREAD OF HIV AMONG PREGNANT WOMEN IN THE RUSSIAN FEDERATION<br />

SUMMARY<br />

Key c<strong>on</strong>cepts: human immunodeficiency virus (HIV), epidemic, <str<strong>on</strong>g>risk</str<strong>on</strong>g> groups, <str<strong>on</strong>g>risk</str<strong>on</strong>g>s, behavioural studies,<br />

sentinel surveillance, injecting drug users, sex workers (SW), <strong>pregnant</strong> <strong>women</strong>.<br />

This work focuses <strong>on</strong> HIV epidemics am<strong>on</strong>g <strong>women</strong> in five HIV high prevalence areas <strong>of</strong> <strong>the</strong> Russian<br />

Federati<strong>on</strong> (RF) (St Petersburg, Kaliningrad, Yekaterinburg, Irkutsk, and Tver).<br />

The survey’s target group c<strong>on</strong>sisted <strong>of</strong> HIV-positive <strong>pregnant</strong> <strong>women</strong> first registered as HIV-positive in<br />

July 2004 – July 2006. The total sample size was 758 pers<strong>on</strong>s. Participati<strong>on</strong> in <strong>the</strong> survey was voluntary<br />

and an<strong>on</strong>ymous. The completed questi<strong>on</strong>naires were statistically processed. The basic survey was a<br />

specially developed standardized questi<strong>on</strong>naire aimed at investigating behavioural <str<strong>on</strong>g>risk</str<strong>on</strong>g>s am<strong>on</strong>g <strong>women</strong><br />

before and after HIV diagnosis. A total <strong>of</strong> 111 questi<strong>on</strong>s formed <strong>the</strong> survey’s indicators.<br />

The investigati<strong>on</strong> <strong>of</strong> socio-demographic and behavioural characteristics showed that <strong>the</strong> surveyed<br />

sample can be classed in terms <strong>of</strong> HIV infecti<strong>on</strong> as a high-<str<strong>on</strong>g>risk</str<strong>on</strong>g> (IDUs, SWs, IDUs selling sexual services,<br />

sexual partners <strong>of</strong> HIV-positive pers<strong>on</strong>s) or low-<str<strong>on</strong>g>risk</str<strong>on</strong>g> group (IDUs’ sexual partners, pers<strong>on</strong>s<br />

engaging in sexual c<strong>on</strong>tacts with n<strong>on</strong>-regular partners, SW’s sexual partners). It was found that<br />

female vulnerability to HIV varied geographically: while in Saint Petersburg, Yekaterinburg and<br />

Kaliningrad HIV <str<strong>on</strong>g>risk</str<strong>on</strong>g>s were largely present in <strong>the</strong> behaviour <strong>of</strong> <strong>women</strong> at <strong>the</strong> moment <strong>of</strong> <strong>the</strong> diagnosis,<br />

in Irkutsk and Tver, a much lower number <strong>of</strong> <strong>women</strong> reported practicing <str<strong>on</strong>g>risk</str<strong>on</strong>g> behaviours. In all<br />

<strong>the</strong> cities, many <strong>of</strong> <strong>the</strong> resp<strong>on</strong>dents’ partners represented high and low-<str<strong>on</strong>g>risk</str<strong>on</strong>g> groups.<br />

Women tended to suggest that <strong>the</strong>y had been infected sexually. In additi<strong>on</strong>, <strong>women</strong> that were <strong>on</strong><br />

remissi<strong>on</strong> at <strong>the</strong> time <strong>of</strong> <strong>the</strong> diagnosis <strong>of</strong>ten did not think it necessary to report past drug use experience.<br />

This may have had an impact <strong>on</strong> <strong>the</strong> objectiveness <strong>of</strong> HIV transmissi<strong>on</strong> route assessment at primary<br />

epidemiological history collecti<strong>on</strong>. Some correlati<strong>on</strong> between <strong>the</strong> subjective assessment <strong>of</strong><br />

transmissi<strong>on</strong> route and behavioural <str<strong>on</strong>g>risk</str<strong>on</strong>g> <str<strong>on</strong>g>factors</str<strong>on</strong>g> was <strong>on</strong>ly present in <strong>the</strong> Saint Petersburg cohort (every<br />

fourth resp<strong>on</strong>dent regarded drug use as <strong>the</strong> cause <strong>of</strong> her infecti<strong>on</strong>). There is a link between generative<br />

behaviour and behavioural HIV <str<strong>on</strong>g>risk</str<strong>on</strong>g>s. Thus <strong>the</strong> proporti<strong>on</strong> <strong>of</strong> planned pregnancies as well as <strong>the</strong> number<br />

<strong>of</strong> patients forgoing antenatal care was reliably low in Kaliningrad and Saint Petersburg. In Saint<br />

Petersburg, up to <strong>on</strong>e half <strong>of</strong> <strong>the</strong> HIV cases in <strong>women</strong> were not diagnosed till delivery.<br />

After HIV diagnosis, <strong>the</strong> total number <strong>of</strong> resp<strong>on</strong>dent’s sexual partners droped, while c<strong>on</strong>dom use<br />

could ei<strong>the</strong>r increase or decrease. Much smaller number <strong>of</strong> <strong>women</strong> reported practicing drug injecti<strong>on</strong>:<br />

by two times in Irkutsk and Kaliningrad, by three times in Saint Petersburg and Tver and by<br />

seven times in Yekaterinburg. However, no significant changes in injecting equipment use could be<br />

identified.<br />

The analysis showed that <strong>the</strong> involvement <strong>of</strong> <strong>the</strong> social envir<strong>on</strong>ment <strong>of</strong> high-<str<strong>on</strong>g>risk</str<strong>on</strong>g> groups in <strong>the</strong> epidemic<br />

occurs at different paces. As <strong>the</strong> epidemiological assessment <strong>of</strong> <strong>the</strong> stages <strong>of</strong> <strong>the</strong> HIV epidemic<br />

is based <strong>on</strong> <strong>the</strong> evaluati<strong>on</strong> <strong>of</strong> behaviours both in <str<strong>on</strong>g>risk</str<strong>on</strong>g> groups and within <strong>the</strong> general populati<strong>on</strong>, it is<br />

advisable to m<strong>on</strong>itor <strong>the</strong> behaviours <strong>of</strong> <strong>the</strong> general populati<strong>on</strong> in <strong>the</strong> cities studied.<br />
