DUKE ANESTHESIOLOGY ALUMNUS | 15 Celebrating Thirty Years of Innovations in Monitoring and Informatics BY FRANK BLOCK, JR., MD, AND LARRY DOWELL, WITH ROSEMARY CUMBIE Monitoring is not merely gadgets. It also comprises displays, information, the patients who are monitored, and the people who use the monitors. The ultimate success of a monitoring system depends on each of these as a link in a chain. If one link is broken or even weakened, the entire system may fail or, worse yet, function suboptimally. In the development of monitoring devices, much attention is paid to the physics and physiology of the measurements and to the pharmacological needs of patients. The needs of users, who are even more diverse than patients, are more difficult to deal with. Rarely does the opportunity arise to introduce a complete monitoring system. A DAME monitoring cart. Under the leadership of the department’s first chairman, Dr. Merel Harmel, from 1972-1983, the Department of Anesthesiology designed, built, and maintained most of its own operating room patient monitoring equipment. Construction of a new hospital facility with its opening in 1980 provided the opportunity to design and test a new computer-based system, the Duke Automatic Monitoring Equipment (DAME) system. It relates an experience available nowhere else in the world. Indeed, it was the first anesthesia recordkeeping system installed in more than two ORs at Duke and one of the very first in the world. (The University of Miami had a system in two heart rooms.) The department’s engineering and maintenance staff comprised 15 people: three anesthesiologists, three electrical engineers with PhD’s, a computer programmer, a manager, a senior electronics technician, two junior monitoring technicians, a mechanical supervisor, two junior mechanical technicians, and an administrative support staff person.
